Air Crash Investigation in Teardown

[00:00:00]UM, OH MY GOD, THE DTSB is down. A beautiful day at the DTSB. Hopefully there are no plane crashes today. Oh my god. I'm going to head out to the uh the old training facility down the road. We need to do some tests of our equipment. Oh my god, that does not look good. That does not look good.

[00:00:26]>> Oh god, it just went into the ground.

[00:00:30]Oh god. Roll the trucks.

[00:00:32]>> DTSB's on the way.

[00:00:33]>> We got fire trucks responding right now.

[00:00:35]We got fire trucks heading out.

[00:00:37]>> Okay, I can see the fire already from here. We're on scene. We're on scene.

[00:00:41]Don't you touch my airplane. I don't think so.

[00:00:43]>> Oh boy.

[00:00:44]>> Oh god. There's nothing left of this.

[00:00:47]The fire is out, it looks like. Nope.

[00:00:49]Still burning inside. Any survivors? No survivors.

[00:00:52]>> No. No. Uh, unfortunately no survivors in this one. And you're just going to have to use the physical evidence to figure this one out.

[00:00:59]>> Wow. So, this thing went into the ground, but it rolled over because the tail is facing that direction. That's not the way we saw it crash.

[00:01:09]And we're going to go over here. There's a lot of burn marks all around this spot right here. And this looks like an engine. I got a wing here. Parts of a wing. Got wing structure right here in the flaps on this side. And I got a wing over here. So, the wing is intact.

[00:01:27]There's the nav light right there. I think the DTSB needs to get their helicopter out of uh DJIA. All right, here's the scene. Oh my gosh. That's not good. This is not good. This is not good. Pull up. We don't want to be part of the problem. All right. From the air, it's not looking uh like we have a nose section missing. I think it is buried underneath the airplane from what we're seeing. Safe to say it's completely intact over here. Completely intact. All right, let's see if we can land this thing in one piece. This is our pride and joy. The DTSB helicopter. Get out of it before it explodes.

[00:02:09]We're going to have to move the uh middle section up in the air if possible.

[00:02:16]>> All right. I need you to Yeah, we're we've we've got the wings. We've got the tail section. We just need to move the main fuse lodge up just to see what's underneath here cuz I I don't know what's underneath here. I haven't found the cockpit yet. There you go. Okay.

[00:02:32]Wow. Um there's no nose section. It's completely gone. Obliterated.

[00:02:41]How can that be? This is a big airplane.

[00:02:43]There's the other engine. Okay. It was buried underneath here, but it is completely obliterated here. This is what's left of the cockpit basically right around here. So yeah, the airplane clearly stalled. I saw it stall and fall from the sky. Uh, and it went nose down.

[00:03:01]I thought it was facing away, but what I think it did is I think it impacted right here. That's why there's nothing left of the cockpit. And then the tail section flipped over that direction towards the trees.

[00:03:14]>> Well, Jeff, that's spot on. That is exactly what happened. Uh we had a wing stall. We had an engine that was not really working too great and we had that wing stall and the plane went down and uh well you could see that it sort of disintegrated the front portion as it went in and then the rest of the airplane kind of came to rest and broke apart and caught fire. So that is exactly what happened.

[00:03:37]>> We're just going to leave this here for the uh for the other DTSB employees to come in and take care of. I'm headed back to headquarters now. Oh, we made it back to the DTSB and we're about to enjoy a steak dinner. Um, something that those pilots did not get to enjoy. So, here's our steak dinner in here. And it's going to be a beautiful uh evening meal. All right. Well, we had our steak dinner. There goes a departure right there right over the house.

[00:04:06]Okay. Now, they should be on the Soore 2 departure. So, they should loop all the way around and we should see them.

[00:04:13]That's similar to what we saw yesterday with that airplane. Although this one looks like it's in distress as well.

[00:04:20]UM UM OH MY GOD, THE DTSB IS DOWN.

[00:04:40]There's an engine inside the house. Roll the trucks. We need fire department. We need fire department. This is bad. This is real bad. Oh my god. My steak is fine. I left half of it for later.

[00:04:54]It needs to stay medium rare. Oh, this is just tragic. I hear the fire department. You guys good?

[00:05:05]>> Yeah, we Oh, we got a little bit burning over here. Oh my god. And look at this.

[00:05:32]>> Um, I really don't know. I mean, it just narrowly dropped next to me. Like, I I just what I saw was just so unbelievable. DTSB is on scene. We're trying to figure out what just happened, but >> you'll never guess.

[00:05:50]I'm looking on the side of this airplane. OH MY GOSH, IT'S FALLING.

[00:06:10]Somebody spray paint on the side of an airplane. And these are the DTSB colors.

[00:06:25]>> We're trying to uh investigate. I think we have a disgruntled employee is what I think we have.

[00:06:36]That's the one. I'm thinking this is the uh the employee we had to let go from yesterday.

[00:06:43]This is not good. All right, cuz I see yellow on the side here. But all right, let's let's do our investigation. This is still smoldering. By the way, you're suspect number one. I'm calling the police. Don't you worry about that.

[00:06:54]>> Oh, that's >> the DTSB took significant damage on the left side here. Look at this. The entire cutout of an airplane. That is uh unfortunate. We were having our steak dinner. uh downstairs and we went upstairs to have coffee afterwards and we looked out and that's when we saw this airplane come crashing through here. The left engine separated right here. Fuselage continued forward. Here is uh part of the wing assembly in the flaps. Looks like the right wing. So here's a wing right here. And then this is the middle fuselage right here. And then we have the other engine in the center. This is probably the left engine. The right engine looked like it came through the house and stopped right there immediately. Uh and then the whole airplane cartwheelled over. I can see this uh is the other side. So this is the left wing assembly here. And then yeah, it cartwheelled over and the tail section is facing towards the trees at the DTSB training center.

[00:08:01]And uh I think this was a uh I think it was an inside job from one of the employees we let go yesterday. That's my uh final verdict on it.

[00:08:10]>> But you'll never truly know, will you?

[00:08:12]Unless you find those black boxes.

[00:08:14]>> That is true. I need to find the black boxes. They're around here somewhere.

[00:08:18]>> I have a sneaking suspicion that you're not going to be finding the black boxes.

[00:08:22]>> Oh, they were removed.

[00:08:24]>> I will say while you were investigating, I set a small fire as a diversion. I then stole the black boxes, put them on top of my car, drove into a lake, blew up the car, and then uh well, yeah, you're not finding the black boxes. But despite that, yes, the disgruntled employee, aka me, uh decided to get revenge by taking my several thousand ton airplane and uh burying it inside of your uh somewhat rundown headquarters.

[00:08:57]Yet again, there is no cockpit because it went into the ground so hard it just disintegrated. Yeah. Here's a look from the air. It is an absolute travesty what happened here to the DTSP today. You're on my uh YOU'RE ON MY PROPERTY.

[00:09:21]>> We're good. We're good. We're good. Left turn heading 0 five 0 to send 2000 American 127. Yeah, American 127. We do not have the airport in sight. We're uh we're going to probably go miss here.

[00:09:35]>> Look at that. That's a beautiful airplane flying there. Absolutely beautiful.

[00:09:40]>> What's that? We got engine loss on that right side.

[00:09:45]>> Uhoh. Uhoh. Uhoh. All right. Time to deploy the DTSP. I'm the only employee here. So, uh or volunteer rather. We're going to go and Oh my gosh. Huge fireball.

[00:09:57]Massive fireball there. I heard the scanner go off. I heard uh engine failure or something happened. Engine trouble. So, we're going to take our uh our big truck here. It looks like it crashed right over by the gas station.

[00:10:10]So, we're going to need to pull up here.

[00:10:12]Oh my gosh. The plane is inverted. I can already see that.

[00:10:17]Holy cow. Uh, let's get this fire put out before it spreads to the gas station. We don't want that.

[00:10:24]Man, the plane is actually in intact. We might have survivors. Let's quickly put this fire out. Let's go to the other side here. Oh, man. The the cabin is burning up like crazy. Man, we're inside the fuselage of the airplane putting the fire out.

[00:10:36]>> All right, volunteer firefighters have SHOWN UP.

[00:10:39]>> OH, I DIED.

[00:10:40]>> Are you kidding me?

[00:10:44]>> Well, good thing the volunteers showed up finally.

[00:10:47]>> Thank God. We're doing everything we can here. It's just kind of tough with this water right here.

[00:10:52]>> First things first, I'm going to look for the four corners. Uh it's very important we find the four corners. And I'll mark these with the uh DTSB spray paint here. Uh and we got the one. I think this is going to be the left wing cuz it's upside down. Here we've got the left engine. So, I'll mark that.

[00:11:11]Let's cross over to the other side. And we've got to look for that right engine and right wing. I'm already seeing the right engine. There's the core and the actual uh casing for it.

[00:11:23]I'm not seeing the right wing. I think it may have partially disintegrated here. I think it may have struck this point and went down. It looks like we have the right wing controls here. So, definitely a sign of where the right wing was. I am going to get a crane out here shortly. I want to identify that the cockpit is intact. Then we'll get a crane out and uh we're going to try to drag some of this up onto the land a little bit.

[00:11:51]>> Yeah, just as a um so just so you know, uh I did SURVIVE THE CRASH.

[00:11:58]>> OH, OKAY. We have a witness, but you're probably going to be in the hospital.

[00:12:01]>> Yeah, I'm in the hospital. They they've already taken me away, but I I did survive it somehow. Okay, first things first. We're going to deploy the uh anchoring points here. That will prevent us from having any issues. And I'm going to get this wing out of the way here. And we'll we'll investigate this a little bit later. But we got to create a nice little open area to uh get the cockpit resting on because we want that out of the water. We want to look at the controls cuz that is one of the first things I want to uh investigate here is that we have the um the cockpit controls and everything. Yeah, I'm slowly pulling it up. It looks like we're getting uh a problem here. I'm going to jump out. Yeah, look at that.

[00:12:42]It looks like there is a big uh split right there. Oh my goodness. Well, it it gave way. Yep.

[00:12:48]>> Uh let's go ahead and get that on land.

[00:12:50]All right, perfect. We are hooked on.

[00:12:57]Oh yeah, this is a good operation here.

[00:13:06]>> Let's go ahead and drag this a little more onto land. Let's get it over here.

[00:13:13]Getting hooked up on something here. And while it's hooked on, we need to like hold this in place cuz it's going to fall into the water. So, I'm going to build some temporary support joists here to hold this.

[00:13:28]Uh, makes sense why the pilot survived.

[00:13:30]It's intact here. Uh, so that would check out for the pilot's survival. Um, they explained that they had engine issues, but what's interesting is I'm not seeing too much damage to the engine besides post impact damage. All right, let's get those black boxes out of the uh airplane as well. We want to do that, too. And we're going to have to cut into the aircraft here. Okay. Well, there's the black boxes right there. We're going to pop these out.

[00:14:02]American 127. We don't even feel the site.

[00:14:08]Let's do it. We got the uh looks like N2 is dropping on to the birds or something.

[00:14:22]From what I can make out, uh, it sounds like we definitely had engine problems, possibly a bird strike. Sounds like there may have been the word bird uttered in there. So, I think we may have had a bird strike situation. Uh, I do want to see the crash points here where our uh, witness marks are. I see we've got a big trench dug into the parking lot here. There is definitely some damage to the tree. You can see it kind of got sliced at an angle. The light pole is fine.

[00:15:02]I think this airplane struck birds and basically went into a a possibly a bit of a left stall. Maybe um a right stall actually.

[00:15:17]It is a little hard to tell with the uh position of the airplane here, but based on the damage, I'm going to have to say the airplane struck birds and the right engine.

[00:15:30]The right wing dipped down, the left wing came up, it struck the ground, and then kind of rolled over and landed in this water, but it was moving very slowly. So, they were definitely trying to recover the aircraft. That's going to be my final verdict for this crash.

[00:15:46]>> You're pretty uh pretty spot on. I mean uh yeah, so bird strike low altitude on a goound because they didn't have side of the runway and on the goound um the uh the right side engine took a bird. They just didn't have enough didn't have enough thrust to get out of that climb >> and it stalled and went over on its side.

[00:16:10]So that would that would explain the the stall the the tilting over that would explain why the whole airplane tilted over and actually crashed at such a short distance. It wasn't moving very quick. It was very slow when it came in >> for the uh the fire airplanes.

[00:16:26]>> Yep. We got some forest fires in the area and we are doing some operations.

[00:16:30]>> That does not look normal right there.

[00:16:32]That does not look normal.

[00:16:34]>> Okay, it's fine. It's fine. That was scary looking. Uh, I need to move the steak dinner out of the way so I can get to the portaotties.

[00:17:37]>> Yeah, >> that's just a lamp. Okay, that's just a lamp. This person's bedroom. Good thing they were at work today >> and not sleeping cuz that would have been parts of an airplane coming right through here. Although, I'm not seeing any parts of the airplane in this house as of right now. I don't use the four corners method, but we should. Um, we've got an engine here.

[00:17:58]That is an engine. It's a little smoldering, too. Um, we've got parts of an airplane over here. Looks like another engine that was completely separated over here. So, we have two engines.

[00:18:11]Uh, looks like the wing section right here from what we can tell. And then we've got parts of a wing. Looks like in the side of the house right over here. Go ahead and just mark that with an X.

[00:18:25]Here's another section of the wing right here. I can't tell if that's part of the wing or if that's part of the fuselage.

[00:18:30]That looks like the side of a fuselage right there. Part of the plane. This is the uh the main section right here that we can see. Looks like the wing routts right there in the middle. Damage over here on this house. We might have a part of a wing or something right here.

[00:18:48]I'm not sure what that is, but that damaged the the house. There's a part of the airplane in their hallway. We're going to roll the porter house.

[00:18:58]>> Okay. And yeah, this is a mess right here, dude.

[00:19:13]We have to come over here to see if we can find any more of the airplane. And we do. We have parts over here by the railroad tracks. Oh my. Is this the cockpit? No.

[00:19:25]That looks like the side of the fuse lodge. I can see the writing on the side of it. T whatever one three or whatever that is on the side there. We have parts over here strewn about all the way back to here. Oh my dude, look at this. This is a whole part of the crash site. We have anything else? We have some scouring right here. Not sure what that is. I'm going to have to see this from the air for sure.

[00:19:51]>> Are we going to have to get the uh DTSB helicopter airborne? I think we're just going to rely on the DTSB drone. As we fly the drone forward, we can see that uh yeah, we have scouring that leads all the way up into the house. So, we know that the airplane um struck the ground early before it came to its final resting place over there at the house, scouring on the ground there. And then it hits right along here across the railroad tracks. Parts of the fuselage are there. And then we have uh the engine on the right, engine on the left, and then the whole rest of it towards the front. It just disintegrated as it went forward. We have two wings up there. Still parts of it, but very much blown out. Uh the only thing we do not have here at the site is the tail. So the tail might have separated somewhere else. We have found the tail over here at the uh at the lake, believe it or not. So, we had a complete separation of the tail section uh before it made its way over there. I mean, we're pretty far away from the crash site. So, that's not good. But, this is completely intact from what we can see. What else we got over here? We have any burn marks or anything on the sides? Looking down here, we do have some burn marks. It looks like possibly right here.

[00:21:15]Right in this section. Unless that's just gray.

[00:21:19]Uh, really hard to see. Oh, okay. Yeah, that tail section is not in great shape.

[00:21:24]I might have to get the the crane to pull this thing out. All right, I'm going to lower this thing down a little bit more. Try to get a hold of it.

[00:21:34]>> We got it. I'm going to bring it on up.

[00:21:36]Hold on. There we go. The best we can.

[00:21:39]Nice and easy. Nice and easy. Over to the side.

[00:21:43]That's good right there. All right, we have some possible burn marks happening here. I don't know on the inside. That's definitely burns.

[00:21:53]So, it looks like the tail separated.

[00:21:57]Maybe a fire in the rear of the airplane or in the rear belly is what I'm going to go with on that. So far, we have more damage over here by the power station.

[00:22:07]Okay, so yeah, this is the tail section.

[00:22:12]uh airplane was making its right turn over here towards the uh the runway and had a tail separation right here. That's what happened. But it looks like it was due to a fire towards the rear of the airplane from what I can tell. We have the tail section over here and then we just have this whole mess right here. So the tail separated back there. Airplane continued to fly, lost altitude rapidly. uh began to strike the ground. I think that first scour there is from the wing striking the ground. Uh that impact right here before the railroad tracks is from the impact of the one of the engines and the wing uh and then it just broke apart as it went forward. The wings fell forward with the uh force and then we had the wings separate to the right to the left and then it cart well the front part just kind of flew outward in the front there. We did not witness a lot of this, so it's really hard to say, but that's uh pretty much what we think happened.

[00:23:13]It was a fire from the rear uh maybe in the rear belly that separated the tail and the airplane lost all its ability to fly.

[00:23:24]>> Well, that's on point. That is exactly what happened. Uh there was uh a fire which uh caused a brief explosion in the back of the airplane. the tail uh came away and it began a left bank and dive and uh crashed exactly how you described it into the ground, disintegrated uh over here and basically came to rest.

[00:23:46]>> And the main reason why you know that is just because of how far away the tail section is from the rest of the airplane.

[00:23:52]>> Yep. It is a telltale sign.

[00:23:55]>> It had to it had to have been that way.

[00:23:57]So yeah, there it is. Well, I'm almost I'm getting to restoring the uh DTSB headquarters since my successful investigation of the uh plane crash from the other day. I've been upgraded from simply working as a volunteer cleaning the thing up to actually being an employee again. Uh it's amazing how uh I was let back so quickly. And oh, look at that. A little small plane taking off from the airport there. Well, anyways, let's continue to uh get this all cleaned up here. Just going to do some more little cleanup work. And as you can see, we are doing pretty good right now.

[00:24:37]It looks like we're going to get this thing back up to its former glory in no time. Oh, there he goes. Whoa.

[00:24:47]Something very interesting is h Oh, he's stalling. He's crashing. He's crashed.

[00:24:52]Oh my gosh. We got to rescue the pilot right now. You got to get out, sir. You got to get out. I'll break the window.

[00:25:15]>> Well, it looks like most of the airplane is actually intact here. It's going to make this uh investigation pretty easy.

[00:25:21]So, let's wait until the morning and uh see what we can get out of this. What happened here?

[00:25:26]>> Uh he was trying to go to the hospital, but uh he had a concussion.

[00:25:31]>> You told him to go to He's got to get to the hospital, so he got in the car. And >> that's okay. That's okay.

[00:25:38]>> That's okay. I'm okay with that. All right. The DTSB van is here. I have no way of getting the back doors open to get the tools out. So, uh this should do.

[00:25:49]>> There we go. It's the uh specialized uh door opening system. And as you can see, uh we forgot to put tools in here. That was a waste of time. Okay. Um great.

[00:26:02]>> Do you need the porter house? I'll go get the porter house.

[00:26:04]>> We actually just turned it into a uh truck.

[00:26:16]>> Well, this one shouldn't get too damaged.

[00:26:26]>> There are also no tools back here. What are we doing?

[00:26:30]That's okay, though. We'll just uh toss that over there. Anyways, let's take a look at this wreckage. The airplane is mostly intact, and we actually witnessed the crash, which helps. Um pilots in the hospital.

[00:26:45]I'm going to have to say this has got to be possibly an engine out situation.

[00:26:53]Uh we don't have a recorder on it cuz this is a an airplane that's doesn't need one. It's too small. Um let's see.

[00:27:02]Yeah, it's totally broken.

[00:27:06]Let's see if we can go find any other wreckage um around here. I'm going to go over to the airport. Just make sure we have the whole plane there. It looks like it does, but uh yeah, I'm not seeing too much anything out here. H, this is going to be an interesting one. I actually think that this airplane was losing engine power because you could hear it sputtering. You could see the plane trying to make a landing. It came around and as it was trying to climb out of a missed approach, it suddenly just began a wing stall to the right and landed flat. Right here, you can see it didn't really go much uh forward. It just landed flat. I think this is going to be my uh my final uh explanation for this crash is it had engine problems and stalled.

[00:27:58]>> That is 100% correct. So, the airplane was doing a missed approach procedure, IFR training flight. Uh, and then after the low approach, it started the climb, had an engine failure. Uh, the airplane stalled, and the pilots tried to get it away from the houses over here the best they could, and it worked out. We've stalled right into the street.
