Hearing Aid Dome Trapped by Infection & 91-Year-Old's Impacted Dry Skin Build-Up – EP1268追加:
Coming up in today's video we've got a referred patient from a pharmacy and we've also got a patient who has a sign of an infection after having their watch removed.

My name is. Taylor Green I'm one of the audiologists here at Audiology Associates. My name is Rhys Barber. I'm the other audiologist here at Audiology Associates. So this is this is my patient now this lady who went to the pharmacy they had one done so one was cleared by the pharmacy but struggled to do this side and I'm not surprised because it is solid.
Let's say solid it's it's a harder kind of dry skin it is so I've had to resort to the the ear hook the right hook sorry right hook right hook.
Tried suction for a couple of seconds but I could already kind of feel on the suction tube but it was not really going to go anywhere so that's why I've kind of swapped over you can see it's all kind of unravelling. I was going to say it's that unraveling wax has got a hard underlay and they just fold out. Yeah and then it's got this dry piece there and it's just just kind of loosened sitting there behind all this uh dry skin so I'm just trying to just get beyond that little gap there there you go.
Oh I was kind of hoping I was going to bring the whole thing forwards. You can see it feels quite tight though and and tough I meant to say not tight in there because it's uh it's just flowing down the side of the gaps isn't it really it's just not really not really lifting.
Oh uh blessing this patient was 91 years of age.
91. 91. They were lovely as well literally sat through the whole thing they were like a trooper to be fair.
We sometimes well I say sometimes we pretty much see this all the time don't we with you know patients that are in their sort of late 80s 90s maybe a little bit younger than that as well um a lot of dry skin but I know we've mentioned a few times it's just where the ear kind of stops producing all the oil the natural oils in the ear to you know it keeps helps to keep the ear nice and you know moist and um yeah it just kind of dries up and this is what you kind of end up with then is these these very thick layers of like keratin.
Yeah right at the entrance so just go in about. I don't know a couple of mil and it it sort of really it stops a lot of the wax it kind of almost like it becomes a bit like speed bump for wax so it kind of hits that and doesn't really it's not even good to get past it and it just sits there and dries out a bit and then everything backs up in the system from there then. Yep so we've got the forceps here now just trying to remove all these kind of loose bits near the entrance that's a nice uh nice little piece we got there I can see this a little bit at the bottom I'm just trying to hold the ear open ooh gotta go for this piece. I think now just delete yeah let's go just ask right in front of the wax sometimes it makes that clarinet noise and sometimes you're lucky and as it bunches the sides in it'll it'll get a good suction grip and it starts peeling away quite nicely so that's yeah so far so good not the biggest of ear canals is it no you look at the size of the suction tube there compared to the the kind of diameter of the tube compared to it's almost filling that you cannot when it goes in yeah it's pretty tight there to be fair because there's a little bit of dry skin you know which is yes yeah i'm trying to go beyond it to get that wax you can see it's just lifted it's just following the suction tube sometimes if you can get a grip on the wax as well as that bit of skin as you pull it it pulls the skin with it as well than that yeah like it's quite loose that bit behind so I put a little bit of oil in now just to try and get this bit of skin away good idea sometimes it helps to shear away that skin a little bit easier oh you can see this so much as it's holding on it's all kind of stuck right up the side of the ear canal here you can kind of see as that skin is peeling the oil is kind of following that peel and it's getting it's you know it's allowing us to get it away more comfortably so I mean it's not uncomfortable but sometimes you know you have to mention to a patient when we're peeling skin away especially really dry skin we always mention there might be a sensation of like pulling yes that's just the skin kind of peeling away but you know some some patients don't like it I mean I don't I'm not saying they don't like it but you know it just feels really weird it's a strange it's a strange I always say to patients you're going to get a bit of a pull you might feel it might feel a little tiny kind of scratch as it comes away at the end but that that's it it's not not not ridiculously painful it's just a little bit of a bit of a discomfort and it lasts a split second so yeah it's solid isn't it yeah it's like a nest it's full of hair as well yeah there we are lovely I had to use the forceps to pick that one up I'm not surprised as well gosh a bit of a mixture there we've got a engine a lot of water yeah I'd probably Simon every leaf left a bit of a gap in the middle there probably by three centimeters there that one oh there you go I didn't know we were doing this one either and so yeah foreign body here this is Katie's video so this is a hearing aid dome where you can see just just inside the ear canal there that's a tight one doesn't happen too often you know but you do get it every so often best tool to use is exactly what she's using here I think she attempted it with a with a right hook but we've got that nice central section this is the infection patient yeah because you can see those canal walls have sort of come right in um I should have watched the video before we yeah there you go you can kind of see what's I mean you can almost kind of guess what's happened here you know as patients uh almost had an infection yeah um bit of swelling as uh has caused the ear canal to almost kind of close up a little bit yeah patients pop a hearing aid in and where the dome has got beyond that that sort of swelling as patients try to pull it back out it's just pulled away from the hearing aid that looks that looks narrowed isn't it look at that you could see that vacuum yeah yeah I would have left that she tried to do it but you could just see where it just started to collapse in around that tube unfortunately um so normally what we've got is a really good relationship with a prescribing pharmacist so we can actually they send a lot of their ear stuff up to us and if we get anything like ear infections into that because it is a bit more difficult at the moment to get a GP appointment isn't it here in the in the UK so we'll send our patients down to a prescribing pharmacy book the appointment there and then while they're here and send them down uh and they can get the the treatments usually the same day or if not the same day usually the following day whereas like I've tried to get a GP appointment recently we tried to get a GP I think it was a photo it's four eight o'clock in the morning together the point but and then they said oh yeah is it urgent I said no that's just routine appointment oh yeah you got a phone back tomorrow for a routine appointment so they get released once a week and you go to phone on that day to get it so I was like oh it's ridiculous um so yeah very kind of sticky wax so Katie opted to use some irrigation they just to flush that last little bit out um and now she's very wet and sticky it is there you can and this is what was left over after irrigation you can see because it always softens the edges of the wax so it becomes really mushy on the edges near this kind of hard a bit in the middle um so yeah she's done really well there to get those out let's just peel in these last couple of pieces I think this was the first wax remover as well oh it was it I did say on the note yeah she's just aiming for this last piece you can see the humidity in the canal that's just what's left over after the uh irrigations being done she's like I'm gonna get up I know you can see she's really determined to peel that last piece off and she oh here it comes lovely that's almost almost there oh is she gonna be able to get it or is it just gonna sit on the canal I don't know it's going it's going it's nearly there there you go yeah I made a big old difference to get that out I think there's a little bit down here that she's aiming for that's more just sluffy kind of sludgy wax that's just left over that would actually come out with your next bath or shower as well but I don't know got out there no problem at all yeah obviously no ruler shot there because it was so wet and sticky yeah exactly uh well guys thank you very much for watching our video today I hope you enjoyed
