Dotyeni: Mfazi Was Murdered. I Know Who Gave The Order.

[00:00:00]On Sunday 19th July 2026, I came across a Sunday Times article with a headline that stopped me. There's a plan to take my life. Those are the words of Lieutenant General Cindil Mazi, the Deputy National Commissioner for Crime Detection written in his own diary months before he died. I want to hear from you. Was General Mufasi murdered?

But there is a bigger story sitting behind that article and I want to tell you what I have managed to gather because what started as a Sunday Times investigation leads directly to a sworn protected disclosure submitted to Parliament in March 2026 that puts the implications of Mufasi's death at a level this country has not yet fully reckoned with. [00:01:21]Because his rank and his background matter for everything that follows.

[00:01:26]Before Mufasi even became a detective, he was a commander of Kuntu with Shizway. He trained and commanded Becketle and many others in Angola. He served for 39 years rising from warrant officer to the second highest detective position in the country. He was not a man who could be easily dismissed or quietly sidelined. When Fuzzy reached the deputy national commissioner level, he was sitting at the intersection of every sensitive investigation in South Africa. His desk in 2020 and 2021 held an alleged 1.6 6 billion rand personal protective equipment fraud ring inside police headquarters, a 6.1 billion rand forensic systems contract, the assassination of Cape Town detective Lieutenant Colonel Schaw Canir, and the crime intelligence secret services account, a covert multi-million rand fund with a long history of being eluted. Every one of those investigations pointed at people inside the state. And then in June 2020, something else landed on that desk.

[00:02:38]The key witness in this story is Pelisande Honenimus Doyeni, a former crime intelligence analyst who served in SAPS from 2004 to 2012 and stayed in contact with after leaving. On the 3rd of February 2026, Dejeni appeared before the ADOC committee investigating the allegations made by Quazula Natal police commissioner, Lieutenant General Nlana Muanazi. He called his testimony a Mayday call, the last radio call for a general he believes was assassinated.

[00:03:14]>> I make this call that this honorable chair is a Mayday call.

[00:03:22]This is a made a call and a last radio call for a general in the South African police service whom we believe was assassinated.

[00:03:32]>> At the end of that session, Djenni said there was more that he could not share in open hearing. He was told the committee would make provision for an inc camera session where he could provide the sensitive evidence safely.

[00:03:46]That session never happened. So on the 20th of March 2026, Denny signed and submitted a written protected disclosure to the committee under the protected disclosure act number 26 of 2000. That document is what I have in front of me.

[00:04:05]It has not been fully reported. What is in it changes the shape of this entire story. Denny opens by explaining why he brought this disclosure to parliament and not to the president. His words in this document he says the very office that should be the ultimate guarantor of accountability might itself be implicated in the circumstances surrounding General Maz's death. That is why he went to parliament. That is why he made the Mayday call to the institution that elects the president, not the president himself.

[00:04:43]According to the disclosure, Mufasi's involvement in the Pala Pala matter began in June 2020 when Arthur Fraser first alerted him to the burglary. This is important. The official version recorded by the public protector says Mfazi was informed of the break-in by Major General Wally Ruda in March 2020.

[00:05:06]The disclosure says that account is false. The document notes that former national commissioner Kles doule disputes it as well. What the disclosure says is that when Fraser first came to Mufasi, Mfoi was cautious. He initially dismissed the allegations because he believed Fraser was attempting to discredit the president. He did not act straight away, but he did conduct his own inquiries and what he found was far more serious than he had expected. His investigation revealed that an unauthorized cover operation had been conducted using the secret services account and according to the disclosure that operation may have resulted in the death of persons involved or connected to the break-in. This is the part that has not been reported. The polar story has always been told as a story about money, about foreign currency, about disclosure obligations, about whether a crime took place on the farm. According to this disclosure, what Nazi found is that the aftermath of the break-in may have involved people dying.

[00:06:19]The operation to contain the fallout was allegedly run without a registered cast docket. the crime administration system reference that makes any SAPS investigation lawful, traceable, and open to oversight. An operation without one sits outside SAPS procedure entirely, beyond the reach of any auditor and beyond the testing of any court.

[00:06:46]Mazi documented what he found. He recorded it in his official SAPS pocketbook, a small bound notebook used by police officers to record contemporaneous notes in the execution of their duties. In September 2020, Malazi discovered that a senior police officer had recorded a 38minute telephone conversation between them. He wrote in his pocketbook, I want to know from left tenant and the name was withheld as to what informed him to record. Who gave him the authority? He was being surveiled within his own organization.

[00:07:25]He knew it. He wrote it down. Then in November 2020, he recorded a meeting with the Office of the Inspector General of Intelligence. The Sunday Times, which obtained copies of that pocketbook, reported that Mufasi noted with concern that financial documents supplied by crime intelligence, covered only the previous financial year. The pocketbook shows he was already under pressure before the Office of the Inspector General of Intelligence meeting. After that meeting, he recorded an instruction to himself.

[00:08:00]compile a portfolio of evidence regarding the conduct of the divisional commissioner of crime intelligence.

[00:08:08]During that audit process, MFA also flagged the 2020 2021 secret services account records as missing. Now look at what those dates tell you.

[00:08:19]The Pala Pala burglary happened in February 2020.

[00:08:24]The Secret Services account records that Mufasi flagged as missing cover the 2020 and 2021 financial year starting from that same month. The very records that would show what state money was spent on in the period immediately after the breakin are the ones that vanished.

[00:08:47]According to the written disclosure, those missing records were files needed to prove the unlawful expenditure. So the paper record of a covert fund disappeared from precisely the place an honest audit would have looked first.

[00:09:03]Then on 10 December 2020, his diary recorded a death threat. at 1833 from an unregistered number. A call attaining to be from the anti-gang unit warned him of an active assassination plot and specifically named senior police officers as being involved. Mufasi documented it in detail. He reported it.

[00:09:28]No effective protection followed. 5 days later on 15th December, Mufasi chaired an internal audit of emergency PPE procurement. He documented a procurement universe of 1.6 billion rand, flagging verbal approvals without written authorization, payments without stock controls, and an estimated 35 million rand in direct irregular losses. The audit report had already been leaked. An internal contact had received threats.

[00:10:00]Mufasi's response was to immediately request a threat assessment from Deputy National Commissioner Francina Vumar. So Mufasi who had asked Deputy National Commissioner Vumar for protection against a death threat in December 2020 died in July 2021.

[00:10:21]Hold that thought. According to Duty's testimony before Parliament, the driver received an instruction after Mfazi's death from Lieutenant General Francina Vuma to hand over Mufasi's investigative files to Sergeant Pungu. I called the the driver and I'm going to say these names, man, you know, because this thing of always keeping quiet doesn't help anyone.

[00:10:47]The driver told me that uh he received an instruction from General Vulma to give to to to to he the driver received an instruction from General Vulma to give the investigative dockets that Mufasi was handling to Sergeant Pungu.

[00:11:12]That is what he's told me.

[00:11:14]>> Sorry. Are these the documents that were now seized from General Fuzz's house after he passed on?

[00:11:21]>> Yes.

[00:11:23]>> Okay.

[00:11:23]>> The man who asked Vumar to protect him, the woman who allegedly received his files, died on 8th July 2021. SAP said it was COVID 19 complications and parliament was told the same thing. Now, let me tell you what the written protected disclosure says happened on that night.

[00:11:45]I'm going to attribute this precisely because these are serious allegations that have not yet been tested in any court. According to Yeni's disclosure, within hours of Mufasi's death, those who had reason to silence him moved swiftly.

[00:12:04]He writes that they understood that Mufasi's knowledge had not died with him. It lived in his files. Those files held the portfolio of evidence against a senior crime intelligence officer, the complete findings of his Palapala investigation, documents related to the FDA syndicate, and information regarding drug allegations against Minister Beckle.

[00:12:31]According to the disclosure, a chief of staff in the presidency was already on the scene.

[00:12:40]He directed family members and drivers to hand those files over to General Vumar. The disclosure says that for a time, Mrs. Lindel Mufasi was prevented from entering her own home. It was only through her persistence and insistence that she forced her way inside.

[00:12:59]It was then upon that forced entry that she witnessed the circumstances in which her husband lay dead. And the disclosure goes further. According to the document, the minister of police was sitting in the president's office when he received news of Mufasi's death. The disclosure says he sighed with relief. I'm reading you allegations from a signed protected disclosure submitted to Parliament. They are not findings of a court, but I'm not going to pretend they are not in this document because they are. What happened next is on the record. Doi testified about it under oath on 3 February 2026.

[00:13:44]One of MFA's drivers called to say the general was dead. Doi phoned the wife.

[00:13:50]He could hear people around her and she was saying, "Leave the cell phone. You are not going to have it.

[00:13:58]He called the driver back. People from SAP's head office were taking Fuzzy's documents while his body was still upstairs and the blood had not been cleaned. The family went to court.

[00:14:12]Roughly 10 days after the burial, the body was exumed. Toxicology found casting resin, which Doi told Parliament explains why there was blood at the scene. An inquest docket was opened at Cambridge SAPS. Then head office quietly took that docket to be investigated internally for almost 5 years.

[00:14:37]Nothing followed. I then phoned the driver again what's going on there and he says no there are people here from head office who are taking the documents of Rangaz. His body is still upstairs.

[00:14:52]They haven't even cleaned the blood that's there. They're taking these files.

[00:14:56]>> I thought he died from COVID 19. Um blood would >> there was blood there. There was blood.

[00:15:03]The family, one of the family members is an advocate as well. So they then pushed for this report, the court actually to ensure that it is actually done, you know. So they went and dug him up, >> exumed him, >> exumed him. An inquest was then opened after they found that there were there was cast and resin in his system which meant that the forensic evidence was saying that this man had been poisoned with casten resin which explains why there was blood in that room because the man was choking in his own blood.

[00:15:44]Here is the thing about trained intelligence officers. They understand that devices can be wiped. Files can be seized and phones can be erased.

[00:15:56]According to the written disclosure, the people who came that night seized laptop. They seized his files. They wiped his phones clean of data. They believed they had erased his work. They believed truth had died with him. But Mufasi had kept his official pocketbook separate from his investigative files. A small bound notebook, not on any server or in any device, just handwritten entries recording names, dates, phone calls, threats, and evidence, the things that mattered most. They did not find it. His widow gave it to Duteni for safekeeping. On 14th July 2026, Denny formally handed it over to the murder investigation team. Two weeks later, the Sunday Times had its contents. Denny's conclusion in the written disclosure is stated plainly. That pocketbook now speaks for him. When Dini appeared before the committee in February 2026 and called it a Mayday call, he was giving MPs the first sworn public testimony that Mfazi had been poisoned and that no investigation had followed the toxicology results for almost 5 years. The petition that had been building around his death passed 1 million signatures by the end of June 2026. On 6th July 2026, SAPS confirmed the murder investigation.

[00:17:31]That same day, investigators raided four premises in Centurion, two officers, two residential properties linked to serving police officials.

[00:17:42]A person of interest was questioned.

[00:17:45]Police cold case unit has raided at least four properties in Centurion Ptoria regarding the murder investigation into the death of Deputy National Commissioner for Crime Detection Cindy Lefazi. A person of interest is also being questioned. The inquest docket has been changed to a murder docket after a poisonous substance was found in Fuzzy's body.

[00:18:06]Police are also looking at other persons of interest and have seized a number of electronic devices. On 18th July 2026, Infasi's own diary became available. One entry recorded his own words.

[00:18:20]There's a plan to take my life. He saw it coming and he wrote it down.

[00:18:26]Let me tell you what I think this means.

[00:18:29]And I'm going to be careful about the distinction between what is established and what is alleged. What is established? Mufasi was poisoned. That is the finding of the toxicology report obtained by court order after his family pushed for exumation. SAPS now calls it murder. That is the institution's own classification. What is alleged in the signed protected disclosure that the co operation run after the Pala Pala breakin may have led to people dying that a chief of staff in the presidency directed the seizures of evidence from Fo's home within hours of his death.

[00:19:14]That the Minister of Police sighed with relief when he heard Mazy was gone. None of those allegations have been tested in court. The Guini's disclosure is one man's account. It is serious and sworn, but it is not a verdict. What makes it harder to dismiss than just the word of one former warrant officer is the shape of what surrounds it. The pocketbook and the toxicology was real and so are the missing 2020 2021 Secret Service records. The inquest docket being moved to head office for internal investigation.

[00:19:53]The same head office implicated in the theft of files is real and the question it raises is one that this country has not yet answered.

[00:20:05]Yeni says he cannot bring this disclosure to the president because the presidency may be implicated. The committee he brought it to is investigating allegations that reach into the same crime intelligence environment. The murder docket for the general who found the evidence was handed to the very institution he was investigating. This is not a conspiracy theory. This is a series of documented procedural decisions. Each one on its own might have an innocent explanation.

[00:20:40]But taken together, they describe something that needs to be explained by the people who made those decisions.

[00:20:50]Diani put five demands before the ADOC committee. He asked Parliament to establish who authorized the retrieval of materials from FA's home. Who holds those materials now? What became of the toxicology evidence? Who was assigned to the murder docket? and why the investigation sat still for almost 5 years. Those five questions now have a sixth which comes from the written protected disclosure that he signed in Johannesburg on the 20th of March 2026.

[00:21:23]Who is the chief of staff in the presidency who was on the scene that night?

[00:21:30]Who directed those files to General Vuma? General Mazi did his part. He investigated, documented, and warned. He did everything right.

[00:21:43]The only question that matters now is whether the institutions he trusted with that evidence will do theirs. Don't just know what is happening. Understand what it means.
