Geriatric medicine is a specialized field that focuses on individuals aged 60 years and above, requiring comprehensive care that addresses not only physical health conditions like hypertension, diabetes, and dementia but also emotional and psychological support. Unlike general medicine, geriatric care emphasizes treating the whole patient rather than just diseases, with a focus on promoting healthy aging through lifestyle modifications, proper nutrition, and social support. The field recognizes that elderly patients often have multiple chronic conditions (multimorbidity) and may be on multiple medications (polypharmacy), requiring careful assessment and coordinated care. Effective elderly care requires fostering social responsibility, empathy, and respect, as seniors serve as guidance and motivators in society.

Dr. Got a wrong hospital.
So in observance of the world red cross day today we are here in the shalong law college observing this day and here uh we are with us Dr. Dr. Dr. Kit Dingan he is a specialist in geriatric medicine practicing in the Shalong uh in Dr. H Gordon Roberts Hospital Shalom. So here we are with uh Dr. Dingan at least to highlight to us a little bit about geriatrics. Yes doctor thank you so much for being being with us here this uh day and observing this world red cross day. Uh so doctor I would like to inquire from you what is uh geriatric medicine?
>> Yeah first of all uh I just want to thank you uh for the red cross team and then the shriang law college and especially the mali localities from bakila inviting me.
>> Yes.
>> So first of all like what is geretrics or senior citizen?
>> Yes.
>> So mostly geretic medicine is we deal with 60 years above.
>> Okay. So JIC patient is usually a person above 60 years and all and then who have like what to say like uh we need special medical care and then like emotional support also for them. So and then like for all the patient mostly we treat only the disease but for all age we have to treat them not only the disease we have to treat them emotional >> and then we have to make them like uh what because many patient of generic medicine they have multimurbbits which is like hypertension diabetes they'll have this dementia then they'll have this psychological problem and all they'll have this incontinence and they'll have this frequent falls also >> so first of all we have to focus on that also not only like just treating the disease actually we have to treat the patient means not only the disease we have to treat them emotionally and then by psychological support also so >> okay so your presence here today in this camp uh what is the purpose of your presence here in this camp do you think being here in this camp today would be able to you know bring an awareness of uh caring for the elderly >> yes yes uh being here today is like what do you say I'm I'm grateful full.
>> So actually like yes we have to give awareness for the old people especially many old people especially if you think like they'll just bring with all the medication >> like what we call that is a poly pharmacy like the more than five medication five medicine I means like so we have to summarize all those medication sometime what they take we don't know because some old people they'll come with just complain of giddiness or they'll have like they can't eat or something then we don't know we we We just can't just treat the disease. We have to observe the patient.
We have to see all the medication. We have to summarize and like we have to give awareness about them because sometime just because of hearing, >> vision. So they'll have many complication. They'll have this history of falls and all. So that will be a great problem for the old people. So I think we have to spread awareness especially about uh about our senior citizen. So I think this is a much needed awareness for the old people >> because it is in you know it is inevitable that as we grow as we age we will grow >> weaker and then the system the entire physiological system will get weaker.
>> Yeah.
>> So how do we how do we go about in maintaining that health delaying all the you know the health problem.
>> Yes. So that is a nice question actually about old age. So that that is the thing like uh when we speak about old age about our senior citizen. So because uh senior citizen is not like just uh what to say as we think like as I mentioned before just treating the disease because first thing is what is our main is like we have to promote healthy aging.
>> Yes. Yes.
>> Yes. So and then one thing is like we have to prevent them from any like any other falls especially in all those things and then old people they'll progress to dementia also. Yes, they'll harvest that is very common.
>> So the thing is that like the what to say like we have to not only that also we have to provide them a pro what to say proper nutritional and psychological support also that will pro make them a healthy aging. So that is the most awareness we have to spread >> and the theme this year it says fostering social responsibility empathy and care.
Can you a little bit elaborate on this theme doctor? Yeah, mostly because uh old age is like I have already mentioned we need not only the medicine >> live about medicine.
>> Other things we have is psychological support even empathy also as they grow old we have to provide them love.
>> Yes.
>> And then our what to say our uh what do you say our support not only just simply by treating the patient we have to make them we have to make them comfortable.
First thing we have to respect them, we have to love them and then we have to treat them accordingly.
>> So with regards to gender like male or female do you think the the the you know the the aging process does it starts early with the female or does it starts early with the male? Because why I said that because as female we experience menopause and all that. So does does does the these health pro problems come earlier on a female rather than on a male?
>> Ah according to me I think that is uh something what to say I can't comments on that exactly but as uh on my opinion what I think is that it depends on how you how is your lifestyle modification.
>> First thing is our lifestyle.
>> So that is the main thing it play a very important role in our healthy aging.
>> Yes. Yes. So because we can't say like a female because of postmenopausal she'll be like develop she'll become what do you say she'll age more than male so aging faster aging faster than male or something. So but but uh as sometime it depends if we think about our older uh senior citizen because uh it depends on our education also some people who is illiterate they are prone to develop dementia or Alzheimer disease very early. If you compare with this education means uh educated >> people >> educated people.
>> Um what about lifestyle diseases? It's very common lifestyle diseases. So is is there a difference between people living in urban area and rural area regarding the elderly?
>> Yeah, mostly they'll have a difference because uh if you think about uh if you go to our rural areas and all. So mostly they'll be they'll having this fresh vegetables and all.
>> Yes. They'll have their own like no pesticide, no chemicals.
>> So, and then they'll be involving with many people of the neighbors and all they'll be what they'll have their support also is very good. Social support.
>> Social support. Yes. Yes. More than >> any other things for them.
>> More than the urban setup. Yeah.
>> So, in our urban setup mostly we don't mix that much.
>> Mostly they'll be staying at home. There no space for the senior citizen. Yeah.
Only watching TV or staying inside. So, so there is no what do you say like a special park or something like for the senior citizen to just to walk or something hang out or not. So it plays it has a difference.
>> So regarding these lifestyle diseases like you know diabetes and hypertension which is very very common.
>> So how do the elderly you know go about in in controlling them?
>> Yes that also is main thing. So most thing is that lifestyle >> that is why we need the lifestyle modification for them. What they eat is matter the most especially when you grow old we need not only because uh if we speak about genotic medicine as we grow old is uh the old people they'll have this reduced muscle mass and muscle strength also as they grow old. So >> we have to prevent those thing that is called sarcopenia.
>> Yes. So those thing also we have to prevent we have to make them exercises and all that also is play very important role in diabetic patient >> in hypertension.
So most thing is that we have to lifestyle modification we have to implement on them and we have to guide them and we have to treat them also because for all people with hypertension with diabetes it can cause many other chronic disease. So from hypertension it can cause stroke it can cause renal failure also.
>> Yes.
>> So it's difficult.
>> So when it comes to support like the theme said you know fostering social responsibility, empathy and care. I don't know. I'm not aware whether in the urban setup if there is an you know a place where the elderly can go and hang out and have like you said the social support and then the empathy and the care where they you know they meet together you know and socialize and share their their views and their likes and dislikes and all that. I I I'm I'm wondering if this would be a benefit you know for for our city to have such a place because now we have they have hangout place for young people and all that. What about a hangout place for elderly people where you know it will help in maintaining their their health condition?
>> Does that uh absolutely we need that?
>> Yes.
>> So only for young people we can't just say like only for them. So mostly now as we what to say as as we see now many uh senior citizen they are living longer.
>> Yes.
>> And then as days by days come we we need special place for them.
>> Yes.
>> Because this are what you say they are our guidance. M >> we can't just neglect them after they are growing older.
>> So we need they need our support and then I think from my side yes absolutely we need the space for them to spend time between the older people they can share their emotional they can share their problems so that is needed. Yes, because I've seen like in the in in in in our time in our time with social media and you know exposure I've seen one case like a lady at her 60 plus she has she was having back pain and all that and what she did she goes to the gym and do you know strengthening and all that and she got back her her health. So I was wondering like I said earlier we we might have sites facilities >> perhaps >> so that we can you know maintain and support these elderly people.
>> So other than that what are other things doctor that you would like to highlight regarding geriatric uh you know care for elderly people?
>> Other thing is that I just want to conclude and other things what what about as you say now like what is the other things for the older people. So first thing is that for older people what we do is that we have this comprehensive geretric assessment also.
So we have to do assessment everything.
>> Okay.
>> Not only some patient because all the people they'll just come with I'm having back pain.
>> But we don't know what is the other problem because the old people they can't sometime complain everything.
They'll just focus only on the main complaint.
>> Okay. Only on the main sympto.
>> Only the main symptom. Yes. So the thing is that we have to >> what we do is that we have this comprehensive generic assessment. So that is we deal with all the what you say all the assessment for all the people.
>> Okay.
>> And then next thing we have this falls clinic also even for this those people who is having repeated fall also how to prevent fall again.
>> Yes. Yes. Yes.
>> What we have to do in the suppose sometime old people they'll come up in middle of the night.
>> We don't know what is going on. They'll just fall.
>> Yes.
>> So that also we have to prevent. And then other thing what about this mobility balance also we have to >> yes >> make comfortable for them and then even memory clinic we used to have >> okay >> so this is available in >> yeah that is like actually I'm trying to start all those things >> okay that that's great >> so because that is the main thing because we don't know how is like the older people they'll have that forgetfulness and all history of all those things so we don't know so we have those clinic and all then we'll what you say we'll screen those people whether they are having risk of any dementia any memory problem at all.
So what is the age that we have to look out you know for for senior citizen like if they say 60 or maybe a little earlier for these uh symptoms or these signs that they are developing some uh some problems >> mostly in our in India actually 60 years but uh is uh more than 60 years or 60 years is for >> jotic patient so if it's before that or earlier than that then also we can screen >> okay but this problem might have uh develop earlier than that doctor should they is it gradual or is it suddenly reaches because yeah because uh if we talk about this dementia and all and then it's not that only one type of dementia is there >> there are other things like people who is having mostly those people who is having stroke earlier than that they'll have risk of that also that will be more prone then those people who is normal >> okay >> and then those people who is having genetics and all those thing also will have more prone to that things you know >> yes so in conclusion doctor why is Is it important for the society, the family, you know, to take care of elderly people?
>> Yes. Uh it's very important. Yeah. In conclusion, as with uh because uh as uh what to say our senior citizen or our elderly, it's not simply elder.
>> They are all our guidance. They are all our motivators.
>> Yes.
>> So they have shown us what to do >> means how to build up a society. So I think it's very important that we respect them, we love them and then we provide empathy >> that is very important. Yeah.
>> I hope we will not grow to that society where we just throw our elderly to the mercy home.
>> I hope we will not grow but I hope that you know we'll have we'll continue to have a society that we care and like the theme said they have empathy for our healthy. So doctor, thank you so much for your time and thank you so much for your expertise and I hope that God will bless you in you know in your >> Thank you so much Dr. Law College Health Red First day.
