Hey friends, this is Ray, your favorite cardiology nurse practitioner. If you're new to the channel, welcome. If you're not new, welcome back. This channel is all about heart disease, both prevention and treatment, other topics related to the heart and overall health and wellness. So, in today's video, I just wanted to quickly go over a patient that I had recently. I actually had the opportunity to take care of this patient uh both on the cardiology aspect and also on the nephrology aspect. Uh so, I actually uh work in both cardiology and I recently uh joined uh the nephrology specialty a couple of months ago. So, I'm still learning a lot about the nephrology world, but uh for those who don't know, that's the uh kidney specialist. So, um I uh had the opportunity to join a nephrology team, so I'm learning uh more about that. But, I had this patient that came in um because he was having chest pain and um the patient ended up having um an echocardiogram that showed that the ejection fraction, uh which is the pumping function of the heart, was actually a little bit on the low side.
And uh not only did he have uh heart failure, but he also had kidney failure.
That's why I got to see him on the kidney side of it because his kidney function, uh there's a number There's two numbers that we look at for kidney function, something called the creatinine, and that number should be less than 1.2, and his was double that.
It was like 2.6, more than double.
And um there's another number that we look at called the GFR or glomerular filtration rate. And that number, generally speaking, should be above 60 in most people.
Um in some patients, uh it's like over 100. Um his was in the 40s. So, uh not only does he have uh kidney failure I'm sorry, uh heart failure, but he also has kidney failure. And come to find out, um the reason that was attributed to this, uh we did all the normal workup, you know, um he did not have any blockages in the arteries of the heart.
And so, uh the only thing that um is left to basically attribute it to is uncontrolled hypertension. And so, I've mentioned this in other videos, but hypertension is uh not only is it called the silent killer because most people don't feel anything until they have a heart attack or stroke. Uh sometimes people say they feel a headache or whatever, um but um uh hypertension happens to be the second leading cause of kidney failure. It's the number two cause uh right behind diabetes.
And then, of course, we all know uh or we should know that high high blood pressure or hypertension also causes uh heart attacks and strokes, right? So, um anyway, I just wanted to go over that patient with you. Um you know, unfortunately, this patient did have known hypertension for several years, and he unfortunately wasn't taking his blood pressure medication. So, I'm begging you, please, please, if you have hypertension, please make sure that you are staying on top of your blood pressure and uh keeping track of it.
