Hello friends, this is Dr. RB Kushwaha, professor at SKUAST-Jammu, R.S. Pura.
Welcome to my YouTube channel and if you are new to my channel, so kindly subscribe the channel and hit the bell icon for further notifications. Today I am going to uh tell a story of a very unusual case of the urinary bladder rupture and how we managed that case, so we will discuss in this particular video. So, you could see in this video, this is a 5-years-old toy pom. 1 year back uh it developed a urolith in the urinary bladder, but surgical procedure was not performed at right time, so what happened? Uh there was a rupture of urinary bladder. So, rupture of urinary bladder is a very rare in dog, but in this particular case, there was a rupture of urinary bladder and whatever the calculi present in the urinary bladder, [clears throat] that was spread into the peritoneal cavity. So, the surgeon performed the laparotomy and removed the all calculi which are spread here and there in the peritoneal cavity and then performed the cystorrhaphy and also lavaged the abdominal cavity.
So, animal recovered from that episode and it started doing well, but within a 6-month the animal again developed a urolith in the urinary bladder.
So, when we took the radiograph, so we come to know that uh the different calculi present in the urinary bladder and also the one calculi which was probably left out during the recovery from the peritoneal cavity when the bladder was ruptured yeah during the first surgery. So, that was also visible on the radiograph. Since this operation was for the second time and the breed is very small, so size of urinary bladder was very small cause of the presence of the calculi in the urinary bladder that causes the cystitis also. So, overall there's is of the urinary bladder and side wall also reduced. So, when we repair but we going to told that the urine is not coming from the catheter and animal try to make the futile attempt for the urination. Then we suspect further there leakage of the urine. So, we perform the ultrasonography and then we come to know that there is a leakage of the urine from the urinary bladder. Then we open the abdomen again and then find the leakage site and then perform the cystography again and we also inserted the Royal feeding tube in a abdominal cavity for a drainage of the fluid. So, the animal did well for a two to three days.
Means the urine was coming from the urinary catheter but after three or four days then again the urine is stopped coming from the urinary catheter rather it is started coming from the catheter which we inserted for the drainage of the fluid from the peritoneal cavity.
Then we suspected again then there is a leakage of the urinary bladder. Since we have already performed the operation three times and beside our best effort there was a repeated leakage of the urinary bladder even we perform every time the omentalization over the urinary bladder but all the our best effort you can say not worked in this particular case. Then we discuss with the owner thoroughly and then we decided to do something different from the usual cystography.
So, the owner given the free hand that okay doctor so whatever you feel good you just do for the this animal and just try to save the animal. Then we open the this abdomen again. Now you could see here.
So, this is the incision site here. This operation we perform the fourth time.
So, what we did here so since uh there was a chemical peritonitis and when we opened for the fourth time, the size of urinary bladder was almost uh reduced to the half. So, instead of repair the urinary bladder, we decided to perform the urinary bladder muscle pyloromyotomy.
So, this one we formed the we repaired the bladder wall which is of the bladder with the muscles and skin. So, this is the bladder muscle pyloromyotomy.
>> So we could see here. So we performed this masculation 2 weeks ago. So now this case completely recovered and we also removed the this penis up to the hair. So we have done the subtotal penectomy and also performed the castrations. So in this particular case, so here the only one testis was present and the left side of the testis was in the abdominal cavity. So that testis was removed during the second time of operation. So now the animal is doing very good. You could see here. So this is the margin.
So now the urine is coming directly from the kidney to the ureter and then from ureter to the urinary bladder and then come directly outside. So only problem that there is no control over the urination, but this is the toy breed. So here the owner managed by just putting the diaper here.
There is a consumption of the diaper and daily one diaper. So third and fourth time we not use the absorbable suture material for our closure of abdomen. So the third time also we closed the visceral peritoneal cavity. We closed the abdominal cavity using the nylon in the figure of eight pattern and also the fourth time also since there was a peritonitis, so we not supposed to use the any absorbable suture material. So we performed the figure of eight suture pattern in a vertical plane to close the abdominal wall as well as the skin together. So now you could see here. So this is almost the healed completely. The animal is doing well. Only the problem is that there is no control over the urine because it's coming directly from the bladder to the outside. So there is no storage. So whatever the storage capacity of the bladder that has lost.
So but still the owner is very happy and animal is also doing very good. So this is the only you can say rather the story of this particular case which I wanted to share you.
That sometime when there is a the scene of the frustration is comes to you so you should not you can say we lose our you can say hope and even at one point we think of euthanasia of this particular case also.
But since we have discussed and since owner is very good so he told that we will manage by any means you just save this animal.
Although this is the salvage procedure so this is basically salvage procedure in sheep and goat but in this dog only problem that there is a chance of the ascending infection.
So we advise the owner for the routine checkup at you can say monthly interval.
