Time taken for electrical impulse to travel from seno arteral nodeal node AV node fibers and you see so when it has produced here the wave comes down here and it comes this side it's an electrical system and it comes again this side as so that is PQR PQR the ve is still traveling.
That's why when the wave reaches here, the probe that is here, what does it do?

Is it something that you were born with? You know, congenital anomalies. You remember we talked about congenital anomalies, septal disorders. You know, we divided them into two. Cyanotic and nonyanotic.
When I'm talking about cyanotic, I'm talking about with without those heart conditions that are making your body lose oxygen and asotic those heart conditions that you have them but oxygen is there. So we divided them into septal disorders. We divided into tetrology of phot there is something total anomalous pulmonary anomalous you remember those please check that video and update yourself or I can repeat if you have a question. So types of based on ECG finding mostly the first one types caused by this electron probes that we ECG findings are the ones that check the electrical system. So when we talk about type based on the ECG finding we say A is ST elevation myioardial infunction.
So ST elevation myioardial in function.
And then we have the other one B non ST elevation myioardial infunction. So this one is we are basing them on this. If you have not got it please let me repeat it. We are basing it on ST elevation myiocardio infunction and SD elevation myioardial infunction is I've explained how the blood comes. So this side will not contract well or if you have it here the contraction as it relaxes will not be good. There are some people that are born with something that is called the Kent bundle here.
So you these ones they have takadia because when this one is positively charged it is passing electrical system instead of it coming the normal way it turns like this and that's what we see on the ECG and we define that this person has this problem. So let's talk about something that is very important. Why do we divide it as ST elevation and non ST elevation? And I've say if you look at this one is normal and this one isn't normal because you know our our ST has gone up but this one on our electroc cardiogram has gone up.
So I have defined what is I have defined what is myioarding function. So what can make your heart muscles die? And then we have an ST elevation.
That is the first thing we need to talk about. ST elevation mioardial infunction or ST elevation heart attack or non ST. So we are on ST elevation one. This is it. Coronary artery muscles. Coronary artery block. Coronary artery block. This heart of yours, this heart of yours has got something that is called coronary artery. And what do they do? They are somewhere here. Let me put this coronary artery. You remember what I said? They come back to the heart to help the heart to get oxygen, the muscles of the heart to get oxygen. So when the blood is being pushed away from the heart, some of it comes back to the muscles of the heart through the right coronary artery and the left coronary artery. You see when this coronary artery are blocked part of your side of your heart will not get oxygen. So one that can make the ST elevation cor is a coronary artery becomes blocked.
So when this side is is charged it loses energy and if it has contracted it loses energy to come back to normal. So it comes slowly and that's why you get an elevation.
But if it is normal you will not get an elevation. It will contract and relax and come back. So normal you see st the another thing that can do is the affected mioardium re receives too little oxygen is chemia little oxygen. Another thing is that the constriction of the right let's say we are dealing with the left coronary artery you get that side it's receiving too little oxygen and what can happen maybe you've been smoking and the cells have been destroyed maybe the coronary arteries that are bringing blood are constricted and blood that is going there is not enough if severe and prolonged the cells become injured. H the cells become injured.
You see? So ST elevation we deal with the how your blood gets to the heart and receives oxygen.
You see?
So if it goes on for so long and the cells are not getting oxygen as per needed what will happen the cells will become injured they will die off and that's why it is called myioardial in function in function is dead cell death cell you see h it is called stem st elevation mioardial in function and what can make it happen is blockage of the coronary arteries.
This coronary arteries that come back to the lungs to feed it with oxygen.
Something can happen like uh you can have a blood clot that can block those arteries. Complete occlusion of the blood clot atherosclerotic plague. Sometimes you can have some some let me say like it's called what cholesterols that have packed in your lungs and then they block blood to to go. So est elevation. So ST we have said is this so it is we are talking about what we are seeing at the what we are seeing at that machine that is called the echo cardiogram. So it will tell us if this part is elevated it means the part of your muscles of your heart muscles they are not getting enough oxygen contraction and relaxation is being interfered. You see so here is where we get some findings.
You do some findings. You take troponine and which markers you know when your heart sense are dying the most elevated marker is tropon. So we check them off.

An ST elevation mioardium is type of heart attack in which the damage to the heart muscles but no ST elevation. You have damage to the heart muscles but you don't have the ST elevation on the ECG.
Why does the ST segment elevate?
blockage you have said of the arteries that brings oxygen to the heart and stem nonst elevation is usually partially blocked. Those vessels are partially blocked. So they are taking oxygen that is not enough but can help your heart to to contract and relax well. Yeah. This is temporary reduction in blood flow causing some endo endocardial infection.
a part of your of your blood of your heart is suffering but not the whole of it. So the ST is normal [clears throat] because you have some oxygen that is going damage limited to the to the inner layer of the muscle. This does not produce elevation.
You just have damage to the inner part but the other parts are okay. That is called ST. So ST elevation is is blocked coronary arteries, arteries that give your heart oxygen but the other one they are partially blocked and only the inner part of your heart muscles are suffering but the other parts are okay and why the inner part you know the inner part of the heart muscle is called sub subendocardiogram endocardium subendocardium it is called the subendocardium it is the furthest from the coronary blood supply because if you see if this coronary arteries come the first places they will serve it is the outer layer it is the outer layer of this heart but inside it will not reach there they experience high pressure during ventricular contraction so when you contract this inside receives very high pressure has great oxygen demand than outside you see.
So it has a greater oxygen demand than the outside layer. So what are the causes? We are still asking ourself. The other one was blocked esting giving us an electrical system that the the the the STL is elevated while contracting. So what are the causes of this other one? H partially coronary artery occlusion. Occlusion means blockage but blood is passing but it's just partially blocked. And then we have severe coronary artery narrowing.
Sometimes those uh this coronary arteries they narrow so they're taking little blood little blood and coronary vessel spurs temporary spurs of coronary artery reducing when I'm saying spas you just know the blood vessels contract the blood vessels contract potential coronary artery dissection a tear from within the coronary artery wall. Sometimes it's just you call it internal bleeding. Ah t beauty is saying eating too much [laughter] more common in young women during short-term pregnancy.
T beauty it people should eat a little right oxygen demand imbalance. Mhm. The coronary artery may be completely blocked but the heart muscle need more oxygen than it receives.
Need more oxygen that it receives. And when we talk about oxygen demand, when we talk about oxygen demand, anemia can make you have a heart attack. Infection infections can make you have a heart attack.

Hypertension will lead your kidneys to shut down.

So what does that mean? Even the cells of your heart will die. So we have talked about two types of heart attack.

The T-wave is whereby it is deolarizing before before it comes back then it depolarizes again depolarizes again. So QRS QRS QRS QRS is when these bundles are being polarized. So when you you you hear that when you hear that someone has something called the bundle block bundle block means these are the bundles the bundles are left bundle right bundle at ventricular bundle bundle of his.
