Added:Hi, welcome back in.
How are you? I can see that we have an accident happen.
I had your file out. Let me just grab that now.
Has anything changed?
Okay.
All right. Now, let's get you a little cleaned up.
>> [snorts] >> All right. I just want to clean up around the areas of scarring and then then we'll figure out what happened.
Okay. [snorts] right above your eye and below.
I've heard some stories, so I'm excited to hear yours.
All right, it looks like it's all cleaned up for now. Now, what were you doing that caused this?
Okay. Well, so you were playing. Oh, you were watching.
No.
Interesting.
I mean, you have to be really careful with bedding, but I'm just your doctor. I'm not here to tell you what to do.
Now, typically I would send you off to a specialist to go get the cranial nerve exam, but since you are a patient of mine and I want to make sure that you're properly taken care of and I want to make sure that it gets done because I don't want to recommend you to somebody and then you not go.
So, I have everything set up to do a cranial nerve exam today. So, let's get that going for you today. Is that okay?
Okay, good.
So, tell me your symptoms of how you're feeling. Do you feel dizzy? Do you feel laded?
Okay. Now, does it hurt when I press around here?
Okay, hold on one second.
All right. So, does it hurt when I touch here.
Okay. What about Okay.
Okay. Well, we're going to run a series of tests that are going to go through many and all of your senses. We're going to go through your touch, taste, smell, hearing, and a couple other reflexes as well.
We're going to be testing multiple things. Don't worry, I have it all set up already.
The first thing that I'm going to be doing is testing your smell. I have some essential oils.
Don't be alarmed. They are going to be pretty common smells. So, just do the best that you can. You're not getting graded. We're just trying to figure out if there's anything that has happened during this altercation or if everything's normal.
I'm gonna assume probably normal, but let's make sure.
Hold on.
Let me wipe it under your nose.
You ready?
Good. Good. Orange.
I know. It's pretty orangey.
All right, this next smell is good. Peppermint.
See, you're doing already better than you thought.
All right. Okay. This next one.
Good. Cinnamon.
And this is the last one.
Are you ready?
Good. Pine.
[snorts] Yeah. All of those. Right. Just like I thought you would. [snorts] Now, we're going to do just a follow my finger. I just want you to keep your eyes on my nose. Yes. And I'm just going to bring my finger in close and you're just going to let me know when you see it and then when you don't while keeping your eyes face on my nose.
Okay. Okay. Good. All right. So, eyes on my nose.
Good.
Good, good, good, good, good, good, good, Good.
Okay. Good. Now, [snorts] I want to check your dilation and your eyes with my light.
Now, I have a few different lights that I'm going to be trying just to really make sure that I'm getting the correct diagnosis here.
take my gloves off and I'm going to need you to face your head forward. Staring at my nose again at all times.
I'm going to just pull the light in kind of like I did with my fingers. I want to check your pupils. So, I'm just going to be looking at your eyes at first and then I'm going to have you follow the light and do a few other series of tests. Okay? Don't worry, you'll be fine.
See the look.
Okay.
Okay, now I want to check from a little bit farther back.
again face on my nose. I'm just checking for dilation.
I'm going to do what we call a swing light test. And this is just going to be moving a light in a swinging motion. And again, I'm just trying to pay attention to see how your eyes focus and exactly.
So, just pay attention to my nose and we will get started.
Okay.
Well, next we are going to test your sense of touch.
Let me just get a few things.
Okay. So, I will have a sharp object.
Don't worry, it's just something that you can tell has a sharp point like a pen tip and then something soft.
And we're going to see if you can tell the difference between these light pokes and these little pricks. Okay. Okay.
So, first this is the brick.
This is the light touch.
You can feel the difference. Okay, good.
Now, close your eyes.
You don't have to, but it'll just help the effect. And I'm going to be coming in from sides, so you're not going to be able to tell which one I have. And I'm going to mix them up. Okay. So close your eyes.
Okay.
Good. Soft.
Good. Hard tip or sharp tip?
Good. Soft.
Soft. Good. Good.
Good.
Good. Yeah.
Sharp tip.
Good. Good. Okay. Couple more.
Good.
Was a trick. I did both.
both again. Okay. Well, it looks like you got all of these correct.
Next, we're going to move on to some muscle memory strength.
Yes. So, the first one we're going to do is going to be your eyes. I'm gonna first try to force close your eyes. I'm not going to be pressing like into your skull. I'm just gonna be trying to lightly close your eyelids.
And you're just going to try to keep them open. Simple as that.
Exactly. And then I'm going to do a few others. Okay. So, I'm going to get a little closer and try to keep them open.
Let me get some gloves on.
Okay. So, try to keep your eyes open. Try to keep them open.
Okay. Good.
Now, close your eyes. And I'm just going to try and pull them open lightly and just try to keep them closed.
Okay. Keep them close. Try to keep them close.
Try to keep them close. Try to keep them close. Try to keep them close.
Good. Good. Okay. Well, he did really good. There's a few other reflexes and muscles that I do want to check. I'm going to try to push your shoulders down. So, I'm going to need you to shrug your shoulders up and I'm just going to try and push them down. You're just going to try and keep them up and I vice versa going to try to pull them up and you're going to try to keep them where they're they are. Okay.
So shrug your shoulders up. Good. I'll try and push you down.
Good. Good.
All right. Again. Shoulders up.
Good. Okay. Now, keep them relaxed.
Just wiggle it out. And I'm going to try and lift your shoulders up. You're just going to try and keep them down when I try to lift a little. Okay.
Good.
Good.
Okay. Good. Good. Now I want to see how you respond to emotions. And exactly. I'm going to need you to What is smile? You know, show me your smile.
Okay. Can you frown?
Okay. Good.
Can you Can you pout stick your stick your lip out?
Okay. Good. Good.
Can you show anger?
Okay. Can you show excited?
Good.
Can you puff your cheeks?
Good. Good.
Now, I'm going to have you relax your jaw and I'm just going to try and shut your mouth. You're just going to try and keep me from shutting it. Okay.
Relax your jaw. Good.
Good. One more time.
Good.
Now, I'm going to have you shut your jaw and I'm just going to try to lightly open it and you're going to try and stop me. Okay.
Good.
Good.
Okay, good.
Reflexes and your muscles all seem to be working properly.
So, next we are going to be doing your taste test.
And I have multiple different tastes that we're going to just drop onto this little popsicle stick and give to you.
[snorts] Exactly.
Pretty common taste. Just like the smells were, just not going to be the same thing. Both the same flavors basically. So, let's get started.
Okay, you ready? Open up.
Good.
Now, what taste is that?
Good. Strawberry.
Okay, our next one.
Open up.
Now, what does that taste like?
Good. Good. Cinnamon.
See, I lied. So, they're going to be the same.
All right. Next one.
Okay. What flavor?
Blueberry. Good.
And this is our last one.
Open up. Good.
Coffee. Good. Yeah. I like to throw a little bitter in there to make sure we all of those different flavors and smells.
And next we are going to move on to those ears.
All right. So, next we are going to be moving on to your ears.
I just want to get out a few things to look at your ears. I'm going to check your left side first.
Okay.
Okay. Okay. Everything looks good. Good.
Good.
and let me know if you feel any discomfort. I'm going to check your right side.
Okay, good.
Looks normal.
Everything looks good.
I want to put my cap on to really get in your ears and Check deep in go back in.
Okay.
I see.
Okay. Okay.
Check the other side.
Okay.
Everything is checking out. [snorts] It looks good.
Now that we I got to check the inside of your ears, I want to just make sure that you can hear everything properly.
We're going to be doing a hearing test.
I'm going to whisper some words, phrases, numbers into your ear. Any one of those things, and you're just going to let me know. Repeat back to me.
Okay.
Now, water.
Water.
Water.
What I say?
Good. Good. Water.
21.
21. 21.
One number.
Good. 21.
Charlie went to the market. Charlie went to the market. Charlie went to the market.
What phrase? [snorts] Good. Charlie went to the market.
All right, let's check one more.
Let's go on a picnic. Let's go on a picnic. Let's go on a picnic.
Let's go on a picnic. Let's go on a picnic. Let's go on a picnic.
All right, we're phrases.
Let's go on a picnic. Okay, good.
Well, you did wonderfully on the hearing test and your ears looked good when I was checking them out.
Now I want to just do a couple sounds in your ears like clicking and hand sounds. Anything that you can describe or basically just tell me when you hear a sound. Exactly. So, I'm going to check your left side first and let me know when you hear something.
Good.
Okay. Good.
Good.
I'm going to check your right side.
Good.
Good.
Good.
Good.
Okay, let me get some gloves on. And I want to check your airways as well as your nose and your Exactly.
Okay. I'm just going to touch your nose, your neck, and a little bit around your temple. Okay.
Okay.
Good.
Feel your head temples.
Check your nose one more time.
Okay, good.
Okay. Now, I'm going to be doing a peripheral vision for your eyes with my fingers.
I'm just going to have you focus on my nose and let me know when my fingers leave sight. Okay? Then we'll go to the eye chart. All right?
So, keep your eyes on my nose and just let me know when it leaves.
Good.
Your sight.
Good.
Good.
Good. Now, a little bit closer.
Good.
Perfect. Eyes on my nose.
Good. A little bit farther back now.
Good.
Excellent.
All done. Okay, everything looks normal for your following my fingers and watching things in your peripherals.
We're going to go move ahead onto the eye chart. Okay.
Okay. So, now that we got you in a better position, let's get you tested with your eyes. Can you read line four for me?
Starting here.
Good.
Good.
Good. L. Let's go down to the green line. Line six.
Okay.
Start over here. Good.
Good.
Good.
Good.
And let's see if we can challenge you on the red line.
Line eight.
Okay.
Well done.
It's okay.
Well, you Yeah, you did excellent. Don't worry.
Now, I do typically like to with all my patients, especially since we had a a little bit of an injury.
I know you need to play a little bit less rough when betting on sports, but for now we will I want to get your temperature as well as your blood pressure and check your heart rate before we let you go. But everything else has checked out for your cranial exam. And now I just want to get the last few things. So, I will get your temperature if I can have you open up.
Uh, good.
And then I will get your blood pressure and we will check your heart rate. Mhm.
Okay. Good. 97.7.
Feeling good today.
Let's check that blood pressure.
Can I have you roll up your sleeve?
I will help you.
Okay, let me get it.
All right, let me get you hooked up.
That's no problem.
Anyway, let's Okay.
Okay. Good.
Let me just do that one more time just to double check everything.
Okay, good.
Get that off of you.

And I just want to jot everything down that I just recorded.
All right.
It was a pleasure getting to work with you today and I'm so glad that we were able to perform the cranial exam. Everything looks normal. Of course, I will be testing and double testing everything with my other doctors and making sure if we come back with anything, we will let you know. But you can check out with my lovely nurse and if you want to set up any other appointments, just let us know. It was no problem. You have a wonderful day.
