[00:00:00]Let's talk clozapine. So, clozapine is an atypical antipsychotic that has, of course, the two blockade.

[00:00:09]And what makes it atypical, um in contrast to Haldol, is that it has 5-HT2A antagonism.

[00:00:21]Well, this means that because of that 5-HT2A antagonism, it will also be less likely to cause EPS uh symptoms, extrapyramidal symptoms, like akathisia, uh we're talking tardive dyskinesia. However, the risk is lower than with haloperidol, which is a typical antipsychotic, or one of the first uh antipsychotics. But, the risk is still there, it still happen, you know, but not as much as haloperidol.

[00:00:48]The thing with clozapine that you must know, if you suffer from bipolar disorder or you suffer from schizophrenia, it treats treatment-resistant schizophrenia. So, studies have shown that clozapine is like one of the It's pretty much like the best antipsychotic that we have at the moment. It really is. Um there's nothing better than clozapine.

[00:01:09]Um that's why clozapine, even though it's a great antipsychotic, it has a lot of side effects. Um the main one is neutropenia adverse effect.

[00:01:19]It can lower your white blood cells.

[00:01:20]What does this mean? Well, you cannot fight infection. If it goes too low, you can die because you won't be able Your immune system won't be able to fight any virus, any bacteria, right? You You get it. You get what I'm saying. So, the other thing is it can cause paralytic ileus because it can cause so much constipation because it does have anticholinergic activity. Um so, it does make you constipated. It also has um histaminergic activity, um especially norclozapine, which is the active metabolite of clozapine. So, keep in mind that all these drugs, they're like think about it like as the father.

[00:02:00]They all have an active metabolite. So, your body decomposes them. Guys, that's a good way or metabolizes them to the actual active drug, if that makes sense. Or let's explain it better. Just like the active metabolite, okay? And the active metabolite can actually have, of course, the same properties as the parent drug or even more. Okay? So, the mechanism of action of clozapine, even though we know a lot, is still not well understood.

[00:02:36]What we do know for sure is that the active metabolite has histaminergic um activity. And the future of a schizophrenia treatment is dealing with musca um muscarinic receptors, okay?
