See this gap? This is the Terry Thomas sign. The sign which suggests that there is a tear of the scaphoid lunate ligament.
Injury to the ligament, even when it occurs in a young patient, it predisposes to early arthritic changes.
That's why it is very important to recognize this. And usually the cutoff for this is 4 mm. If the difference or the gap between the two bones is more than 4 mm, on an X-ray we can definitely say that there is a tear of the scaphoid lunate ligament. So there are three components within the scaphoid lunate ligament.
So this is the scaphoid on the axial image and this is the lunate. So the first component is the dorsal component and it is the strongest one. If there is discontinuity or tear of the dorsal component, it is highly likely that the rest of the scaphoid lunate ligaments, the interosseous and the volar components, will also be torn and there would be widening of this. And this would lead to the instability of the wrist. Thank you.
