[00:00:06]What started as simple back pain was revealed to be a chordoma, a rare, aggressive tumor that develops from remnants of the embryonic spine.

[00:00:15]Here, we see the total destruction of the third lumbar vertebra.

[00:00:24]2/3 of the vertebral body have been eaten away, a process called osteodestruction.

[00:00:30]The protective bone barrier to the spinal canal is completely shattered.

[00:00:34]Measuring nearly 5 cm, the tumor is pressing directly against the dural sac, displacing the nerve pathways. To prevent a complete spinal collapse and potential paralysis, surgeons operated immediately.

[00:00:48]They installed a massive internal fixator with pedicle screws in L2 and L4 to stabilize the spine.

[00:00:56]But the challenge remained. The tumor was already infiltrating the surrounding muscles and nerve exit points.

[00:01:03]Despite partial resection and radiation, the chordoma remained unpredictable.

[00:01:09]In 2017, it showed only faint, garland-like contrast enhancement, almost like camouflage.
