First I would like to congratulate department of KYA GJ Patel Institute of Vetic Studies and Research for organizing a national webinar on the occasion of World Liber Day that was celebrated on 19th of April every year to create awareness first amongst the students about the liver health the preventive measures and the diseases which are occurring where very frequently in the society. But first our students should be encouraged to uh take preventive measures about the liver disease.
So the topic for the today's webinar is uh metabolic dysfunction associated styotic limo disease that is MS2.
So this is the outline for the today's presentation. Significance of the day.
Introduction to the stadotic fever disease. Criteria for MSLDPathogenesis.
Clinical features. Investigations.
Management and its VA correlation along with the management.
First coming to the significance of the world liver day. European Association of the Study of the Liver that is EASL that is established in 1966 which is a body in liver education as well as in research. The CAS has officially establishes world day in 2010 annually at 19th of April as onethird of the global adult population is living with the condition MSLD and other steroidic liver disease. 2 million leaves are lost each year due to the liver disease and 1.5 million people are suffering from the chronic liver disease. So this is the response of the ESA to create awareness amongst the society to take preventive measures for the liver diseases.
This is the prevalence of the MSLD and other liver diseases. In future it has been forecasted that prevalence of MSN will reach up to 50 will reach up to 55% globally by the year 2014.
The theme for the this year 2026 world deliver day was solid habits strong liver which emphasizes the 90% of the liver diseases are preventable. So the preventive measures should be incorporated in the society. So the awareness should be done. The four bricks of this uh campaign are balanced plantbased diet and sh this contains the overall emphasis was given on the vegetarian diet containing whole grains, proteins and foods. Sharp reduction in ultrarocessed foods and hidden sugars which are the primary drivers for hippatic fat accumulation as thetosis emphasizes on the fat accumulation in the liver. Then the second brick was physical activity. Regular and moderate exercise like 30 minutes work help to burn the body fat more efficiently which can ultimately reduce the deeper fat.

So the regular blood test and fibro scan that are essential for the early prevention or early diagnosis of the liver diseases mostly in the patients suffering with comorbid conditions.
Roughly the liver diseases are classified as alcoholic fatty liver disease and non-alcoholic fatty liver diseases. Initially the term was NFT but in June 2023 the NFT has been replaced by MST that is metabolic dysfunction associated steroic liver disease as only alcohol is not a factor or conditions causing stattoic liver disease. The other metabolic dysfunctions are also responsible for the uh liver diseases.
So considering these other factors the term was replaced by MSH and the term NH that is non-alcoholic state of hepatitis that is replaced by MASH that is metabolic dysfunction associated stat of hepatitis. So what is metabolic dysfunction? Metabolic dysfunction is a abnormal chemical reaction that happens in the body which disturbs your metabolism that is your energy production regulation as well as storage and which results into the obesity insulin resistance hypertensions and other metabolic syndromes.
So coming to first we will go with what is steotic liver disease. As I said earlier stattosis means accumulation of fat. So these SLDs are associated with ALD that is alcohol associated liver disease that may be associated with secondary physiology maybe specific pathologies drug induced nutritional or genetics. Some are cryptogenic and no any identifiable cause has been shared.
Then metal that is metabolic and alcohol associated liver disease that means MSD along with alcohol associated liver disease.
Then MSD MS can be associated with one or more cardium metab factors such as overweight that is BMI more than or equal to 25 obesity in which BMI more than or equal to 30, blood pressure more than 130 by 85, triglycerides more than 150, plasma HDL less than 40 and fasting sugar glucose more than 100.
So any of the these things one one of the cardio metabolic risk factor is associated with MST.
So definition it is defined as a presence of hypatic steattosis with one or more cardometabolic risk factors with no other identifical cause of steosis.
The MSL the name itself suggest what is the Pathogenesis behind this image where M stands for metabolic dysfunction. This leads to obesity, insulin resistance and type 2 diabetes mal hypertension disease. Then stand for adequin inflammatory signals because of the oxidative stress released to hypatic injury and further may uh may be the healthy liver tissue by the fibroic treatments. Then stereosis and fibrosis that is fat accumulation and fibrosis linky gut and microbiota gut is pipiosis and disease progression and cardiovascular may further gives rise to atherosclerosis and cardiovascular mortality we are all aware about the risk factors that are obesity diabetes sleepmia, metabolic syndrome, hypertension, hypotarism, PCOD, obstructive sleep because of some drugs like methotric, acid, glucotic, some nutritional deficiencies, kinesia and genetic factors.
Here we can see other positive factors like fast food.
Then sedentary lifestyle, alcohol addiction and excess of stress.
So uh how what are the stages of MSD?
How the disease progresses step by step?
Initially a healthy liver which contains less than 5% of the fat fat in the liver cells but gradually the fat deposition start because of the ideological factors and that exceeds to more than 5% and leads to steosis that is again it progresses into the mash without fibrosis that is metabolic dysfunction associated stippatitis stereotis steosis along with the inflammation of the liver cells which leads to ballooning of the cells. Then it gives birth to fibrosis in the later stage that is diatrosis along with inflammation and fibrosis of grade 1 to three. Then it further progresses to cerosis and in the later stage it goes to the cellular carova.
Here is a small presentation for uh the understanding of the pathology of the disease because of the dietic factor that is physical in negativity. We know all the fast food type carbonated drinks and the other positive factors leads to weight gain. This weight gain or increased obesity and overweight leads to increased visceral adiposity.
Increased central and peripheral insulin resistance which further lead to type 2 diabetes and increase in the free fatty acids which can deposit into the liver.
Then in the liver also it increases the lipo genesis that means excessive production of lipids instead of their destruction. So the lips level also increases and then get inated at the site of the skin. Then because of the endopplasmic reticulum stress and ROS reactive oxygen spaces generation because of the oxidative stress it leads to the liver cell injury. If the copper cells get injured then that leads to release of cytoines inflammatory medals and further leading to hippatics and because of the states still activations that leads to excess production of fibros. The healing may take place with the help of fibers and leads to fibrosis of the Then coming to the clinical features most of the patients are asytomatic that's why the routine health checkup are very much important in the liver diseases. What are the complications of ASV? The major complication of the MSV is increased cardiovascular risk. It has been uh surveyed that most of the liver disease patients bleed to death because of the cardiovascular complication rather than the liver diseases because it increases the hyperagulability.
Lipid deposition leads to atherosclerosis of the vessels and endothelial damage. So it leads to How to evaluate the patient suffering from MS? First history taking about the anthropometric we know from the comorbid condition. If the patient is suffering from diabetes, menitis, hypertension or history of the medications, family history because genetic factor is also plays as important role in the liver disorders. So family history should be asked. Then use of alcohol this is the very much important point. Then coming to the physical examination. During physical examination we can observe mechanism spome we should evaluate the body fat distribution to or to know about that type of then features of insulin resistance can be evaluated of this test then laboratory test which states should be carried out is the first choice function test which shows The elevation of the AL and AS serum varing can be evaluated serum V alum from time that are usually normal except in the patients of cerosis. These factors should be evaluated like BMI diabetes malitis lipid profile thyroid function taste and tissue serum and betic ion levels to rule out hemogramosis.

So the treatment of MSAD has been divided into three parts that the specific treatment of MS treatment of MSLD associated with coorbid condition and the treatment of its obligations.
So the management emphasizes on the diet and exercise theapy, diabetes management, hypertension management, dysipidemia management, biatric surgery in the obese person and nea transplantation that is the last option.
So first diet and exercise as liver diseases are preventable. So we should make awareness in the society that diet and exercise are the very much important for the health. So reduce the sedentary lifestyle. Increase physical activities. Weight loss 3 to 5% can improve stereatosis and seven more than 7 to 10% of weight loss over 6 months can improve the stroitis.
low carbohydrates and low fat. Increase polyunsaturated fatty acids like omega3 and omega6 which are found in nuts and seeds like sunflower flax seeds. So increase the consumption of unsaturated fatty acids increase the consumption of fibers. Then intermittent fasting and that are also indicated in so intermittent fasting that can reduce the fat deposits in the then avoid alcohol regular exercise medications as the uh comorbid conditions are also treated along with the liver diseases. So the medications for the comorbid conditions like for diabetes for hypertension that should be continued along with the other medication then use of antioxidants use of polyunsaturated fatty acids and use of statins to treat the dispute.
Here I am not going in the details of the management as the management of comorbid conditions. We know the management about the different different diseases. So here I am not taking much time and emphasize on the management of that conditions.
Then lifestyle recommendation. This is the very much important for the people with MS. People with MSD those are overweight or obese they should be reduce their body weight more than or increase 5%. Reduce the fat then reduction is 7 to 10% improve liver inflammation and reduction of more than 10% improve the fibrosis. Then beriatric surgery for the patients who have suffered a obesity.
Diet as we know and previously we have seen the consumption of fruits, vegetables, nuts, olive oil, unprocessed poultry and fish. Limit the consumption of processed food and saturated fats. Avoid sugar and increase daily physical activity.
So from the ministry of Aayush they have released some dos and don'ts on the day of world yoga day. So in the D they have indicated the use of adraa leaf and fruit of unar like amalaki daring daksha papaya uh nuclear then leaves of that and of the don't avoid fat reach heavy dudestide contamination excessive redemption of alcohol and excessive exercise.

So coming to the second last slide as the word liver day emphasizes on solid habits for strong liver small challenges today strong liver tomorrow. So they have given some guidelines to eat healthy, stay active, never help, get vaccinated as heis B and he C that can be the ideological factors for many. So timely vaccination of hepatitis B is very much important. Maintain healthy bed to reduce the chances of obesity.
