Let's quickly understand the relation of vitamin D and chronic pain in body and its importance for physiootherapists.
Osteomalia means weakened bone density and softening of bone caused most commonly from low vitamin D and calcium.
Patients presenting with proximal joints pain in shoulder, hips, lower back and knee joints mainly. There is difficulty in stair climbing and overhead activities in patients due to proximal muscle weakness. On X-ray you might see losers zone which means presence of a pseudo fracture in bone which confirms osteomatia. So for a physiootherapist it is important to understand that pain here is non- mechanical and fatigue is noticed in patient. Such patients progress to chronic and painful conditions very fast such as osteoarthritis, lumbar stenosis etc. if left untreated. NIC guidelines NG115 recommends for such patients to undergo vitamin D, calcium and phosphate screening and get supplements to balance these minerals to improve and in physiootherapy treatment focus more on weightbearing exercises and close kynyatic strengthening first and then gradually progression in strengthening should be done along once vitamin D is stabilized.
