A person can have a 'normal' scan while experiencing significant pain due to central sensitization, where the nervous system becomes overly responsive to pain signals. This condition is invisible on scans but explains much of the daily pain experience. The key insight is that while scans show structure, they cannot measure functional pain sensitivity, which is a separate and often more important factor in understanding and treating chronic pain.
