[00:00:00]So, hello everyone. So, today we are going to tell you case of a patient who has overcome with the recurrent sarcoma with metastasis. Normally, sarcomas are considered as the bad cancers. [music] Metastasis is very common. The disease survival is also not very long.

[00:00:17]So, this patient who is diagnosed as a undifferentiated pleomorphic sarcoma, which is also known as malignant fibrous histiocytoma, is a aggressive soft tissue cancer. It typically appears in the [music] late adulthood, often in the lower extremities, and carries a challenging prognosis [music] due to its high rate of recurrence and metastasis.

[00:00:39]For many cases, actually, [music] surgery is the only primary option. But when the cancer is spread to the lung and the adrenal glands or some other part of the body, the road ahead becomes significantly more difficult. [music] So, in we have got a patient who is also our colleague [music] in diagnosed in August 2023, is a 66-year-old man who spent his life others found a sudden painless and swelling in his own left thigh.

[00:01:13]And this was the beginning of the complex clinical journey. Despite an initial operation and radiotherapy, the margins remains positive.

[00:01:21]>> [music] >> And then by early 2024, the disease had progressed and [music] follow-up PET CT scan revealed a local recurrence. And disease also has spread to the both lungs as well as in the adrenal gland.

[00:01:37]>> [music] >> The patient began patient began experiencing the symptoms of a dry cough, shortness of breath, and weakness.

[00:01:46]Faced with a poor prognosis and known side effect of the traditional chemotherapy, the patient made a pivotal decision. He refused for chemotherapy and opted for more targeted approach.

[00:01:59]For this patient, we did the next generation sequencing and genomic analysis. So, with this testing, we >> [music] >> found there is a multiple genetic mutation, which includes six AR and FANL amplification. Based on these specific genetic markers, [music] I advised patient to go for the combination chemo immunotherapy treatment.

[00:02:25]So, patient agreed and we started the immunotherapy and the response was remarkable.

[00:02:30]>> [music] >> Following just six seven cycles of this immunotherapy combination, a repeat PET CT scan is done in around August, which shows the complete response in the metastatic [music] disease.

[00:02:43]But there was a small active found in the thigh, which was successfully excised [music] by the orthopedic surgeon in September 2024.

[00:02:55]So, 1 year later, the patient during follow-up patient has again gone for the PET CT scan, >> [music] >> which which shows the complete response.

[00:03:08]Today, this patient, who is also our colleague, is a completely disease-free, asymptomatic, and remains [music] under follow-up.

[00:03:16]The case is stands as a powerful example of how immunotherapy guided by genetic testing can achieve a complete response in the patients who have metastatic sarcomas and without the need of cytotoxic chemotherapy.

[00:03:36]So, this is a example of patients who have a sarcomatous cancer with metastasis with poor prognosis can still go for the complete remission.

[00:03:48]As in this case has happened, they should go to the [music] oncologist, discuss the treatment options, and the available genetic testing so that educated treatment [music] can be performed and patient can get complete remission and enjoy the life. Thank you very much.
