Fibromyalgia or Autistic Burnout? 10 Physical Signs You Might Have Both
Added:You found the checklist. Maybe you watched the video. 31 questions about autistic burnout, the sounds that hurt, the sleep that never restores, the way your entire body crashes after a day that barely happened, the muscle tension you've been carrying so long you've forgotten what it feels like not to hurt. And somewhere in that list, a thought crept in. This sounds exactly like my fibromyalgia. That thought is not a coincidence. That thought is a neurological fact that most of your doctors have never connected. I'm Dr. Michael Lenz, and today we're going through the 10 physical signs on that autistic burnout checklist, the ones that show up in your body, and I'm going to show you exactly what is happening in your central nervous system when each one occurs. Not because the two conditions are the same, but because in a person with fibromyalgia who may also be autistic, they are being driven by the same central nervous system in the same crisis. Keep a mental tally as we go. We'll come back to your number at the end. Before we dive in, please remember that this video is for educational purposes only. In today's video, we cover the 10 physical and sensory signs. Part two covers the cognitive and neurologic signs, and part three covers the emotional, identity, and systemic signs, and closes with a recovery framework.

Before we get to the signs, I I you to understand one thing, because everything else builds on it. Fibromyalgia is a disorder of central sensitization. The volume knob for your body's entire pain and sensory system has been turned all the way up. Signals that a healthy nervous system would filter out as background noise get amplified to genuine pain signals and distress signals at the level of your brain and central nervous system. Autistic burnout is also at its core a disorder of central sensitization. The same up-regulated nervous system, the same failure of sensory gating, your brain's natural ability to filter irrelevant input before it overwhelms you.
When the burnout checklist and your fibromyalgia symptom list look nearly identical, that is not a coincidence.
That is one nervous system at its absolute limit expressing that overload in every way it knows how.
The 10 physical signs. Sign one, sounds that normally don't bother you suddenly really hurt. The refrigerator hum, the air conditioner cycling, your partner chewing. Sounds that don't register for most people are sending you into headache territory or straight into shutdown. The clinical name for this is allodynia. In the past, researchers thought it was at the spinal cord, but the true story is in the brain. Think of your central nervous system as having a master fire alarm that it's been set to the most sensitive setting possible. It is now interpreting normal neutral stimuli like a refrigerator hum as a genuine high-stakes threat to your safety. Your brain is not malfunctioning, it's stuck in a state of high-alert threat detection. Having a fire alarm blaring in your head 24/7 when there is no actual fire is physically and mentally exhausting.
Sign two, no matter how much you rest, you feel completely exhausted. You sleep 9 hours and wake up more tired than when you went to bed. The blood work is normal, which helps us rule out other diseases, but doesn't explain the neurological drivers. In fibromyalgia, we see this alpha wave intrusion. Your brain wave pattern of relaxed wakefulness intrudes into deep restorative sleep. Your body is technically asleep, but your nervous system is essentially awake.
Appropriate physical repair does not happen at optimal levels. You wake up unrestored. This is not a willpower problem. This is biology.
Sign three, you can't fall asleep and you can't stay asleep. This is about initiation and maintenance. Instead of just looking at melatonin, we need to talk about what is often missed. Many neurodivergent people suffer with restless leg syndrome or periodic limb movement disorder and have completely normalized it as a quirky or insignificant trait. You may have lived with this for decades. Furthermore, we must screen for sleep apnea, which is significantly more common in ADHD populations due to a higher prevalence of tissue hypermobility in the throat as well as obesity prevalence rates that are higher. If you are struggling with sleep, you aren't just bad at sleeping.
You may have mechanical or neurological barrier that needs to be treated. Sign four, basic tasks that used to be automatic are now incredibly hard.
Showering, deciding what to eat, getting dressed. What is frequently misinterpreted by others as laziness or depression is neurologically a nervous system that has genuinely run out of the fuel those tasks require. In fibromyalgia, this is post-exertional malaise, a phenomenon where even minimal effort triggers a disproportionate cost. The brain has an energy accounting error that interprets that you don't have enough energy to go on, so puts you in the shutdown mode.
Sign five, your muscles hurt constantly and the headaches won't stop. Your nervous system has been running in sympathetic overdrive, fight, flight, or freeze, for so long that your muscles have forgotten how to release. People will feel muscle tension throughout their body. It feels like there's something wrong with the muscles, but what's actually happening is the brain's threat detection system makes the pain levels hypersensitive.
It feels like your muscles are under extreme tension. You feel like there's some kind of knot in your shoulders, but this is a threat detection system that's cranked up so high that normal muscle tissue is interpreted as being injured.
You're running two simultaneous loops, the fibromyalgia sensitization loop and the autistic burnout loop. The pain is not imaginary, it's in how your brain is processing information.
Sign six, you feel disconnected from your own body. This connects to interoception, your nervous system's ability to sense your internal state. In fibromyalgia, this is often dysregulated toward amplification. Everything is loud. In autism, it is often reduced awareness. Sensations don't register until they are extreme. In burnout, these two collide. The result is a floating quality, a nervous system oscillating between too much sensation and not enough, with no reliable signal about where your body actually is.
Sign seven, masking has become impossible. Chronic masking is not a psychological strategy, it is a physiological expenditure. It elevates cortisol and suppresses the parasympathetic state, the rest and repair mode that your body requires to recover. Masking is not just emotionally costly, it is physiologically destroying the conditions your body needs to heal.
When masking becomes impossible, that is your nervous system protecting you. It is not a failure, it is a biological alarm. Sign eight, you feel a lot more anxious than usual. In the ADHD brain, this is compounded by emotional hyper responsiveness and rejection sensitive dysphoria known as RSD. Your nervous system doesn't just experience the world, it processes it through a high grain filter. You are doing three to four times the cognitive work of a neurotypical person just to navigate, self-regulate, and process sensory input. This leads to profound exhaustion. The anxiety you feel is a direct consequence of a nervous system trying to process a high volume, high threat environment while managing emotional dysregulation.
Sign nine, a number of other health issues keep popping up. Your skin breaks out, your digestion is unpredictable, or you're dealing with chronic urticaria or eczema. These are tied to the neuroimmune and neuroskin connection. We see high rates of disorders of the gut-brain interaction in neurodivergent patients because the gut-brain axis is highly sensitive to the threat detection mode your nervous system is stuck in. We also look at what you eat, not just for calories, but for the biochemical impact. Certain foods can be pro-inflammatory or disrupt the production of serotonin and dopamine in the gut.
And certain foods can be anti-inflammatory and increase serotonin and dopamine.
These are foods that are whole plant foods. Your digestion and skin are reflecting the inner state of a system that is constantly being told it is under attack.
Sign 10, you feel like you're getting better one day only for the next to be a total disaster. You have a window of clarity, so you do some of what you've been unable to do, and then you crash harder than before. This is post-exertional malaise. A good day is not a sign that burnout is ending, it's a sign to rest harder, not to do more.
Using windows to build reserves instead of spending them is the most actionable insight in this series.
So, how many of those 10 did you recognize? If your number is five or higher, I want you to hold on to that.
What that number tells you is that fibromyalgia management alone may not be sufficient for the nervous system you are actually living in. I encourage you to take this list to your next appointment as a starting point for a conversation about your care plan.
This is a clinically meaningful difference between treating fibromyalgia in a neurotypical nervous system and treating it in an autistic one. The triggers are different. You need a clinician who understands both.
