ASMR Full Cranial Nerve Exam + EvaluationAdded:
Hi, welcome in.
Oh, okay. You're No. Oh, nice. Okay, perfect. Yes, we spoke on the phone a few days ago. Okay, great. So, um yes, I do remember you telling me about um your symptoms and everything. Let me pull your chart up here.
Okay. Right, right, right. Okay. So, has there been any major changes since we last spoke? Any new symptoms? Any sessation of symptoms? Um, let me think. Like, have you been having any in particular? These are usually more important, but like any dizziness, nausea, trouble breathing. Okay.
Have you been having any trouble sleeping? I know we talked about you feeling a little extra tired. Um, but I was just wondering if you've been having any like insomnia type symptoms. Okay.
Okay. Um, that's good to hear then that hopefully um even though you're extra tired, you could still be able to get to sleep. Um, okay. And then have you been taking any um medications? I know you said you haven't been able to get a prescription yet, but just any um Okay.
How has that been helping or has it been helping?
Okay.
All right. And then I just uh I noticed your nose is a little bit red. Have you still been having that really runny nose? Okay. H Have you been having any like hyper salivation? Like just a lot of like saliva?
Okay. Um any like sensitivity to light? Any changes in your uh other changes in your vision or in your sense of smell?
Okay. What about taste?
Okay.
Have you been having any like ringing in your ears or any hyper sensitivity to sounds?
Okay. And then any like tingling or numbness or um tender areas? I know we did mention aching. Mhm. Okay.
So, just still some aching in the same areas. Okay. Okay.
So, um the first thing I want to do is uh actually take a look at those areas where you said you were having some aches and pains. Okay. I'm going to put some You said it's some in your wrists. Okay.
So, let's start with your uh hands, fingers, wrist area.
Can't get my gloves on. Okay.
So, you can just go ahead and place your hands out flat on the table. I'm just going to go ahead and give them a a little feel here. Okay.
Okay. Okay. And any any discomfort or tenderness as I'm just feeling your fingers? Okay. I'm going to give each a little shake. Each finger a little shake here. Just get that flexion.
Little Okay. Any pain with that? Okay. So, your joints, your fingers seem to be good.
Um, I'm going to have you curl ball up your hands into a fist. Both hands.
Okay. And then stretch them out as wide as you can. Okay. All right. So, looks pretty normal in terms of strength and um flexion here. Okay. So, I'm going to move on to your wrist. I'm going to be a little bit gentle or I'm going to be very gentle, but let me know if there is any pain or discomfort or anything with what I'm doing. Okay. So, I'm just going to give them a very light feel here.
Okay. I'm not noticing any um palpable abnormalities or anything. No lumps or bumps. Visually, I don't see any bruising or external damage.
Let me let me just look with my light just to see if there's any, you know, sort of light discoloration or anything here.
Okay, I'm not seeing much. I'm gonna actually feel the temperature.
Okay, temperature feels pretty evenly distributed. So, um there might be uh signs of like inflammation or something if certain areas are uh abnormally warmer than they usually are, but I didn't notice any of that. Um do you do a lot of typing or a lot of like work with your hands or anything?
Okay.
Okay, that might be contributing to it.
Um, it's also just, you know, when you're sick, you can have some aches and pains. So, we'll just keep an eye keep an eye on that. Um, I think I would like to do a cranial nerve exam for you today. So, um, we're going to go through that. It shouldn't be too long. Um, but I just want to do a pretty thorough examination just to make sure we're not missing anything important here. Um, and then we can hopefully, you know, get you some some care and some treatment for to to help you manage your symptoms. Um, okay. So, I'm going to start just by looking at your eyes.
Okay.
And you can just look at my nose. You can look at a spot on the wall behind me. Wherever you wish. Um, just don't look at the light.
Okay.
I'm just going to hold my hand in between your face just like this. In between your eyes, I mean. Okay.
Mhm. Okay. So, your pup response, dilations, contractions, response to light, um, looks pretty normal for both eyes. I'm going to ask you to follow my finger.
Okay, good. Okay, so yeah, convergence looks good. Um, tracking was that small. Sorry about that. Okay. Um, I'm just going to take one more look into your eyes. Uh, can you just look over here at my finger?
Okay. And then look here at my finger.
I am noticing just a little bit of redness in the corners of your eyes. Um, is that typical for you like poor sleep signs or anything like that?
Okay.
Okay. Got it.
So, other than that, your eyes seem to be good. I'm going to test uh or do a few peripheral tests here. So, go ahead and look at my nose and tell me which side moves. Okay. Good. Perfect. Good.
Nice. Good.
Good.
And one more time.
Perfect. Okay, that seemed good. I'm going to have you look at my nose again.
Tell me when you cannot see my fingers anymore. Okay, they're going to spread outwards like so. Let me know here.
Okay, perfect. I'm just going to do the same thing a couple times. Good.
Perfect.
Good. Okay, my computer's updating. Uh, okay. Perfect. Perfect. Perfect. Let me think. Is there anything else I need to do? Let's look at my nose. Tell me how many fingers. Good. How many fingers?
Perfect. How many? Good.
Perfect. Okay. Nice.
Some good. So, that makes me a little less uh uneasy about that redness. Um, we will check. I'm gonna schedule for Holy moly. I'm sorry. I cannot speak.
Um, I'm going to schedule you for a followup and we will um obviously address all these symptoms that I'm noting today and make sure that things are getting better for you. Okay. So, after eyes is all good. I actually before I move on, I do want to just take a quick peek into your nose. Okay. So, just tilt your head up a little bit. Perfect.
Okay.
All right. And then go ahead and um open your mouth. Good. Say, "Ah, perfect." Okay. Yeah, definitely a little bit of redness in the back of your throat. Um nose has a lot of secretions out there going on, but um not anything super concerning. just normal signs of sickness and, you know, a little bit of that irritation. Um, cool. So, next I What did I say I was going to do? Um, we're going to be doing Let's actually since I just looked into your nose, let's uh we're going to test your sense of smell. Okay. Okay. So, first I'm just going to get a little bit close here. I'm sorry if this is uncomfortable. I'm just going to be super quick. Just fill your nose here down the bridge. Face the nose. Okay, perfect. Sorry about that. It was a little close.
Okay, now to test your smell, we are just going to do a few tests, a few reps I should say, one set.
Okay. So, uh once I spray this little Q-tip, it's going to activate the um flavor agent. You're just going to go ahead and smell this and tell me what you smell.
Chocolate. Very good. Okay. And we'll do the next one.
Okay. Go ahead. If you need a like Okay. All right. Here's the next one.
Cherry. Perfect. Okay, last one. Oh my gosh. I just dropped it, so I'm kind of Okay, go and smell this vanilla. Nice. Okay, perfect. So, um, does it smell?
It's good. I know you didn't report any concerns with that. I just wanted to double check. Sometimes it's honestly it's hard to notice.
Um, okay, perfect. Um, so on a kind of a similar note, we're going to do your your um, sense of taste. So, um, similar, I'm going to activate the flavoring agent with just a little bit of liquid. And then we're going to have you let me know what you taste. Okay. Are you ready to stick your tongue out? I'm just going to gently Okay. And what does that taste like?
Orange. Perfect. Okay. These these were obviously pretty recognizable.
Okay. Go ahead and stick out your tongue. Perfect.
And that one's coffee. Perfect. Okay.
All right.
Okay. So, smell and taste is good. Which is good. I know, you know, a lot of congestion, sinus issues, um those can get a little bit um impacted, but it seems like you have a good good enough functioning for now.
Maybe not ideal, but um maybe not our main concern for today. Okay. Okay.
Okay. Makes sense. Um all right. So, next what we're going to do is Okay. What we have left is sensory. So, touch and um hearing, right? We did. Okay. Good. With the five senses. Okay. So, next I am going to be testing your Let's do hearing. Okay. So, I'm going to first as kind of a sort of diagnostic just to see if there's anything immediately to be concerned about. I'm just going to be making a little flutter sound on next to one of your ears and just tell me which side you hear it on. Okay. I'm going to have you close your eyes. Perfect.
Good.
Perfect.
Mhm.
Good.
Perfect. Okay. Nice. So, next, um, I'm going to it's going to be a similar thing. I'm just going to whisper some numbers into each side. You're going to have your eyes closed and then let me know or just repeat back to me whatever numbers you hear. Okay. Okay.
Perfect.
7 2 3 7 2 3 Good. 4 1 2 4 1 2 4 1 2 Okay, good. Three three.
Good. Triple 3 9 87 987 and 987. Perfect. Okay, we're going to do same thing one more time. I'm just going to be using words instead. Okay.
All right.
Orthodox. Orthodox. Orthodox.
Good. Perfect. Perfect. Perfect.
Perfect.
Perfect. Perfect. And future. Future.
Future. Future.
Good. Okay, last one. Music. Music.
Music. Music music.
Okay, perfect. So, hearing also seems to be good. I know you really didn't have much problems in that area, so that's good.
Okay. Um, so the last thing we're going to do is just some tests for your sense of touch. I'm actually going to also do a few um like strength neuro tests here.
So, I know we did like balling up your hands, making a fist. I'm going to have you hold your arms out like this, like at a 90° angle type of thing. Good. And I'm going to pull them. Don't let me.
Good. And I'm going to push them. Don't let me. Good. I'm going to try to spread them apart. Perfect. And then don't let me push them in. Perfect. Okay. Next, I'm going to have you hold them out.
Palms face down. Perfect. And then same thing, just resist. I'm going to push down. Good. Push up. Don't let me.
Perfect. Okay, you can relax your arms.
I'm going to do the same thing for your shoulders. So, push them up and I'm going to press down. Don't let me. And perfect. Okay. And you can relax. I'm going to have you smile.
Good. And frown. Perfect. I'm just going to feel a little bit right here.
Okay. Good. And then can I have you um fur your eyebrows?
Good. And then raise them. Perfect. And then can you make a fishy face? Good.
And then bluff out your cheeks.
Okay. Perfect. Perfect.
Okay. So, it's good so far. Okay.
Next.
If I could find my brush.
Okay. Sorry, that was a lot. It's way down there. Okay. I'm going to touch I'm going to either brush you with the soft side or tap with the with the I guess uh we'll call this dull and we'll call this sharp. Okay. So, I'm going to have you close your eyes. I'm going to touch your face first. Um and then just let me know which end of the of the brush you feel. Does that make sense? Okay. Okay. Okay, close. So, close your eyes. Let me know. Sharp or dull? Good.
Good.
Perfect.
Good.
Perfect.
Good.
Perfect. Okay, you can open your eyes.
I'm going to have you lay out your hands, palms face up, flat on the table.
Perfect. Same thing. Close your eyes.
Let me know short. Okay.
Good.
Perfect.
Good.
Good.
Perfect.
Nice. Okay. Great.
Did you have any like pain, numbness, tingling, anything like that as we went through these?
Okay. Okay.
Yeah. I didn't notice any signs of um discomfort. So um that is so good.
So I'm glad that you performed very well on this cranial nerve exam. Um so just based on your symptoms, I'm going to give you a couple prescriptions here. Um this should help uh manage the symptoms. Um again, if um things are just not getting better for you, we will definitely address that at your followup. But for now, um seems like you have been improving a little bit. Okay, that's good. So, um yeah, I will have you pick up these prescriptions from the pharmacy downstairs and then um just let me know.
Of course, if anything happens sooner or, you know, just doesn't feel right, of course, feel free to contact me before the next appointment. Um, but if not, then I will see you then. Okay.
Okay. Thank you so much. Take care.
Good night. Good night.
