[00:00:00]I recently cut an apesiotomy for one of my patients and I want to talk about why.

[00:00:04]>> Wait, I have to say that this doctor does an incredible job of explaining the reasoning behind why an apesiottomy was necessary. But the comments in that post are ridiculous. I didn't know there were so many board certified OBG weigh-ins out there. Saw one in particular that looks like she's a board-certified OBG wayan who owns a tea shop. Aka, she's not really a board-certified OBG weigh- in, but she says that all aziottoies are bad. Let me tell you this. I've had an apesiottomy. I had it with my first. She was vaginal delivery and she was a shoulder dystocia. It was quite clearly one of the most terrifying moments of my entire life. I had a perfect pregnancy.

[00:00:41]I had a wonderful labor up until the very end. That's the thing about shoulder dystocas. And if you're not familiar with them, it's essentially when the baby's head comes out and the shoulders get wedged behind the mother's pubic bone. It's like trying to fit a square peg out of a round hole. and you only have a very short window of opportunity to get that baby out before real problems evolve. Things like hypoxia, brain injury, death even. It was a code blue situation for myself.

[00:01:06]Thankfully, they were able to get my daughter out, but she was unresponsive for about a minute. Um, and had very low APGAR scores. She had to be down to the NICU for a while. Um, she had an apnea scare a hour and a half later where she stopped breathing altogether. It was really bad. And the only reason why they were able to get her out on time is because they cut an epziottomy. That way my doctor could literally reach in, turn her, get her out. Um, yeah. So anyway, this is just to say I'm backing up this OB/GYN. Um, I would highly suggest, like she says, to talk to your provider as to what circumstances would warrant for an aesiottomy. That entire experience that I went through was terrible. It was traumatic. um for a multitude of reasons obviously being that but also my provider did not discuss with me about the epziottoies I didn't even think to talk to them prior about anything related because I kind of went in naively to the entire delivery thinking that they knew what they were doing which they clearly did um but I didn't think of what questions I needed to ask how to advocate for myself and make sure that I felt heard during the entire experience because you see the reason why it was so traumatic, aside from obviously the distress, was the fact that I felt like my body wasn't mine anymore. That loss of bodily autonomy is something I wish on no one. And now, as a mom of three, I had two C-sections following that because it was just too much of a risk for me to have another baby again and know that that potential was there. I just mentally I couldn't handle it, but also statistically it it it wasn't looking good for me. Going into my second, my third, I made sure to ask all the questions. I worked with midwives and they were wonderful. The midwives and the OB/GYN who they paired with who did my surgery was wonderful.

[00:02:54]Expressed my concern. They listened to them. Both of my deliveries after that were beautiful.
