The Speak Out program is a structured speech therapy approach for Parkinson's disease that uses progressive vocal exercises (warm-ups, AHs, glides, counting) to strengthen speech muscles, followed by e-library flashcard exercises that train patients to speak with deliberate intent when completing sentences, addressing the common challenge of starting and finishing thoughts in conversation.

Parkinson’s Speech Exercises追加:
Hello. Thank you for joining Parkinson Voice Project for today's Speak Out Home Practice Session. My name is Samantha Allen Derry. I'm a speech language pathologist and the founder of Parkinson Voice Project. I'm here to help you learn the Speak Out exercises and also help you establish and maintain a consistent Speak Out home practice routine. I know how hard it is to have to exercise your voice every single day.
Um so hopefully these sessions are helping. Today we're going to be working in the e-library. We're working on that challenge that people with Parkinson's can have with starting to say a sentence and then struggling to finish the thought. Um that can be a real problem with Parkinson's, but I'm telling you it can improve. So if you're experiencing this problem, it can get better. And if you want to help keep from having that problem, then I really want you practicing in the e-library. So we're going to do the basic Speak Out exercises first and then we'll get into the e-library. Before we start, let's practice smiling with intent. Give me your best smile.
All right, take a sip of water. Do that with intent, too.
All right, first Speak Out exercise, the warm-up. Do this with me.
May me my mo moo.
Good, again.
May me my mo moo.
Really speak out. Get your voice in front of you.
May me my mo moo.
Connect the sounds. Connect them. Don't let it be may, me, my. No, no, no.
Connect them. Ready?
May me my mo moo.
Just one more.
May me my mo moo.
Good. We're working with intent. Take a sip of water. We'll move on to the ahs.
All right. For the ahs, open your mouth wide. Project that ah forward. Make it fly up and over your computer. Here we go.
Ah.
Don't worry if your ah is not as long as mine. It's a maximum of 10 seconds, so shorter is okay. Let's do another one.
Ah.
Good.
Let's do another one.
Ah.
Good. Make sure you're not closing your eyes because you're working so hard.
Make sure your eyes are open when you do this. Let's do a shorter one.
>> [snorts] >> Ah.
Good.
One more.
Ah.
Very nice. Take a sip of water. Do it with intent. Be deliberate as you pick up your glass.
And always swallow with intent.
We're moving on to the glides. Project out a steady ah and then slowly glide up. Here we go.
Ah.
Ah.
Even though you're going up and down, the sound is always in front of you. The name of the program is Speak Out. Ready?
Ah.
Ah.
Good. Let's do another one.
Ah.
Ah.
Are your eyes open? I say that because in therapy it's not unusual for people to close their eyes when they're doing this because they're working so hard.
Well, we want these muscles down here working, okay? Not up here in the face.
Keep the eyes open. Ready?
Ah.
Ah.
I think that's enough of those. Take a sip of water with intent.
I really want us to have enough time to get into the e-library.
Let's go ahead and do the counting next, though.
All right. Lift every number. Lift it and project it forward.
One, two, three.
Four, five, six.
Seven, eight, nine.
10, 11, 12.
Good. We're connecting the numbers just like we did with the may me my mo moo.
Ready?
One, two, three.
Four, five, six.
Seven, eight, nine.
10, 11, 12.
Good. We're going to do it one more time, but let's count backward.
Ready?
12, 11, 10.
Stop.
Nine, eight, seven.
Stop.
Six, five, four.
Three, two, one.
I realized I was going a little bit too quickly and so we can adjust. We catch ourselves when it's going into automatic mode and then we make ourselves use intent. All right, we're working. If you need some water, go ahead.
We are working in our Speak Out e-library right now. You can find the button. It's a purple button on the top right of our website. No login or password required. Today we are going to work in the flash cards that are called complete the sentence. I'm going to click on this one, complete the sentence. If you recall last Monday, we did complete the phrase. When you get really good at the phrases, then we work on the sentences.
This is to help you be able to speak with intent in conversation.
So, it is vital that you and your speech language pathologist are using the e-library because the goal is to speak with intent in conversation. It's not just about ahs and glides. We do that those exercises to strengthen the muscles, but now we've got to use intent.
I want you to answer this question.
We're going to read it together and then you fill in the blank.
Now, when you fill in the blank, make sure you're using your best voice. And when you do the flashcards and you're reading, you're being very deliberate.
So, you're not going to say, "My favorite meal is spaghetti."
Uh you are going to be very deliberate like this. "My favorite meal is spaghetti." That's not really my favorite meal.
>> [laughter] >> I have lots of different ones, but um not really spaghetti. But anyway, very deliberate. So, read it with me. "My favorite meal is Ready? Again.
"My favorite meal is Now, pick a different item. I was going to say, "I really like scallops."
I do. I like going to this restaurant having scallops. I always order it. All right, let's read it again.
"My favorite meal is Okay.
Now, let's do the next one and this is a little These are harder than the phrases that might just be up and down or something like that. Fill in the phrase, think of the answer, and then speak out.
"We turn on Again.
"We turn on Are you able to make the uh just complete the sentence? For example, "We turn on the television."
Say that with me.
"We turn on the television."
Really think about it. Ready?
"I like How did you do? Were you able to speak with intent while you came up with the answer?
Did it take too long to come up with what you wanted to say?
Those are some challenges with Parkinson's and that's why you need to work in the e-library. Let's try it again.
"I like So, I might say, "I like to cook dinner for my family." Say that with me.
"I like to cook dinner for my family."
Do you notice how it's hard? Every word with intent. Repeat it again.
"I like to cook dinner for my family."
Can you say every word with intent?
Fill in the blank with this one.
"Yesterday, I How did you do? Did your voice trail off? Did you exaggerate?
Try it again. "Yesterday, I So, I might say, "Yesterday, I saw my grandchildren."
And I can't let it trail off. So, just say that with me. "Yesterday, I saw my grandchildren."
Again.
"Yesterday, I saw my grandchildren." Or how about, "Yesterday, I went for a walk." Every word with intent. Say it with me. "Yesterday, I went for a walk."
If you want to make it more challenging, then you can go through your whole schedule. "Yesterday, I woke up at 8:00 a.m. I had breakfast. I had two eggs and bacon." You can explain and make it more challenging. Let's try another one.
Complete the phrase sentence. "Next year, I How'd you do?
I want you to repeat it with more intent. "Next year, I Let's do one more.
"The house Say your answer again. Let's read it and use more intent. Have more control. Be more deliberate. "The house Okay, take a sip of water.
Do you see how challenging it is?
Please get into the e-library. Spend 15 minutes working on it. You can work by yourself or you can work with a family member. It would be fun to get on with children or adult children or grandchildren and everybody fills in their own answer. It will help you.
All right, Dr. Boone's words. Exaggerate the G's, the K's, and the NG's. Here we go.
Goo la ga ga Again. Goo la ga ga Make sure you're speaking out and exaggerating.
Coo ca ma goo ca Coo ca ma goo ca Next one.
Gangla Speak out. Gangla And the last one.
Coo cla Coo cla Okay. So, if you're struggling with that complete the sentence, make sure you back up and do some of the other flashcards first.
Start with opposites, so those are a little easier. Up and hot and Okay, that's it. You start that way. Then, that's the opposite flashcards. Then move to complete the phrase.
"Open the Drive a All right, then when you can do those really well with your best voice, then move to complete the sentence. I hope you enjoyed today's session. Thank you for joining me. Have a great week.
