>> All right, let's do it.
>> Does TRT cause heart attacks?
>> No. So, this is a common misconception.
Earlier concerns led to an FDA blackbox warning years ago, but newer evidence showed no clear increase in cardiovascular risk in appropriately treated men. So, that warning has since been removed.
>> TRT in men with high cholesterol.
>> So, this one's tricky. I'm going to give it a maybe because testosterone can improve body composition, insulin sensitivity, increase in muscle mass, burn fat, but cholesterol can still rise with treatment. So, it's very important to get it properly diagnosed, properly monitored, and properly managed if it is diagnosed that somebody has elevated lipids.
>> TRT in men with uncontrolled high blood pressure?
>> No. So, blood pressure should always be controlled before starting therapy. And this is something that if it's unable to be controlled, then the patient in my book does not qualify for TRT until it is appropriately managed and appropriately controlled. Very important.
>> TRT with resistance training.
>> Yes. So absolutely non-negotiable. So exercise actually amplifies the metabolic and muscle benefits of TRT. So improving muscle mass improves insulin sensitivity which then improves metabolic health. In fact, I usually tell my patients only to consider TRT if they're going to be serious about exercising regularly, as it's just one of the most fundamental hallmarks for you to get the most out of your TRT therapy.
>> Giving every TRT patient estrogen blockers?
>> No, absolutely not. So, this is one of the most common misconceptions and actually drives me crazy. In fact, one of the biggest things I see when I'm inheriting a patient from another TRT clinic who's switching over to me is that they're inappropriately put on estrogen blockers. Estrogen we have to remember for men is important for bone health, for brain health and might prevent osteoporosis, could prevent dementia. So it should never be suppressed if not needed unless it's absolutely necessary. So if somebody's blocking your estrogen and it's going too low, they're doing you a disservice.
>> TRT causes infertility.
>> So yes, it can. So exogenous testosterone suppresses the body's natural sperm production. So if somebody's trying to conceive, TRT isn't the best option. The good news is there are some alternative therapies that can boost testosterone without affecting fertility and in some cases actually may increase fertility in certain circumstances. So better options out there if you're trying to conceive than TRT and we're happy to help you with those if you need it.
>> TRT without monitoring labs?
>> Absolutely not. So many important labs.
Hematocrit lipid panel PSA prolactin LH FSH total T freeT estradile sex hormone binding globulin just to name a few. so many more thyroid function that need to be monitored when somebody's on TRT.
>> TRT in men with obesity.
>> So, this one's tricky. I'm going to go maybe. So, basically, adapose tissue has aromatase in it, which can convert testosterone into estrogen. In my practice, I'll generally do a pre-treatment for these patients to have them lose some fat before starting so they can get the most out of their TRT and so they're not aromatizing the testosterone or giving them purely an estrogen and raising their estrogen levels.
