Holding your breath, straining really hard stops the blood from going into the heart and thus into the brain. Now, is anybody going to come and save him? And are they doing the right thing? If you have a seizure, you want to turn the person to the side so they don't choke on their own secretions, but this guy is not seizing. He very simply does not have any blood in his brain. And so what you want to do is raise the legs up so it drains the blood back into the heart and into the brain.
