[00:04:44]The top number is called systolic pressure. This is the force your blood pushes against artery walls when your heart beats, when it squeezes.

[00:06:45]We now know from massive clinical trials that sustained systolic pressure of 170 in a 70-year-old dramatically increases risk of stroke, heart failure, kidney damage, and cognitive decline.

[00:06:59]That old formula gave millions of people false reassurance while their organs were being silently damaged year after year.

[00:07:07]But the correction went too far in the other direction.

[00:07:10]In 2017, the American College of Cardiology lowered the definition of high blood pressure to 130 over 80.

[00:07:19]Overnight, millions of previously normal adults were reclassified as hypertensive.

[00:07:25]Many were over 60.

[00:07:27]Many felt perfectly fine.

[00:07:29]Many were started on medications they may not have needed.

[00:07:33]So, what is genuinely too high after 60?

[00:07:37]The most important study answering this question is the SPRINT trial published in the New England Journal of Medicine in 2015.

[00:07:45]Over 9,300 participants, the largest and most rigorous blood pressure treatment trial ever conducted.

[00:07:52]SPRINT found that targeting systolic pressure below 120 reduced heart attack, stroke, and cardiovascular death by 25% compared to the traditional target of below 140.

[00:08:06]And in adults over 75 specifically, the benefit was even more dramatic. A 33% reduction in cardiovascular events.

[00:08:14]But, and this is absolutely critical, aggressive treatment came with real risks. More fainting, more electrolyte imbalances, more kidney complications, more falls.

[00:08:58]Here are the numbers based on the SPRINT trial, American Heart Association guidelines, and 22 years of clinical experience treating blood pressure in older adults.

[00:09:08]For adults 60 to 75, systolic below 130 at rest is ideal.

[00:09:13]130 to 139 is acceptable if you feel well.

[00:09:18]140 to 149 needs monitoring and lifestyle changes. And 150 or above consistently at rest, that is too high.

[00:09:29]For adults over 75, below 140 is the general goal.

[00:09:34]140 to 149 depends on how you feel and your fall risk. 150 or above consistently, too high regardless of age.

[00:09:44]Diastolic below 80 is ideal. 80 to 89 needs watching. 90 or above consistently puts damaging strain on arteries and organs.

[00:09:54]Now, the hidden number I promised you. A third number you can calculate from any reading in 5 seconds.

[00:10:00]Research shows it predicts heart attack and stroke more accurately than either number alone, especially after 60. It is called pulse pressure.

[00:10:09]Pulse pressure is the difference between your top and bottom numbers. Reading of 150 over 70, pulse pressure is 80.

[00:10:18]Reading of 130 over 80, pulse pressure is 50.

[00:10:22]Why does this matter so much? Because pulse pressure directly measures arterial stiffness. Stiffer arteries push systolic higher and let diastolic drop lower. The gap widens. And that widening gap is one of the clearest cardiovascular danger signals in older adults.

[00:10:40]A landmark study in the journal Hypertension, using Framingham Heart Study data, found pulse pressure was a stronger predictor of cardiovascular events than either systolic or diastolic alone in adults over 60. Stronger than either number by itself. Below 40 is normal. 40 to 60 is acceptable, but worth monitoring. Above 60, warning sign of significant arterial stiffness and elevated cardiovascular risk. If your reading is 160 over 70, most people focus on the 160, but pulse pressure is 90. That is a screaming alarm that your arteries have lost critical flexibility and your heart is working dangerously hard.

[00:11:38]A study in the Journal of the American College of Cardiology found that adults over 60 with diastolic consistently below 60 had significantly increased risk of heart attack because pressure between heartbeats was too low to supply the heart muscle adequately.

[00:11:54]I had a patient I will call Walter, 74 years old, retired mechanic from Tennessee, on three blood pressure medications. Systolic controlled at 128, but diastolic had dropped to 52.

You walk into the doctor's office, check in, walk down a hallway, sit on the exam table. Within 60 seconds, the nurse takes your reading.

[00:13:49]A study in hypertension found blood pressure taken without a 5-minute rest was 10 to 15 points higher systolic than after proper rest.

[00:13:58]That is the difference between 138 and 148, between acceptable and too high.

I know that sounds wrong, but let me explain. You check your blood pressure at home and see 170 over 95. Your first instinct is to panic, call 911, rush to the ER. In some cases, that instinct is correct, but in many cases, that is a temporary spike from stress, pain, caffeine, or anxiety about the reading itself. So, how do you tell the difference between a spike and an emergency? The answer is symptoms. A high number without symptoms is a signal to monitor. A high number with symptoms is a signal to act immediately. Here are the symptoms that turn a high reading into a medical emergency. If blood pressure is above 180 systolic or above 120 diastolic, and you experience any of these, call 911 immediately. Severe sudden headache unlike anything you have felt before.

[00:18:05]Chest pain or tightness. New or worsening shortness of breath. Vision changes, blurring or partial loss.

[00:18:12]Difficulty speaking or sudden confusion.

A study in circulation found that hypertensive crisis carried a mortality risk of up to 25% within the first year if not treated promptly. One in four. But here's the other side. If your reading is 170 over 95 and you feel completely fine, no headache, no chest pain, no vision changes, that is not an emergency. That is an urgency. There is a medical difference. Sit down, rest 5 minutes, breathe slowly, retake your reading. In most cases, the number drops 15 to 25 points within 10 minutes. If it stays elevated after rest, contact your doctor that day or the next morning.
