During the 1962 Cuban Missile Crisis, Soviet submarine captain Vasili Arkhipov refused to launch a nuclear torpedo despite orders from his captain and agreement from two other officers, thereby preventing nuclear war; his single vote of 'no' saved the world, yet he remained unknown for 40 years until his death in 1998, demonstrating how one person's moral courage and adherence to protocol can prevent catastrophic outcomes.
