Deadly Dementia Has Nothing To Do With Memory
Added:If I asked you what dementia is, you'd probably describe it with words like memory, forgetting, senile. But very few people would say swallow, walk, eat. You see, dementia is the number one cause of death in the UK, and it doesn't kill you by making you forget names.
As a GP, I recognize dementia. I see the journey my patients go on, and sadly, I fill out the death certificates when the time comes. What I want to do in this video is show you something most people never really understand.
The physical way dementia affects your body. Not to scare you, but because if you've got a loved one with dementia, or you're worried about it yourself, understanding this changes how you see the whole condition and what you can actually do about it.
I remember an older gentleman who came to see me with his wife.
At the beginning, it was subtle. She said he was becoming forgetful, repeating questions, misplacing things, getting confused in familiar places.
And if you'd met him briefly, you might not think much was wrong. He could still talk, he could still laugh, and he could still recognize people, but over time, things slowly changed.
His wife noticed he was getting slower on his feet. Then he had a fall. It was nothing dramatic at the time, but he never quite returned to his previous level of independence and confidence with his mobility.
Then came coughing during meals. At first, it seemed minor, a small cough with tea, occasional spluttering with food. But every so often, food and drink really went down the wrong way, leading to a chest infection, pneumonia. He had a course of antibiotics and recovered, but not fully.
Then another infection followed. Then more weakness, less mobility, another fall, less confidence. And instead of bouncing back each time, he gradually became frailer and less independent until eventually, the body could no longer recover.
And that's how dementia progresses. Not suddenly, but through a gradual loss of the body's ability to cope.
Now, falls happen without dementia, sure. Chest infections happen all the time without dementia. UTIs happen without dementia. So, if someone has had a fall or an infection, that does not mean they have dementia.
What matters in dementia is the pattern over time.
A gradual decline in multiple systems together. That's what we're talking about here. I've seen that journey more times than I'd like.
So, let's have a think about how this could actually happen. There are loads of conditions out there that affect how we think, feel, and behave. Think anxiety, depression, stress. And in those conditions, the brain often still looks structurally normal, even though the experience is very real and very serious. But dementia is different, isn't it? Because dementia is physically damaging the structure of the brain.
This is a healthy brain, an orange.
And this is what happens in dementia.
The brain is physically shrinking. Brain cells are dying. Connections between them are breaking down. The wiring of the brain is being lost over time.
And that matters because the brain isn't just responsible for remembering.
It's responsible for running the entire body in the background. The way I like to understand to think of dementia and the brain like the electrical system of a house. At first, the phone line becomes unreliable. The TV signal drops out. That's often what families notice first. The memory problems, the repeating, the confusion.
But the wiring doesn't just fail in one room. The heating system goes offline.
And in the body, that's your blood pressure regulation beginning to falter.
The fridge shuts down. And in the body, that's your appetite and digestion starting to fail. And eventually, the lights go out. In the house, that's every system failing at once. And in the body, it's the same. Because here's what a lot of people don't realize about the brain.
Some of what it does, you're in control of. Talking, deciding what to eat, choosing to stand up.
But a huge amount of what it does, you never think about. You just swallow. You just keep your balance. You just coordinate breathing. You just adjust your posture without deciding to. Food goes down the right pipe. Your body weight shifts when you stumble.
These aren't things you decide to do.
The brain just runs them automatically in the background. And in dementia, the things you have conscious control over, those tend to go first. The repeating, the confusion, the forgetting. But what actually ends the journey is when the automatic system starts to fail.
Swallowing that no longer coordinates safely. Balance that no longer corrects in time. A body that no longer bounces back from an infection. Not because someone is forgetting how to live, but because the brain is losing the ability to run the body automatically.
Okay, but here's the important part. If you understand what's physically wrong, you also understand what you might be able to do to intervene, to limit damage, and to preserve independence.
Blood pressure. Think of the small blood vessels supplying your brain like tiny garden hoses.
Run too much pressure through them day after day, year after year, they start to crack and leak internally. That's silent damage accumulating in the brain every single day. It's the single biggest modifiable risk factor for vascular dementia.
So, know your numbers. If your systolic, the top number, is consistently above 150, that's not something to keep an eye on. That's something to act on.
Sleep. Your brain has a cleaning system that only really works when you're asleep. During the day, waste products build up, almost like dust settling on every surface.
Overnight, cerebrospinal fluid washes through and clears the dust away.
Shorten the cleaning cycle night after night, and the dust accumulates.
There's a clear association between chronically poor sleep and increased risk of dementia.
So, when someone tells you sleep matters for your brain, this is why. It's physical. It's not vague.
Movement. Physical activity increases blood flow to the brain and stimulates a protein called BDNF, essentially fertilizer for brain cells.
It helps maintain the connections between them. Your brain isn't a sealed box that either works or doesn't. It responds to how you treat it, and movement is one of the strongest signals you can send.
Cognitive reserve. Think of your brain like a road map. If you've only ever built one highway between A and B, one way of thinking, one skill set, and the disease blocks that road, you're stuck.
But, if you spent years building side streets and back alleys, learning new things, picking up skills, being a beginner again, traffic still finds a way through. It's the act of learning, the struggle, the frustration, that builds that extra wiring.
A second language, an instrument, a club, not mastery, the willingness to be rubbish at something new.
Social connection. Isolation does to your brain roughly what smoking 15 cigarettes a day does to your body.
Socializing is one of the heaviest workouts your brain gets. You have to listen, read body language, pull memories, form responses, all at once in real time. When that circuitry goes unused, it withers. It doesn't require a packed social calendar, a neighbor you talk to, a weekly group you attend, small, regular human contact.
Occasionally, dementia is preventable, and that's worth knowing. Most of the time, what the best evidence says is these things may push the disease back, delay it, and pushing it back even a few years can mean a few years more of independence.
A few years of knowing who your family are, a few years of living on your own terms, and that's not nothing.
As a GP, one of the hardest things to watch isn't necessarily memory loss, it's the gradual loss of physical independence, walking safely, eating comfortably, recovering from illness, managing everyday life.
That's often what families are really experiencing over time.
But when you understand the process, it changes things. It replaces frustration with understanding, and confusion with clarity.
