Hi guys. Nowadays, bimaxillary facial orthognathic surgery people all over the space, Discord servers, YouTube comments, and forums dream of performing this 20 to 40 grand surgery in order to improve their face. But, do they actually need this surgery? And more importantly, do they even understand what actually makes a good jaw? Because I find that people are completely obsessed with forward growth and having tons of advancement, but they completely miss out on the key angles of the entire maxillo- mandibular structure. This is what actually makes a great jaw. A good gonial angle, a good mandibular plane angle, appropriate lower third height, which will influence other harmony key ratios. Whenever FA strays very steep or flat, you have a problem [music] and it can completely kill your aesthetics.
Now, it's actually important to understand what kind of people can actually benefit from a double jaw surgery. And as you can guess it, it will be people with malocclusions. A common one is people that have malocclusion class two with a deep overbite. Now, what actually causes this? And to further illustrate, I am bringing the x-ray of a client of mine.
As you can see, both his maxilla and mandible is recessed, but this is just the tip of the iceberg. Notice how his maxilla is short in terms of vertical length, and [music] notice the gonial angle is extremely flat, and his mandibular plane is very flat as well.
Now, if you just advance this patient's jaws, his aesthetics will still be far from ideal. His gonial angle will stay low. His mandibular plane will continue to be flat, and more importantly, his overall lower third will continue to be short, which will contribute for that short face compressed syndrome from the front. The solution to this is to combine advancement with clockwise rotation. Now, here's the difference between clockwise rotation and counterclockwise rotation. Now, this patient gained facial height, he increased his gonial angle, his jaw is no longer flat, and his smile will also improve. This is because the lines and the way the teeth show when you smile will all be at the ideal angles. Now, what are the cases that can benefit from a counterclockwise rotation? People that have class two malocclusions as well, but more specifically, this can be done on people with gummy smiles, anterior open bites, and hyperdivergent type of faces, which is just another term for long face syndrome. This typically is caused by excessive vertical maxilla and a downgrown jaw, which will make the face longer than what it needs to. This will affect facial harmony quite a bit.
Typically, when this happens, your facial thirds aren't balanced, your jaw is downgrown, your philtrum also might be long, and your gonial angle is high.
By performing counterclockwise rotation in conjunction with advancement, the patient will basically bring these measurements closer to the ideal ranges.
Now, if you don't have any of these issues, and you think that performing BiMax will give you a tremendous improvement in your looks, then you will be very disappointed. If your facial angles are good, the only plausible reason as for you to get a BiMax would be if your jaws are a little bit recessed, but they're still aligned, and you want to improve your airways in order to avoid having obstructive sleep apnea in the future. But when you actually have a need, then the BiMax with advancement and the appropriate rotation will improve your face like nothing else can. It is truly remarkable. You will have a better facial harmony, your dimorphism will be higher, and your face will be much more angular. This is because the jawline visibility will be a lot higher, and your smile in general will be way more harmonious. I typically find that the guys that have a short face syndrome tend to have the best glow ups after this procedure, but remember, this is just anecdotal from years of observing befores and afters from this procedure.
