When I look in the mirror and I see the scars, I feel a little depressed, even sad. It just reminds me of everything that happened.
>> So, you're a little nervous?
>> A lot nervous. I think it'll be all right. 5 years ago, I spent the night in a hotel in Vegas. When I woke up in the morning, my back and my butt were covered in bed butt bites. I didn't think much of it at the time, but what happens in Vegas obviously don't stay there.
When I got home, I ended up having emergency surgery to get rid of the flesheating bacteria, which was literally eating my butt cheek. I tried to cover the scars and the indentation with my tattoos, but they still bother me a lot. I always have layers of shirts, layers of clothes after everything like home stretch, right?
This whole thing has caused me a lot of emotional pain, but it's also left me with some residual physical pain as well. One of the biggest drawbacks uh of all of this is that I love riding motorcycles and it cuts out of my riding time.

>> Well, I got bit by bed bugs and the bed bug bites were infected from the dirty sheets at the hotel. So, I ended up with a hole in my butt cheek. Flesh thinning bacteria does not just occur from insect bites, but it can occur from any break in the skin that creates an injury where this aggressive bacteria comes into the body and eats away at the tissue.
>> If it would have been one more day without it being treated, then my whole right ass cheek would have been gone.

>> My name is Pam. After battling cancer, I'm ready to get back to normal. I had a bilateral mastectomy which means they removed both of my breasts. At this point in my reconstruction, I have tissue expanders in. These tissue expanders are really limiting my lifestyle right now. Hey guys, I think I may need to stop for a second and catch my breath. They basically push your pectoral muscle out. I feel tightness in my chest. I can feel the hard plastic.
It's been a constant source of pain since the mastctomy. All right, another day. Another day we'll do the trick. All right, thanks guys. I'm sorry. Cancer can be a bully if you let it. But I have found strength that I had no idea that I had.

>> This has been a difficult journey. It really has. And I need this reconstruction in order for me to feel like a real complete woman again.

>> My history with Pam is that I came in after her mastctomy and placed tissue expanders in her breast cavities. It's probably been about 4 months since your mastctomies.
>> Yes.
>> The tissue expanders are placed after the breast tissue has been removed in order to preserve a lot of that skin for the reconstruction. Once you get that diagnosis, it's like, you know, what am I going to do? Am I going to go lock myself in a closet and cry, you know, forever, right? Or am I just going to continue to live the rest of my life like Pam? I have a personal cancer experience. Unfortunately, I was diagnosed with stage 4 large bell lymphoma, non-hodkins.
It's a huge blow to what you know as normal life, you know, to where Okay, y'all.
I take from my experience that I can relate to my patients in in that regard.
You know, I just know what it's like to be on that receiving end. Tell me a little bit about how things have been feeling, you know, during the whole expansion process.
>> It feels like like rubber bands across your chest, Neil, and that's uncomfortable. it does feel um hard >> is because the expanders are they have a firmer shell than you know a regular permanent implant. Okay? You know that's really to help kind of push that skin out and so that's a lot of the discomfort that you may feel >> and so we're planning right now to remove your expanders and replace them with your permanent implants. Do you have any desire on the shape and look of your breasts?
>> I'm very active. Dave and I enjoy cycling, yoga, so I don't want to feel encumbered. I'll, you know, look at your measurements and then we can pick a desired size.

>> You know, to kind of get you to still be able to be active and free and not have, you know, boobs bouncing up in your face.

Hi, I'm here to see Dr. Jones. My name is Deborah. No one could have told me 20 years ago that I would be 45, divorced twice, living in an apartment. Nobody.
I've been unhappy with my body for the last, I would say, 10 years. My heaviest weight was 305. In 2005, I had a gastric bypass. It left a lot of unwanted skin.
The left side of my breast is a lot larger than the right side. A lot of times I shower in the dark. I get dressed in the dark, so I just don't have to see them. With the second divorce, it just kind of really broke me because the divorce was his decision.
So, it kind of compounded all of the negative feelings I have about my body.
>> My whole thing is this, you know, I want to get back into the dating world. I want to be able to be really, really confident, go out on a date. The competition is fierce in Atlanta. The ratio to men to women, you're doing good if you get a man that only has two women. Yeah, it's difficult. I've been on a couple of dates, but I don't allow it to go any further than that because I'm afraid that anyone who sees me with my clothes off is going to run for the hills.

>> Yeah.

>> I definitely want to find love, but I want the type of body that gives me the confidence to be able to go out and get it.

>> You're the total package. I need to meet the competition to find love, but at this point, I'm not confident enough to do that. Hopefully, this surgery will allow me to do that.

>> Just stand on this step stool. There we go. Now, take a look. Today I'm examining Bri. Trying to determine the appropriate sized implant for a little person is tough. I'm not sure I'll be able to get her exactly what she wants, but I'll do my best. These are normally the sizers that we'd use to help patients understand how much volume we can add to their breasts. I'm not sure this is going to be an accurate assessment for you because your chest wall is a little bit more narrow than what we would ordinarily see. And it's all because you're one in a million.
Looking at the dimensions of Bree's chest, I can special order the appropriate implant that'll give her the volume and projection she needs without being too wide for her narrow chest.
>> It'll add more shape to this upper portion of your breast. So that right now where this is concave and flattened, we want to have volume.
>> My plan for Bri is to perform breast augmentation with an implant that will project enough for her narrow chest.
>> I got a question.

>> All right. Pam is one of my breast cancer patients who has had tissue expanders placed after mastctomy and now she's here to talk about her upcoming surgery to put permanent implants. We've gotten out all those creases. You have good projection of the expander and your scars have healed up really nicely. I kind of call them Insta breasts. She actually gets to see the size that she potentially wants to be before surgery.
So your expander looks like this, >> right?
>> It's filled with water. That's why it feels so tough and firm in your body.
This is a regular silicone implant. So you can see it's a huge world of difference than the tissue expander.
Much softer, much more natural.
>> Much more natural, much more lifelike. I know you've been waiting to touch that.
>> We're thinking about the shaped one.
I'll choose one that's a little bit more tapered. But, uh, these come in different profiles, too.
>> Dave is really having a fun time checking out these implant samples. So, who's getting these implants again? Is it Dave or Pam?
>> I'm really trying to downsize the appearance of this to maybe about 15 20% of what this size is.
>> Okay. And I think that would probably put you at what you want. The surgical plan for Pam is to remove her tissue expanders, do some contouring, and place permanent implants. So, right now, Pam's breasts are literally rock hard, but if everything goes well after surgery, all of that pain will be gone.

In addition, she will have lipo suction of her abdomen, flanks, and back and fat graft buttocks. I'm hoping to alleviate some of the pain that she's experiencing by excising the scar that goes deep into her tissues and down to her muscle.
>> All right, so let's get you back and we'll get going. This pain in the ass has been a true pain in the ass.
Hopefully in a couple hours that'll be all over.
Incision. We're staying inside the confines of the tattoo, leaving her tattoo intact as much as possible.
Hopefully, this operation can dramatically improve DA's life. She may be experiencing pain because of a huge amount of scar tissue which compress on her nerves. So, by removing that scar, she's going to be able to bike without pain, hopefully. And also, the constant reminder of her bed bug experience will be gone. She has a good tattoo artist.

Just a big crater here. My fingers I can get all the way in there. So, it is like a bucket handle. So, we have to fill this all with her dragon tattoo in mind.
The challenge with raising DA's scar is all centered around contour. We have to excise all of that scar, bring all the tissue together, and leave her with a smooth skin surface that blends in with her butttocks.
Good job, team. We grafted her butttocks. We revised her scar.
Hopefully, she'll be happy. I feel that the surgery went well today. Her big scar went an inch and a half deep. So, it was a little bit challenging. Her scar was oddly shaped. Hopefully, by removing that scar, she will be able to live freely without pain.

My surgical plan is to do a breast reduction and a lift, an extended tummy tuck and lipo suction.
There's a big asymmetry there. So it may not be perfect. I'm not God.

Don't need to be nervous. Today, Bri is getting breast augmentation with silicone gel implants. She's a mom and she encountered the problem that a lot of moms have after having babies. Her breasts are deflated. Most patients that we have don't weigh 75 lbs. It's a little bit of a struggle on our back moving them after surgery, but for you it'd be kind of a light day.
>> Bree's a little person and because of that, her chest width is a little bit more narrow than what we would ordinarily see with a breast augmentation. So, I'm using breast implants that are specific to her chest wall.
>> I'll see you back in the operating room.

>> Hopefully, I'll have normal breast soon and get back the confidence that I used to have.
The biggest difference when operating on a little person is getting them under general anesthesia. The airway may be smaller than an averagesized woman. So, we need to make sure the airway isn't compromised.
>> Do I have another 20 or 22 there?
>> Before surgery, Bri was making a loud breathing sound.
>> Hold on.
>> This doesn't look right. We had difficulty managing her airway because of the smaller airways and little people.

>> I was starting to become concerned.

So, I decided to replace the standard breathing tube with pediatric size tubes just to make sure that the surgery is safe.

>> For a little person, breast augmentation is tough. The chest width is often narrowed.

>> And so that can make it a little difficult to place the breast implant.

>> Did you say it's cute?

I'm choosing a little bit of a taller implant so that it kind of fills this area here. Compared to a tissue expander, a permanent silicone implant is much more soft. The challenge for me is trying to remove her expanders through her incision opening so that her scars will be minimal.

Goodness, I almost dropped the baby. The tissue expanders are placed after the breast tissue has been removed. That's her expander. In order to allow some of that skin from the mastctomy to recover and also preserve a lot of that skin for the reconstruction, I've chosen an implant that will fit that diameter of her breast.
I think that's much nicer. I think she'll be happy with this.

>> To no longer have those expanders inside my body is such a relief. They just hurt. They were just miserable.
>> All righty. So, I think you're doing great. The implants look awesome. Pam has been through so much with breast cancer. So, I really hope that she'll feel like a complete woman again. So basically this was you after your tissue expanders were placed. You could just see how they kind of just jut out from your chest wall and they just don't have a as natural appearance. I was feeling pretty sad looking at the photos.
Whenever you feel ready, go ahead and take a look at the new you in the mirror behind me.
>> Okay. All right. You ready?
>> I'm ready.
>> Okay. Here we go.
>> Look at that.
>> Wow. It's amazing. Wow, they look wonderful.
>> I've been waiting on this day for a long time.
>> Yes, Dr. Baron did it. Woohoo.

>> This has been a really uh long journey.
The bilateral mastctomy was very traumatic. It definitely took away some femininity, but this surgery has uh given me a huge boost in confidence.
Pain will be gone now. And to know that I can get just back to doing the things that I enjoy. It feels amazing.

Hopefully, it's my final appointment and I get to be really confident and just live out loud. I cannot wait to see my new body.

>> I'm going to take a quick little peek today. I want to check and make sure that the incisions healed very well. Got some swelling. Nothing weird or unusual.
Considering where we come from, I'm very happy. This is where you were.
>> Wow.
>> That's very difficult to lose with exercise because of the excessive skin with your breast. Your nipples are in totally different positions.
>> As I'm looking at the before photos, it reminds me of the divorce, the loneliness, the self-doubt.
>> And now we're going to see where you are.
>> Say goodbye to that person. And Oh my god, you look nice. Yes, >> good.
>> This is great.
>> I got really lucky on you.
>> I never thought it would look like this ever.
>> Like somebody pinched me. I don't want to be dreaming. Before this surgery, I was not comfortable in my own skin. My biggest fear was getting into a relationship. Now I feel confident enough to not being afraid to be intimate, you know, and just kind of be sexy and desirable. The level of confidence is through the roof right now.
>> You're ready for Marty right now?
>> Yes. We can run through the streets naked now.
>> That's on my bucket list. Now, >> when I see a patient get emotional and really happy, I smile, but deep down I'm like, "Yes, yes, yes, yes."

>> Okay. Recovering from surgery wasn't that difficult because I had help from WA.

>> Not to myself in the mirror. I can feel it.

>> Well, let me just take a look and we'll see how things are healing up. Bree's final result looks great. The most difficult part of her surgery was really choosing the appropriate implant. And I think we hit the nail on the head. You can see what we have here is exactly what we see after every mom has had babies. Uh, >> this looks sad.
>> That's right. They look sad. They look sad. So, you know, we just had to cheer them up. We had to perk them up a little bit. And so whenever you're ready, I want you to take off that robe and show us an >> Okay.
Oh my gosh, you look good. Like really good. You did a really good job. I love them.
>> My first reaction was, "Wow." Like these boobs are perfect. They couldn't be more perfect than they really are.
>> You look amazing. Everything looks fantastic. I mean, they really fit your body. Everything's proportional. Brings back the volume. How do you feel?
>> Good. I'm really comfortable with my body now. What do you think?
>> I like them. This mean you going to be walking around the house like that? No.
>> On a scale of 1 to 10, it was really a 10. Very natural looking.
>> Now I can be really comfortable with my body. I have no worries.
>> Before surgery, like I had a lot of insecurities. Now that I have new breasts, I have so much more confidence in myself that I feel like I can help my son get through life with more confidence.
>> You know, since you're feeling so good, you can listen to Woo. You can walk around the house like that if you want to.
>> Just don't knock anything over.
>> Okay.
>> Thank you so much.

>> Today is my final follow-up with Dr. Jones and I'm feeling anxious. This has really been a a crazy roller coaster of a journey because some bed bugs had literally ate my right butt cheek up.
Hopefully, this will be done and let that chapter be closed.

And you had lack of projection of your butt.
>> I'm feeling something like mortified.
Didn't realize I look so much like a box.

>> Look what I got. Look what I got.
>> When I looked at myself in the mirror before, my eyes will always go straight to the indentation. So, I hated that.
Now, when I look at myself, what's not to like?
>> I have what is considered to be a regular booty now.
>> Mhm.
>> No indentation.
>> That's not regular.
>> It's very much so above average.
>> The scar that we reconstructed is nice and smooth. Blends in with the rest of her butt. And we maintain her tattoo.
>> Can I ride now?
>> You can ride. You can sit. You can do whatever you want.
>> Yay. That's good. You can do whatever you want, girl.
>> There you go.
>> Thank you.
