The drug then reaches its peak effect in the middle of the night. The kidneys excrete fluid.
The bladder fills. The patient wakes at 1, 2, or 3 in the morning and gets out of bed in the dark with blood pressure that shifts when they stand up. This condition is called orthostatic hypotension and it affects nearly 30% of adults over 70. What happens next should stop every cardiologist and geriatrician in their tracks. The National Institute on Aging reports that nighttime falls in adults over 70 are the leading cause of hip fractures. and hip fractures, listen carefully to this number, carry a one-year mortality rate of between 20 and 30% in patients over 70. One in every four to five people who fracture a hip after the age of 70 will not be alive one year later. That is the real consequence of taking your diuretic at 6:00 in the evening instead of 8:00 in the morning. A Brighamin Women's Hospital analysis affiliated with Harvard examined 14,872 patients receiving diuretic therapy. Researchers at John's Hopkins University tracked 8,211 patients with hypertension over five years. Those with documented dose doubling events after missed doses had a 38% higher incidence of emergency hypertensive crises compared to those who maintained consistent single dosing.

As cortisol levels increase, the sympathetic nervous system activates and your body prepares for the demands of the day. This morning surge is the single most dangerous cardiovascular window in a 24-hour period. It is when the majority of heart attacks and strokes occur. Now, think about what a morning dose actually does.
You take your medication at 7 or 8:00 in the morning. It absorbs over the next 1 to two hours. It reaches peak blood concentration around 9 or 10:00 a.m.
Just as the morning surge is already subsiding. By 4 or 5:00 the following morning, when the next surge is building again, the medication has been declining in concentration for 18 to 20 hours. The window during which you need the most protection is precisely when your medication is at its weakest. A bedtime dose changes everything. Take your medication at 10 p.m. By 4:00 a.m. it is at or near peak concentration. It is fully active during the morning surge.
The surge meets a pharmaceutical wall.

This is not speculation. The Hyia Chronotherapy trial, the largest study ever conducted on blood pressure medication timing, enrolled 19,084 Spanish adults with confirmed hypertension.
