Facial feminization surgery (FFS) is a transformative procedure for trans women that typically involves multiple procedures performed in one surgery, with recovery varying by anatomical area—chin swelling takes 4-6 months, forehead sensation returns over 6 months, and overall results continue improving up to one year post-op. The forehead is the most impactful procedure for 95% of patients, significantly enhancing facial femininity. While initial recovery may be challenging, patients typically experience improved confidence, better breathing function, and reduced gender dysphoria, with the surgery primarily serving personal identity affirmation rather than external validation.

My Facial Feminization Journey [with Facialteam Part 5/5] | 1 Year update!Añadido:
Hi everyone, welcome back to the channel. My name is Alexis Blake. I'm a trans woman and I share my life and stories on social media. So, we're not at home today. Where are we? You might ask. We're back at Facial Team cuz it has been one whole year since I got my full facial feminization surgery. I had four procedures in one day. It was wild.
We'll have a look back in a moment.
We're going to have a post-op consultation with the surgeon and I'm here with Liam today because he's come with me.
>> Hey guys. So, um this time round is actually a lot nicer because I don't have anything to worry about. Alexis is having no more surgery cuz I spent about 14 hours on my own waiting for Alexis to come out of surgery and it was it was it was a it was a stressful time, but she was in good hands. But this time round we're just here to have the follow-up consultation. So, we're really excited.
This feels so surreal being back here 1 year later. Like, my face is done. It's just weird. It just reminds me that I had all that surgery anyway. Started by taking some 1-year post-op FFS photos of all different angles which will then compare to my pre-op photos. Welcome back. Thank you so much. Thank you.
>> It's been a year already. I know. It's gone so fast.
>> went really fast cuz I vividly remember you being here.
>> literally feels like yesterday and then being here for 1 whole year and seeing that is Yeah, it's crazy. How has the year been?
Amazing. Like the the first few days were a little rough and then it just got better and better and better and then after a few months I got more and more confident. I would never wear my hair back. Now flick it up. Uh I would never go out with no makeup on. Now I go out with no makeup, minimal makeup on. Even when I used to wear makeup I wore it quite heavy and like lashes and a lot more and now this is it. So, to be able to do that it's You look amazing. Thank you. Thank you. Thank you. Thank you.
How was for example the swelling going?
Fine. Everything's fine there. Um I'd say the chin, this here. I don't know if it was from the chin um Yeah. sliding genioplasty or if it was the trachea. Probably the chin.
Yeah, that was the slowest to go.
Um I have lost a couple of stone between the pictures as well. So, I think that's also molding my face [clears throat] a little bit. Um so, that that helped with the chin, but yeah, I'd say the chin took about 4 to 6 months to really go down.
>> Yeah, for some people it can take up to 1 year or even more. So, that's sagging Yeah, the little Yeah, the toughness, but now it's can barely feel anything.
>> And it will keep improving and it's the skin keeps regenerating and everything keeps getting collagen tension. Tighter. Tighter. But we want always tighter. Always, always, always.
And for example about the nose, the nose looks fantastic. How's the breathing?
How is the breathing?
>> I used to have a collapse where this is.
Yeah and um now it doesn't collapse anymore and I can sleep with my mouth closed and so that was a nice bonus I didn't expect from it.
>> There's always like a 90% aesthetic result on our [clears throat] rhinoplasties, but we always try to if we can since we're there improving the function a little bit that helps a lot.
>> Yeah, which it 100% did. The breathing's definitely better. Absolutely amazing.
Then the forehead. The forehead is usually pretty good nonetheless, but how is the sensation mobility? The the scar too? The scar's fine. I can't even find it. I was at the hairdresser's recently and they were trying to find it and they couldn't physically find it, which is great. The sensation took I'd say on top here took a while to come back like 6 months, but it wasn't uncomfortable. It was just it was just slightly numb and then I had like one nerve where I touched my eyebrow and I could feel it back here.
>> Yeah, that happens. It's weird, but that happens.
It's go it's nearly gone now. It's getting better and better and better, but I'd just be drawing on my eyebrow and I'd feel like this and I'm like, "Oh, what's that?" That's the nerves reconnecting and still send signal although they don't know which signal to send exactly. Yeah, so that was unusual, but it was it was fine really. I mean, the forehead was fine. Like that I think made the most difference. The forehead is the most important one for FFS for sure for 95% of the patients. It's difficult to explain when you are in the first consultation and you tell this is very important, but why?
Yeah, I mean like it's You look at the difference is much better than any explanation looking how it opens.
How brightened lifted and the weirdest thing was when after surgery like I looked at my eyebrows and they were in the wrong place on my face because I was like the bone had gone and they'd been lifted a little so they looked so weird.
So, I had to like change that was something I didn't expect. So, I had to change my eyebrows. Not that related to the surgery or so this question, but how has it affected in your work because you have people looking at your face all the time and probably commenting and I'm I'm sure you have a very positive community, but there's always we know how it works.
>> yeah. So, I'd say at first I did I didn't post like much of the results for a while cuz I wanted to like a reveal, dramatic reveal, which we did and people were like, "You look beautiful, but I can't see the changes." It's only when I did this was my face guys. Like look what I looked like and I did a before and after and then people were like, "Wow, it looks great." And a lot I got one of the most biggest comments I got was people saying, "I didn't understand why you wanted the surgery before you got it cuz you already looked feminine to me, but now you've had it I get it. I can see why you did it." And then I got some troll comments which was used to it and people just saying that I looked not unique anymore or like my face had changed too much or my nose was like everyone else's on Instagram. I was like, "That's what I wanted."
>> [laughter] >> Like they're like, "But your ski slope nose." I'm like, "That's what I specifically asked for. I didn't want straight. I wanted that little yeah." I didn't see what everyone else saw and I didn't see what she saw until after the surgery and after it healed and then I when I look back now when I look back at old video content it's quite jarring and then I'm like, "Oh my god." Cuz they say love is blind and I I was I was blind to what she had an issue with though. Like you know all the features I was blind to it cuz cuz I see her every day. So, when I look back now I yeah and I can now understand why if you want to look more feminine for yourself. It's always for you.
>> absolutely Yeah, it's not something that you need to do for the rest of the world. Well, sometimes it's for safety yes depending on your environment and everything. I get it, but in most of our cases it's something that you do for you and just for you. Literally. So, what someone said I don't get why you did it in one of the comments and I had an old clip from ages before that and it was me saying this surgery is just me. So, it doesn't need validation from you. You or you doesn't need validation from my husband, my mom. Like of course I want that, but it's it was to make me look in the mirror and be like, "That's who I've always meant to be." And that's 100% why I feel now. I wanted to ask this question because it's not often that I get to know that part of that point of [snorts] view which is very [clears throat] external.
So, I think it's also great to know this and know how it affects when a lot of people look at you. Yeah. I think people could see me happy and that's what was the most important thing. Like they could see that shift in there. Like I was instantly happier. Yeah. Fantastic.
Very happy to hear all that. Thank you.
I'm going to have to take a closer look.
I will try to not mess up in here. Okay, no you're fine. You're fine. the hair styles.
Let's see if I can find Okay. Okay. I have seen a few a few shorter hairs that are like that long now.
Not many. So, nothing that's noticeable.
So, I I know there must be something there, but I just I can't find it. If I'm not able to find it, I think it's the best job we have ever done ever. So, the first trick is to try to find it here and then follow. Oh, but yes it has healed fantastically because you can only see a pink a very pale pink line here.
It's almost imperceptible, but you you can see a little bit there.
And you keep following it.
But it is one of the best I have seen.
It's also clear I'm struggling to see.
Oh, oh no, I can see now. Yeah, very very fine. Very fine. Yeah. It's true that you have taken great care of it. It's just a mix of our job, your care and sometimes just genetics. You know, you're healing well. The color is fantastic. The thickness of the incision is minimal. Okay, so sensation is more or less back at least in all of the forehead and the scalp 99% right? Movement Yeah, I get Botox.
So, that's kind of a too much, but yeah.
You can see when some of the moment movement is impaired, but it is very rare nonetheless.
Yeah, the readaptation has happened flawlessly. Fine. Is it very tight still or do you feel more relaxed and better?
It feels fine.
>> good. Can you take a deep >> [snorts] >> breath? Inhale.
Perfect stability there. As good as it can be.
I know.
>> Flawless.
You are flawless.
I'm really happy with it with everything really. Do you know the weirdest thing that I didn't expect? It's not a bad thing, but like feeling my nose cuz obviously it's different. I'm like, "Okay, that's different, but that's that's fine." Feeling here, change fine.
Feeling this feels odd to me sometimes.
>> It it still feels pretty odd.
>> Like not in like a weird way. It just feels cuz I just I'm very like, "That's not my chin." Yeah, like feeling something on top of it or But it's not bad. It's just something like, "Oh, that feels different." But yeah. Usually we expect that to keep changing. If it's not perfect, sometimes it's not fully perfect, but it improves even after 1 year most of the times. I I guess that be the case.
All of the rest is a absolutely 11 out of 10. I can't believe it's been a year.
Yeah, this is crazy. Absolutely. One year consultation is now done. I'm as I've said very happy with the results.
As you can probably tell I'm always smiling and I'm feeling much more confident than I used to be. It did exactly what I needed it do for me.
