This video perfectly illustrates the clinical necessity of mechanical debridement to ensure pharmacological efficacy. It is a visceral yet essential reminder that medication is only as effective as the surface it can actually reach.

FUNGUS on the EAR DRUM! Episode 614Hinzugefügt:
Coming up in today's earwax removal video, we've got some really interesting cases. Um, the first one is a fungal infection really deep in uh this lady's ear as you can see there. And the second one it was quite a painful uh procedure uh for the patient and a lot of manual tools used in that. So, there's a a bit of everything in here and as usual I will drift off topic cuz I've got a lot uh to tell you about as well. Uh a few updates on stuff I've been telling you about recently.
So, this first patient, this lady, she came to see me. Interesting um uh piercings on the way into her ear that are getting the way sometimes those.
Um Uh she came to see me because she uh she'd seen a doctor for um on a few occasions I think and I think they'd done a fair play to them. They'd done a swab and they'd diagnosed with a fungal infection and they've given her an antifungal drop. However, when she came to put in the uh fungal drops in, she said um felt some of it go in and then it it just started pouring out. I didn't feel like it was actually doing anything. And she's dead right because this was in the way which was the um the infected debris, the skin that there's that the fungal infection has created and left behind. So, you know cuz a lot of people contact me and they say, "I've been treated for an infection so I don't think I can have it done yet." And I said, "That's absolutely not the case." I said, "You should absolutely have it treated because when and I know a lot of you watch these videos will already know this.
Um if you're going to treat an ear for an infection, it's better that that it's empty of all the debris that the infection has created for it to work best because if you put the um antifungal drops in here, it's just going to sit on this gunk and debris and it's not really going to do its job. So, it won't get to the outer so you need to start with a a clear ear.
Uh and then get the treatment in. So, she rightly so she stopped it herself and she said, "I I think I need to go and get this removed and then I'll start the treatment." So, uh that's what she did and that's why she ended up in my clinic and I'm glad she did as was she.
Very tricky this though.
All over the eardrum this this debris so it was particularly tricky to remove. Um Before I forget subscriber of the day, let's do it today and uh today's subscriber of the day is Courtney Mower.
Thank you very much for your message or your messages Courtney. I saw her a couple uh asking to be subscriber of the day. Thanks for your support of the channel. I really appreciate it. If you want to be subscriber of the day, just leave us a comment down below there in the comment section and say, "I would love to be subscriber of the day."
So, what I'm trying to do I'm trying to bit limited in space here because it's um there's a there is a layer of skin that's really close to the edge of the canal wall and you really want to be careful when you go down there obviously. You don't want to touch the canal wall. That's very sensitive, very uncomfortable if you get it wrong.
So, I'm just trying to figure out which way I can peel it from whether it's from bottom to the top, top to bottom, left to right, right to left.
So, at the moment I'm trying to try bottom to top but that just broke off there.
Still couldn't hear terribly well at this point.
So, I'm going to try I think a bit more from the right to the left here.
And as you can see coming out nicely.
Now, at this point we can see the whole eardrum. There's a few little flakes and you'll see in a minute um I may I'm making a clinical decision because I'm looking at that flake in there and I'm thinking, "Oh yeah, I'll get that."
So, as I entered with the tube cuz it was really bendy this. As I pulled back there you can see there's there's a bend there. So, and I'm I'm looking at it and thinking, "Do you know what? I really don't think it's worth the risk trying to get that little flake. It's not on the eardrum and when she puts the antifungal drops in, it will loosen up the last few flakes that are in there and it will fall out when the fungal drops come out. So, really that's what I'm saying about risk versus benefit. There's no benefit to getting that but plenty of risk. You know, if she moves or I catch the side of her ear canal.
Uh just no benefit to it at all to be honest but lots of risk.
Um right, second patient was a a bit of a squigy matted affair in this ear. Bit more solid in the other ear so I think we had to use um some manual instruments in the last one.
Uh but I really don't like it when it's this consistency when it's just it's too squigy. This one is too squigy and soft uh to get any tools in there. It would just spread it around.
But there's a lot of uh loose hair that's been matted in it in this squigy horrible plug.
Uh so, I think I think we'll get some oil out to get that out um shortly.
I'm telling you what I'm about to do cuz I'm going to start drifting off topic again. Um and in the second one yeah, we used tools if it did find it quite painful in the second one cuz he hadn't put any oil in cuz he didn't think he had a problem with it. Uh so, he didn't put oil in but it was a lot firmer the second ear.
Right. Few bits to tell you about.
Um First of all, um I did mention in a few videos ago that I was in um um a potential partnership with a really large uh healthcare company to try and develop this product that I had the idea for.
And I also did say at the time, "Sometimes these things fall apart or don't happen." And in that in that that instance, that is the case unfortunately.
I have still got this idea and I'm determined to make it happen but it probably will be with um a different company. Maybe these things were lost in translation because it was in a different country I'm not going to give away too much here.
Um but I absolutely am determined to get this healthcare product out there and I will make it happen so there's but there're going to be a a longer delay than I originally thought.
Like I said at the time, these things often fall apart.
Um and it's you >> [laughter] >> it's usually all over money and that kind of wasn't it kind of wasn't the the case this time.
Um because I thought they were funding the development, they thought I was funding the development. Now, I'm one person with a small business. They are a multi-million pound company. So, maybe I assumed that they would turns out I was wrong.
>> [laughter] >> Uh anyway, right, cleared that ear.
But but still keep your eyes peeled because I am this idea is too good for me not to persist with it. Uh I'm just going to have to find another company to work with cuz it's too much to do all all by myself just as one person, you know.
I can only spread myself so thin before there's nothing left of me so I am I am uh in the process of finding some other manufacturers. Anyway, I know that sounds really cryptic.
>> [laughter] >> Um what else?
I'm going to weigh this tonight. I will have already been and gone if you're watching this cuz I think I'm about 2 weeks ahead of schedule at the moment as far as recording them and then posting them.
So, um I'm not going on holiday to where you can see me out now cuz I'm on the beach lately.
>> [laughter] >> I wish I was going there.
But I'm going to stay with me sister and family up in the beautiful borders of of Scotland which is just I call it the borders cuz it's on the border.
So, it's it's just in the very in the very south east of Scotland uh is where my sister lives. Just about 45 minutes away from Edinburgh and it's absolutely beautiful part of the world so we're going to spend a few days uh with her and the rest of the family so I'm really looking forward to that. So, I'm having just half a day today.
And I'm going to uh um what else I've got to do? I've got to take the dogs to the groomer. So much to do. Got to pack, got to do this.
Because I work for myself, I can say to Mrs. Wizard, "Yeah, I'll I'll I'll sort everything out. You carry on working till like 5:00 and then I'll have sorted everything out." And that's yeah, the dogs to the groomer, packing the kit, picking the kids up, uh packing the suitcases.
Although I might not be trusted with that once because one time I did I was left responsible uh for packing the suitcases on a family holiday uh [snorts] for the kids and I forgot to pack my daughter any knickers.
This [laughter] was when she was quite young.
Uh so, yeah maybe I'll maybe I'll be be stopped from doing that anymore.
>> [laughter] >> We did have to we did go and buy some more, don't worry. We didn't leave We didn't leave without any.
Let's quickly This was quite uncomfortable this for the patient. Oh, I've got no Have I got time for that joke of the day? Yeah, go on.
Let's have a couple.
Um Conversation between dad and his daughter and his daughter. "Daddy, are you going to take a bath?"
Dad says, "No, I'm leaving it where it is."
That does that uh uh irks me that. Irks me, it bugs me, it annoys me.
Now, I know this is an American thing when you go into the toilets, "I'm going to take a whatever. I'm going to take a or I'm going to take a bath." Where are you taking it to?
>> [laughter] >> Where are you taking it to?
Use the proper English. I'm going to have have one I'm going or I'm going to get in the bath. But I'm going to take a bath.
Where you going to take it to? I don't get it. Why do you lot do that?
>> [laughter] >> Anyway, as you can see, clear the second year very nicely. Here we are I'm from from the beach. I wish you a fond farewell.
But for now, take care of yourself and I'll see you soon.
Ta-da.
