For 99% of human history, to step into a doctor's office was to gamble with your life. The very people sworn to heal you were guided not by science, but by ghosts, foul odors, ancient philosophy, and centuries of unquestioned tradition.
They would bleed you until you fainted.
They would bore holes directly into your skull to release invisible demons. They would paint your throat with mercury, [music] prescribe toxic chemicals as medicine, and insist that the sickening sweet smell of rotting garbage was the physical embodiment of death itself.
What makes this story so unsettling is that none of these physicians believed they were causing harm. when the cure was infinitely worse than the disease.
For over 2,000 years, humanity shared a singular terrifying belief. If you could smell death, death was already inside you. This was the theory of miasma or bad air derived from the ancient Greek word for pollution. The miasma [music] theory asserted that diseases like chalera, plague, typhus, malaria, and countless other deadly illnesses were not [music] caused by microscopic organisms, but by poisonous vapors rising from [music] decaying organic matter, stagnant water, overflowing sewers, and rotting corpses. To ancient physicians, the logic seemed undeniable.
Wherever [music] foul smells lingered, sickness often followed. Swamps bred fevers. Battlefields filled with decomposing bodies [music] became centers of disease. Crowded cities overflowing with waste suffered repeated [music] epidemics.
Since bacteria and viruses were still completely invisible to the human eye, smell became the most convincing evidence available. Modern pop culture views this haunting mask as a symbol of Gothic horror. But to a [music] physician living through the plague, it represented one of the most advanced [music] forms of personal protection ever designed. The long hollow beak was carefully packed with dried lavender, mint, rosemary, camphur, cloves, rose petals, juniper berries, and expensive imported spices. Every [music] fragrant herb served a single purpose, to purify the air before it entered the doctor's lungs. Why? because they believed [music] that if you could block the smell of death, the disease itself could never reach your body. But this beautiful [music] poetic theory had one fatal flaw. It made doctors completely blind to [music] the actual source of disease.
Let us travel to Broad Street, London during the catastrophic chalera outbreak of 1854.
Entire families [music] collapsed within hours. Healthy adults who woke up feeling [music] perfectly normal could be dead before sunset.
Victims suffered relentless vomiting, violent diarrhea, and catastrophic dehydration [music] until their skin turned pale blue, and their bodies became almost unrecognizable. Food itself became medicine. Patients suffering from a hot disease might receive cold [music] vegetables, watery fruits, or dairy products. Someone believed to [music] have an excess of cold fleg could be prescribed spicy herbs, roasted meats, wine, or warming spices. Meals were chosen not because of nutritional science, but because [music] every ingredient supposedly possessed its own elemental qualities capable of restoring balance. According to Galen, sickness was almost never [music] caused by something invading the body. Instead, disease reflected an imbalance already existing within it. A fever meant excessive heat. A cold meant excessive moisture. Headaches, stomach pain, seizures, infertility, even mental illness could all be explained through the movement of the humors. The doctor's responsibility was [music] simple. Force the body back into equilibrium.
And this is where medicine [music] became terrifying.
If physicians believed a patient carried too much flem or black bile, [music] they prescribed powerful aimed to induce vomiting and aggressive peratives to empty the bowels. These were not mild herbal remedies. Many contained [music] toxic minerals capable of causing severe dehydration, organ damage, [music] and sometimes death. Patients could spend hours violently vomiting while suffering relentless diarrhea, becoming weaker with every passing minute. Few medicines were trusted more completely. When patients [music] swallowed repeated doses of calaml, horrifying symptoms soon appeared. Their gums became painfully swollen. Teeth loosened inside [music] infected mouths before eventually falling out altogether. Ulcers spread across the tongue and cheeks. An overpowering metallic taste filled the mouth. Most disturbing of all, patients began producing extraordinary amounts of saliva. Some victims drooled continuously for days, losing lers of fluid as [music] mercury slowly poisoned their nervous system. To hummeral physicians, this was extraordinary success. The endless salivation supposedly proved that excess humors were finally leaving the body. The treatment had worked. The patient was healing. In [music] reality, they were suffering from acute mercury poisoning.

Others experienced kidney failure, confusion, tremors, memory loss, and agonizing deaths caused not by their original illness, but by the medicine itself. Yet, because many diseases naturally improved with time, doctors often [music] credited recovery to treatments that had accomplished nothing or worse, [music] had nearly killed the patient. The failures were blamed on the severity of the illness.
The rare survivors became proof that the theory worked and so the cycle continued generation [music] after generation, century after century. Entire civilizations remained trapped inside one of history's most convincing medical illusions, never realizing that the greatest imbalance threatening their patients was not hidden within the four humors.
It was hidden inside the treatments themselves.
But what if the doctor decided the fluid you had too much of was your own life force? What if they concluded that the fastest path to recovery was to remove the very substance keeping you alive?

For more than [music] 2,000 years, opening a vein was considered one of the safest and most [music] reliable treatments in medicine. Fever, bleed them, broken bone, bleed them.

[music] To increase the flow, heated glass cups were placed over the wounds. As the air [music] inside the cups cooled, a vacuum formed, pulling blood rapidly toward the surface. In December of 1799, the former president of the United States awoke with a severe sore throat after spending hours outdoors in freezing weather. By modern standards, the [music] illness was likely treatable. But his physicians believed the inflammation [music] resulted from an excess of blood. Over the next 12 hours, they repeatedly [music] opened his veins again and again and again. By the end of the treatment, [music] nearly 4 L of blood had been removed. An astonishing amount for a man already weakened by illness. Washington struggled [music] to breathe. His pulse weakened. His strength disappeared. He did not simply battle infection. He also battled catastrophic [music] blood loss inflicted by the very people trying to save him. Many historians [music] now believe the aggressive treatment dramatically worsened his condition. One of the strongest advocates was [music] Benjamin Rush, a signer of the American Declaration of Independence and one of the most influential [music] doctors in the young United States. Rush believed [music] bloodletting could successfully treat diseases ranging from yellow fever to mental illness. His reputation helped spread the practice even further, [music] reinforcing public confidence despite mounting evidence of its dangers.
But [music] history rarely deals in absolutes.
In a remarkable twist, one small [music] part of this forgotten therapy survived.
[music] Today, medicinal leeches still exist inside modern hospitals, not because [music] they remove bad blood, but because science finally understands how they actually work. If you used a trait often, that part of the brain would grow, pushing outward and creating a physical bump on your skull. Phronologists claimed they could read your entire moral character, [music] intelligence, and criminal tendencies simply by feeling the bumps on your head. Once society accepted that personality, morality, and intelligence could all be mapped onto the surface of the head, the next question became almost [music] inevitable. If the skull could reveal the mind, could changing the skull change [music] the person?
While phronology itself was largely non-invasive, it laid a dangerous [music] pseudocientific foundation. It taught society that the skull was a map that could be manipulated. And if you wanted to manipulate [music] the skull directly, you had to go back to the oldest surgery in human history, trepation, dating back over 8,000 years [music] to the Neolithic era. Treponation is the act of deliberately scraping, cutting, or drilling a hole directly [music] through the skull to expose the brain's protective membrane.
Archaeologists [music] have found tree pan skulls on every single continent. In ancient [music] times, it was used to release evil spirits that caused seizures, migraines, [music] and erratic behavior. In the 18th and 19th centuries, surgeons modernized this practice using a T-shaped hand drill [music] called a trefine. If you suffered a head injury during the American Civil War, military surgeons would use this drill to bore holes into your skull to relieve pressure. But they did this in filthy field tents using unwashed hands and bloodstained tools.
