For optimal results, diet should be the primary lever, with high-intensity interval exercise added as a secondary lever.

Statins vs. Foods vs. Exercise: Which one UNCLOGS ARTERIES Better?Added:
My friend Rob died on elliptical at 58 doing highintensity exercise training for a mountain climb. He was similar age, super fit. He's a great person whose life was cut short suddenly. It's why I'm doing all these videos for you to prevent a similar fate.
Today I'm going to rank statins, exercise, and diet on the published evidence. I'm going to tell you the one thing nobody told Rob. I'm Dr. Kevin Ham and this is the reversal equation. R= T * D. Reversal equals tap off which is turning the LDL down below 70 and drain on. This is the reverse cholesterol transport system pathway turned on to reverse plaque. Both of these needs to be solved. So let's score each lever in this equation. So lever one is statins.
In the meteor trial in jamama 2007 there were 984 subjects on rua statatin which is a highdose statin 40 mg per day 2 years LDL dropped from 155 to 78 49% reduction the wall thickness changed by a negative.4 mm per year. That's essentially zero.
Why? because LDL was 78 which is above this threshold number of 70 where reversal starts to be a little more significant. So the tap isn't quite turned off enough and statins don't open the drain. So the score on the statins are tap is still partially on and the drain is still plugged. So the reversal plaque is halted and it didn't reverse meaningfully. So press pause for a moment. So, let's look at the next lever, exercise, and compare how exercise reverses plaque. Kardashian Aussi in 2020 in Iran. They had 74 sedentary type 2 diabetics, 12 weeks of highintensity interval training. The high intensity were 4 minutes long. They did six intervals or six times and the intensity was at 85 to 90% of maximum heart rate. So out of breath croted wall thickness fell from 83 millimeters to 71 millimeters. This was a 14% absolute reduction. Statistically significant. So a highdose statin over 2 years produced essentially zero regression.
Highintensity interval training over 12 weeks produced 14%. Which is about 80 times more effective on the wall on a per week basis. The score on highintensity exercise, the tap is essentially still on. Why? Because LDL only dropped by 7 mg per deciliter.
Modest drop if you have high cholesterol, but the drain was turned on. So Stanton in 2022 in the journal of the American Heart Association showed 12 weeks of moderate exercise raises ABC1 cholesterol elux 13.5%.
So, exercise opens the drain. The wall regresses. But why did exercise kill Rob? He was on the elliptical. Now, the part nobody told Rob. Exercise alone does not reverse plaques. Exercise activates the drain. Removes cholesterol out via HDL. But if the tap LDL is not turned off, if your LDL is still 160 or 200 and 250, it's oxidized, the drain is open, creating more plaque into a system that's still being flooded with new plaque from above. So highintensity exercise in the wrong body chemistry is the thing that likely led to Rob's demise. His LDL was actually not that high. He was on a statin. It was about 80 to 90. His wall was likely inflamed though. The intervals he did on the elliptical in that body chemistry with inflammation won't activate regression.
It activate rupture of plaque. So the right body chemistry is the soil and it's important. Then the exercise is the seed the accelerator that allows the plaque reversal. But if the the soil is not right, the exercise will flame the plaque and increase the chances of rupture. So let's look at lever 3, diet. This is the most powerful lever in the entire medical literature. Dr. Cowbell Esselton at the Cleveland Clinic in 1985, 22 patients with extremely severe coronary artery disease. He educated them each week and they committed to a strict whole food plant-based diet with no oils or fat, 10% fat, and 12 years of follow-up. They had 49 cardiac events in the 8 years before joining. After enrolling zero vents in 12 years in all the adherent patients who followed the diet, 11 of the 25 lesions regressed. The mean obstruction fell from 53% to 46%. And Esselton did another study followup in 2014 with 198 patients with established cardiovascular disease. 89% were adherent for 3.7 years. That's astounding. He really focused on educating them so that they would be empowered and motivated to continue the major cardiac event rate just 6%. Now compare the cardiac events in the forier study where they had a PCSK9 with a statin. They had 9.8% over 2.2 years. And let's look at Joe Crowe specifically who refused statins and only did diet alone. His LDL dell to a dramatic 38. His proximal L was 90% and it regressed all the way to zero in just 32 months. Complete reversal. So when we look at this, the top is fully closed. The LDL is in reversal territory. The drains are open with diet and exercise. And especially in Dr. Joe Crow's case diet alone among the levers can solve both of these reversal equations simultaneously when run at full strictness. Then a question of dose and intensity of exercise matters. I produced 53% crowded regression in just 90 days because I stacked all these levers across to turn the tap off and the drain on. But I didn't take statins.
I haven't not yet. Esselton diet anchored to tap off lowering LDL beautifully and activating enos which is endothelial nitric oxide synthes preventing further plaque hit highintensity cycling layered fasting pomegranate juice nattokynise and all these other strategies I employ these led to this rapid reversal in my croteds at full strength simultaneously I just rode a grand fondo 113 miles I have three more fondos a half marathon tripling, quadrupling down on a highintensity exercise. If you can run one of these levers or strategies, choose diet. It hits both of these reversal equations hardest. And then if you want to add a second lever, add highintensity interval exercise. But run diet first or you can have the same fate as Rob if you're still inflammatory and exercising. If you can run three, four or five of these levers, then you become the patient the literature does not yet have someone like me with rapid reversal. So reversal equals tap off, lowering your LDL below 70 if possible or Apo B under 55 and turning the drain on. That's a reverse cholesterol transport.
