Hey there, Per people. My name is Jason.
I'm a registered polyomographic technologist, otherwise known as a sleep pimp. This is one of those things that a lot of people suffer from, and I don't know why I didn't do a video of it sooner. Um, have you ever heard the term wired and tired? Well, wired and tired is something when when you wake up in the middle of the night, typically people notice like, "Hey, I always wake up at 3:00 a.m., hey, I always wake up at 2 am."
One of my questions to them is, "Well, what time do you go to bed?" And usually, if you're saying like that 3:00 a.m. time, people are going to bed close to midnight, sometimes 10:30. Those are very specific times. So, what happens is you have uh REM. So your normal progression of sleep is stage one for a very brief period like 5 minutes at the absolute most and then you're going to transition into stage two stage three sleep for about another hour 20 minutesish and then at that 90 minute mark or hour and a half you're going to have a REM period. Um in these people this population that I'm talking about I'll show you an example here in a minute in those people you have severely fragmented REM sleep. Your non-REM might be bad also, but people generally wake up a lot easier during REM. And I'll tell you bit about that in a second, but you wake up during REM during the first REM period, back to sleep, and now you're another hour and a half out from there. So, if you went to bed at 10:30, uh you're looking at midnight is going to be your second one. U but basically, you're going to wake up again. You have that same thing, but you're so tired, you don't really generally remember it.
You don't really wake up. But during these periods, you're probably having little spooky dreams and you're and you're waking up a lot. You're getting a cortisol dump or adrenaline. Uh you're getting a little hyped up and hiy and you don't even know it. So then you come to your third REM period. So sometimes it's second REM period, sometimes it's third REM period. It's like always third REM period I would say is generally uh when people wake up out of it. But you have REM again really bad either mouth leaking or oxygen detach mouth leaking or sleep disordered breathing events such as obstructive sleep apnneas or obstructive hypopnas even with CPAP or B level uh or mouth leaking and it caused you to wake up during those REM periods and now you're quote unquote wired and tired. You're super sleepy but yet you can't fall back to sleep and this is a really common problem. So let me show you a little bit about what that looks like. Here we have Mr. Wired and Tired. Super high leaking. So, that's that's definitely part of it. But, this is REM breathing. Very easy stage to wake up out of. And that's going to be dreaming.
That's when you're going to be getting that cortisol dump into your bloodstream that look at this.
That keeps you awake. Wakes you up. And then you have this period of what the hell just happened right there. That's that whoa I dude I can't get back to sleep. And then you go back to sleep eventually. All right. Let's look at the next day. Let's see what happens. Ah, another one. Look at that. This is about two. Well, if we go from here.
So, sleep onset is roughly I'm going to call that 9:30 and they wake up at 11:40.
Pretty close-ish.
Is this REM?
You know, I'm going to be honest.
Actually, don't think that's REM.
Yeah, I don't I don't think that's it.
Looks like it's trying to be REM. I I don't feel comfortable saying it's REM.
Anyway, let's move on to the next one because that one not good. Okay, didn't sleep. Woke up again. Okay, so now severe mouth leaking that pops up right there. And we do have REM breathing right there. So, probably undertitrated during REM, but definitely mouth leaking during REM. Wakes up, cannot get back to sleep. Do you have this? Ah, another night. Look at that.
We have like four hoursish REM right under REM mouth leaking.
Doesn't look horrible during non-REM, but all of a sudden REM occurs.
Doesn't look I wouldn't say great before the the mouth leaking shows up, but mouth leaking just jacks it. He wakes up. Can't get back to bed.
Look at this. Night after night. Here's another one. Is this REM just prior to the awakening?
God, we have another embarrassing moment. Okay, we do have REM right here though. We're gonna count that. We're count that as a win. This might be REM.
I just it's not easy to tell because there's sleep disorder breathing occurring prior to that. This one, not enough sleep really. And then this one definitely we already looked at this definitely REM prior. So pretty much all of those examples there's some form of something occurring and REM is typically right there. So, it can be mask leak from a jaw drop leak with a full face mask. It can be a mouth leak using a nasal or nasal pillow mask. It can also be you're undertitrated. You're on a pressure that's not enough and possibly a mode of therapy that's not quite right, which kind of equates to the pressure is not right. So, uh or in this case, it can be both of them. Uh undertitrated, a pressure that's too low, and a mouth leak that occurs during REM. So, I would say probably a good indication of this is you're waking up a lot. Like this is without looking at any data whatsoever. If you're waking up a lot and you're waking up about the same times and you always seem to remember dreams that are very stressful, anxiety ladden dreams, that's really a pretty good indication that you need to take a look at your data and do something to resolve one of those problems.
