Idiopathic Pulmonary Fibrosis (IPF) is characterized by classical Usual Interstitial Pneumonia (UIP) pattern on HRCT, featuring honeycombing (abnormal rounded areas of air) and reticular opacities; while UIP is less common in females, it cannot be ruled out based on gender alone, and the initial treatment for IPF is the antifibrotic medication pirfenidone.

elderly progressive dyspnea female and on the CT scan abnormal rounded areas of air What is this? Honeycombing.
Honeycombing.
Now, if you see this in combination, if you see this in combination, this is possibility of UIP.
Even though in females UIP is not common, only by seeing female, you cannot say it is not UIP. It is combination of everything. Elderly UIP is possible. Radiological feature is classical UIP. Because it is classical UIP, when you don't know the etiology, it is called as IPF, idiopathic pulmonary fibrosis.
In this situation, the answer is pirfenidone.
Initial treatment is antifibrotic pirfenidone.
