Hyperacusis is a condition where everyday sounds like coffee grinders or slamming doors cause actual pain, caused by a malfunction in the auditory brainstem's gain control mechanism that normally regulates loudness perception; this occurs when damaged outer hair cells in the cochlea fail to compress loud sounds before they reach the brain, causing 70 decibel sounds to trigger the same neural response as 100 decibel sounds, which the brain interprets as pain, and this condition frequently co-occurs with tinnitus, affecting up to 40% of people with chronic ringing.

Why everyday sounds can feel painfully loudAjouté :
If a coffee grinder or a slamming car door makes you flinch in actual pain, that's not sensitivity.
That's a named condition called hyperacusis.
Inside the auditory brain stem, neurons that translate sound pressure into loudness use a gain dial, and in hyperacusis the dial gets stuck at maximum.
Researchers link the shift to damaged outer hair cells in the cochlea, which normally compress loud input before it ever reaches the brain.
When that compression fails, sounds at 70 decibels can fire neurons the same way 100 decibels would in a typical ear, which the brain reads as threatened pain.
It often shows up alongside tinnitus, and in studies up to 40% of people with chronic ringing also report this loudness recruitment pattern.
