2019 paper published in Medical Hypotheses explored the idea that dissociative identity experiences may exist on a continuum ranging from ordinary consciousness to severe psychiatric dissociation. The researchers study highly hypnotizable individuals with no psychiatric disorder who spontaneously experience alternate identities during hypnosis. Importantly, these participants did not meet criteria for dissociative identity disorder. They were psychologically healthy, and their experiences were not considered distressing or impairing.
Brain imaging showed altered connectivity within the default mode network, especially between the posterior cingulate cortex and medial prefrontal cortex regions heavily involved in self-referential processing, identity, memory, and autobiographical awareness. The authors propose that experiences involving other selves may not always be pathological. Instead, they argue there may be a gray area between ordinary identity states and severe dissociative disorders shaped by trauma, distress, impairment, and loss of control. The paper also emphasized something important. Having unusual internal experiences alone does not automatically mean someone is psychotic or crazy. Context matters, functioning matters, distress matters, and trauma matters.
