Pay your respects. Hi, I'm Chloe, a student practitioner. Hi, I'm Chloe.
Nice to meet you.
So, we're going to go through your knee. You came with a knee.
Yeah. We'll talk about that more later.
I'm just going to go through a quick debrief of what's going to happen in this session today.
Um I'll be asking you some questions today.
Like when you did it, how you did it, and what it felt like pain-wise.
Is it affecting your day-to-day life?
That's the subjective assessment.
Then I'm going to go through the physical assessment.
Range of motion of your knee.
Active and passive movements. So, I'm not going to be forcing you into a position or anything.
It's just going to be your own how you deal with it. Um if it hurts at any point, we can stop or if it feels painful, we can stop. And if I have any questions that you don't want to answer, you don't have to.
The range of motion, are you happy with that? Do you want to stop at that?
Do you want me to explain it more? Yeah.
So, I'm going to lie you down or sit you up in the bed and depending what I want to see with your legs, extended or flexed, I'm just going to ask you to do that and then if it hurts, you can tell me where I'll see what kind of injury it might be.
Yeah.
Yeah.
So, everything discussed right now will be remain confidential. I'm not going to tell anyone anything, so it's fine. Um and then I'm going to go through all and see if we can come to some sort of a conclusion about Um is there anything you'd like me to explain again or No, I think I'm good. No? Um just to make sure, I have your consent. Yeah.
Yeah. Okay, thank you.
I just want to um clarify to you that some of the health and safety concerns and the emergency exits. So, we have emergency exit right there to your left, in front of you. We also have one behind you. So, if there is a fire alarm or anything, we can evacuate safely. Um this is an enclosed space, so no one's going to be [snorts] coming in and out, and we'll be safe in this space.
Um is there any concerns that you have? No.
Um also, I'd like to say the beds are all sanitized, and everything's clean.
So, anything that's going to be used today has been thoroughly um cleaned and on the hygiene protocol.
I do want to just make sure I know you've given informed consent already. I just want to clarify that you're okay taking some clothes off if you need to be.
>> Yeah.
>> Yeah, is that okay? Perfect. Thank you.
Thank you to agreeing to this assessment today. I'm going to now proceed with some of the subjective questions that I said before. Um so, I would like to start with how did you get this injury then? What were you doing at the time?
Um so, I was playing rugby. I was running for the try line, and then I got tackled, and then I just felt my knee just pop out, and yeah, I just felt a pop out.
>> Yeah. Okay, so did it go outwards or inwards? It went inwards. Inwards, okay, cool.
Um can you show me where exactly the pain is when you first did it?
Yeah, it was just like a lump on the outside.
Um does this area still hurt specifically in that area, or has it traveled down or upwards?
It does mainly stay in this area, but when it feels more comfortable, I do feel it slightly in my leg as well.
Okay, okay, and when you first did this injury, was there a sharp pain initially?
Okay, okay.
I felt like the initial pain was like a little bit sharp as well. I'm not not too sure about that, but more of a dull pain. And did you stay on the pitch when this happened, or did you come off straight away? Um I did try to play on, but I had to get subbed off, yeah.
So, when you first did um the injury, >> [snorts] >> it was what scale, one being didn't really hurt and then 10 being really hurt.
It was about an eight. An eight? When you first did it. Okay, so that's quite high.
Um So, what's the When is it at its worst and best since the injury?
I'd say it's at its worst in the morning when I wake up. Yeah. Just being in the same position.
I'd say it's at its worst in the middle of the day when I've just been like trying to walk around and do something.
Um So, yeah, you said it was the worst in the morning. Do you think that's because it's seized?
Yeah. Okay.
And then it gets better gradually during the day. Yeah.
Um So, from the injury now to how you've been doing your daily life, is it affected anything? Um I used to like going out for runs, and I can't do that anymore. Okay. Um I haven't been to the gym. No.
Um I can walk, but it's it's slightly Okay. So, yeah, no weight bearing then, no weight lifting, nothing.
Okay.
Um and do you think it stops you from sleeping?
Um I have It has woken me up a few times in the night when I go into like an uncomfortable position, but other than that, I can't >> still You can still get to sleep, it's just the initial trying to move around and get in a nice position. Okay. Um So, it said it is affecting your It's a bit debilitating then.
Yeah. Um Are you currently taking any like medication for this? To help with the pain?
I have tried some ibuprofen, but it's not really >> Not helping. No. And what about icing? I have been icing as well, and that's been reducing it as well.
Any heat? Put any heat on it? No.
Okay, so just icing. Yeah. Um Do you have any relevant medical conditions that could be causing any more pain then?
No. Okay.
Um You said about your injury, how long ago was that?
It was about 2 weeks ago. 2 weeks ago.
And then previous to that, have you had any other injuries of your knee? So nothing at all, no history with that at all?
Okay.
So you said the pain increases when you're running.
Can you tell me Can you tell me more about that?
Um so when I run it just feels like it's going to It feels like it's about to go out of place.
Um it's just really painful. So you haven't even tried really since the injury, you know?
Okay.
Um There's some unusual symptoms that can sometimes happen.
These are being weight loss or stuff like that. Have you experienced any of that? No. Okay.
Um I think I've got a quite good understanding of your injury now. Um I'm now going to move on to a physical assessment just to know your range of [clears throat] movement and see if there's any deeper underlying things that I need to know.
And you said about your history, there's no You have no asthma, nothing? Okay, perfect. Thank you. Thank you.
Sorry, just have a quick couple of questions before we move on to the physical assessment. Just want to understand your physical demands. Like what do you do for a job and what's your typical work day look like? Uh I work in retail. Um a typical day would be um bending over, picking up boxes, um putting some clothes out, uh doing some maintenance, so a lot of bending, squatting. Okay, perfect. So you'd say you're on your feet quite a lot of the day.
Okay, and does it Like you said it involves lifting, bending, standing. And since having this injury you haven't been able to do that. No, it's My movement's been very restricted.
Um managers have moved things to different places to try to accommodate me, but Yeah. So, it's very inconvenient that you have this injury. And you've taken any time off work or Um I took a couple of days off, but um I thought it I'd be fine to go back, but it is not working.
Um so, there's no periods of the day that you're sitting down and resting then if you're at work.
And um when you're at work, your symptoms do increase. They get worse.
Um And like you said, there's you go up and up like stairs. Yeah, quite a lot. Okay, and then from what you've just said, when would you like to return to your sport or what's your goal?
Um I would like to preferably return within the next couple of weeks.
Um I'm just bored doing nothing. So, I I just want to go back to the game.
Okay, we'll try and underpin what's happening, and then we'll try get you a proper assessment done and see what's going to happen.
Hopefully, you can get back to the sport in a couple weeks. Thank you. No worries.
Okay, perfect. So, I'm just going to start off by asking you to take your shoes and socks off so we can go into the physical assessment. Is that okay?
Yeah, that's fine. Perfect. I am going to just ask you some instructions that you can carry out once you're in the first no pain, just to apply and see if your range of movement is decreased or anything.
Okay. Yeah, perfect. So, I've just asked you to lie down in supine, which is back to the bed, which is brilliant. I'm going to just see how your willingness is to the range of movement if there's any hesitation or anything. So, I am first going to start to ask you, can you um flex your knee for me? So, we're just going to see if there's any um hesitation or pain in the quadriceps.
So, Wait, have I said please to you sorry for so long? Thank you for letting me do the exercises. I hope it's helped you a little bit. I've gathered all this information and hopefully come to a clear understanding that it is a grade two injury.
Hopefully in the next couple of weeks you can try and prove this and get that range of motion back cuz there is some hesitation there. So, hopefully you'll be able to play your sports as soon as possible.
