In women with PMOS/PCOS, multiple positive OPKs in one cycle indicate the brain is sending ovulation signals, but high insulin and elevated testosterone block the follicle from releasing the egg, causing the signal to reset and try again; this means OPK-based timing is unreliable without confirming actual ovulation through a sustained basal body temperature rise of at least 0.2°F for three days plus progesterone levels above 10 ng/mL at 7 days post-expected ovulation.

This Is Why You're Not Ovulating With PCOS/PMOS #pcos #pregnancytipsAdded:
Getting multiple positive OPKs in one cycle is a PMOS pattern. Your brain keeps sending the signal to ovulate, but high insulin and elevated testosterone are blocking the egg from actually being released. So, the signal resets and tries again. This is why OPK based timing in a PMOS without confirming actual ovulation is unreliable.
You can test positive three times in one cycle and not ovulate once. Real ovulation confirmation would be relying on basal body temperature, so a temperature rise of at least 0.2° that holds for at least three days in a row, and a progesterone reading above 10 at 7 days after expected ovulation through a blood work.
Multiple positive OPKs without either of those means multiple attempts resulting in failed ovulation. If you want to learn more about basal body temperature and you don't know where to start, check out the link in my bio.
