Testosterone replacement therapy (TRT) typically causes an average 10-30% reduction in testicular volume, commonly described as shrinking from walnut to grape size, but extreme shrinkage to 'pea size' is physically impossible due to the substantial tissue present; most patients do not notice significant changes and the condition is generally not a major concern.

Does TRT cause testicular atrophy and do you care?Added:
So, I did have another uh an interesting one. I got a got a Facebook message from someone to asking me He said he's looking to start testosterone. Um I think he was She's in his guy's in his 60s. Said that the biggest holdback is his wife got on the internet and read an article that said all guys on testosterone have testicles the size of peas.
>> Mhm.
>> And uh he was >> Cool.
>> He was He was certain that she was looking at some, you know, factual thing and this is really what's going to happen and everyone's testicles shrink to the size of peas. Um what kind of you know, what kind of average decrease in testicular volume should guys expect and you know, what's been your kind of feedback from from guys in general on that?
>> Well, if you have walnuts, okay?
Right? For like at a walnut size, right?
We're we're probably going to go from like a walnut to maybe like a plump grape.
Right? So, maybe 20% on average. That's what I tell my all my patients here. 20% on average is pretty is pretty average.
Can be more, can be less.
Some people don't notice any shrinkage at all.
>> Yeah.
>> Peas I think would be like I don't even think that's physically possible if you've had normal size testicles for them to go to peas. Like there's just far too much tissue down there.
As much as I hate to admit it, I've touched many balls in my lifetime from a prior lifespan.
>> [laughter] >> Unfortunately but uh >> Even I have seen an aggressive amount of penises and I'm I was not even in urology. That was just for me to care as >> when you go to career day, it was like, "Hey man, you want to look at dicks every day?" And they're like that guy wasn't there. So >> [laughter] >> He's busy looking at dicks.
>> Right. Probably.
So, that guy was notably absent from career day. So, I wanted to be a chef, but here we are. Um you know, here I am and and and and doing that.
But it's it's somebody's got to do it, right? Somebody's got to do it.
>> That's right.
>> So.
>> Yeah. No, yeah. Def- definitely definitely not that definitely not a that aggressive a shrinkage. And yeah, some guys don't notice much at all.
Yeah, somewhere between like 10 and you know, 10 20% average maybe maybe 30. I think some of the papers I saw it and said like up up to 30% or something like that. And uh but I think the bigger point there is most people just uh don't seem to care. No one gives a [ __ ] It's fine.
>> Out of the way more changes perspective for other people to see.
>> Yeah, just see if you
