[00:00:00]I have alopecia. So, I developed alipcia six years ago. I don't have any hair. I am 100% hairless. I don't have any hair on my head, eyelashes, or eyebrows.

[00:00:34]>> Um, I just was like researching it a little bit on TikTok and everything and they just said it comes with a lot of side effects.

[00:00:42]>> So, any medication, you know, we're going to weigh the the pros and cons of taking it, the benefits of treating the condition versus the potential for side effects. So, um, typically with these types of meds, uh, we're we're essentially, um, reducing immune system function. So, that way, you know, your body isn't attacking itself. Um, there's always a risk for infection and and other things. So, it's weighing pros and cons. We can definitely get more into the particular side effects for sure.

[00:01:14]>> Um, sometimes these types of medications do take, you know, several months to really see an effect. Um, and you know, you can see how you tolerate it.

[00:01:25]>> So, that's the that's that. What other questions do you have before we jump into it?

[00:01:28]>> Uh, that was it.

[00:01:30]>> Okay. So, if while we're chatting, you know, you do have a question, um, you know, that pops up, please feel free to interrupt and ask.

[00:01:37]>> Um, so, Luluo is the brand name of this medication. The active ingredient is called Ritly Nib. Um, we'll just call it Lip Fool because that's easier.

[00:01:47]>> Yeah. Um, so this is a once daily oral medication.

[00:01:52]Um, so it'll come from our pharmacy. Um, we can set up delivery for free if that's the easiest option for you.

[00:02:00]Otherwise, you can get it at any one of our four locations if there's a particular site that's closest to you.

[00:02:07]Um, or of course delivery. Um, so once daily medication, uh, you want to try and take it at the same time of day, the best you can within a couple of hours.

[00:02:17]Um, it does not matter if you take it with or without food. Um, food doesn't affect it, although some, you know, taking it with food could potentially reduce some GI upset. Um, although that's not super super common, but there is potential for that.

[00:02:33]>> Okay. Um, and then if you were to accidentally miss a dose, uh, just take it as soon as you remember. Um, but if it's, you know, more than, um, more than half the day has gone by, you know, like, let's say you take it at 8:00 a.m.

[00:02:48]every morning and it's now like 10:00 p.m. or something, that's fine. Just skip it and resume, you know, your normal schedule. um we just don't want to take two doses, you know, within 8 hours of each other, really um for the infiscus side of things. Now, um you do want to since it's a big capsule, you do want to swallow it whole. You don't want to open it up, crush it, chew it, or anything like that. And then you want to store it in the original container um you know, at room temperature. So, like most, you know, medication, oral medications.

[00:03:22]Any questions about that? No.

[00:03:25]>> No. Okay. Um, so in terms of side effects, I would say most commonly patients may experience headache, potentially diarrhea. Um, just staying hydrated to replete, you know, any fluid losses if that's the case. Um, now your body may adjust to the medication, you know, after some time and and those side effects may decrease. You can, of course, take over the counter like Tylenol if you have a headache. um or uh you know an antid-diarral if necessary.

[00:03:55]Um so those are kind of our more common not severe side effects. Of course I already mentioned infection. So if you do wind up getting you know like a cold or something like that um or like a wound that's not healing definitely reach out to your doctor. Sometimes we have to hold the medication um to let your immune system kind of like fight whatever infection. Um but definitely reach out and you know look for guidance for that. Um sometimes it can increase acne a little bit. Um some patients do get a rash. If that happens, please reach out to your doctor. And some patients do experience like some itching. Um just like generalized itching. So um you know definitely don't hold back of reaching out to your doctor if you experience these things and want some guidance. Um and then in very very very rare cases there is a risk for you know malignancy. So so cancers um but that's um but that's would be very rare.

[00:05:01]Um, so your doctor, since you already see the derm clinic, usually skin cancer is is one of the potential ones. Um, but just if you notice anything kind of feeling off, definitely reach out.

[00:05:13]>> Jeez, I'm thinking like, is it even worth it now?

[00:05:16]>> Um, you know, it's very rare. Uh, and it's totally up to you. Um, >> and you could try it months, see how it goes. if you are having side effects, you know, >> yeah, >> discontin or whatnot.

[00:05:33]>> Um, in terms Well, let me stop. Any questions?

[00:05:35]>> Nope.

[00:05:36]>> Okay. In terms of monitoring, so let me pull up your blood work really quick.

[00:05:42]Um, typically, and it looks like you have recent labs like from last month.

[00:05:47]So, usually we look at your complete blood count. So, that's looking at your white blood cells, red blood cells, all that sort of thing. Um, we'll do uh complet metabolic panel and we have yours from February. Um, looking at your liver function, kidney function among other things. Um, and so all that looks good. So, your doctor will likely want to get repeat labs in about a month or so. Let me see.

[00:06:16]Yeah. So, you have an appointment with them already and set up in about a month. So, let me see if they've ordered anything. They did. So, that's good to go. So once you start this and roughly a month before you have your next visit with them, you can get those labs drawn.

[00:06:31]Doesn't need to well actually they ordered a lipid panel which is not related to this drug but um that you do need to be fasting for. Um so that all looks good. Um let's see any like family planning questions beyond that. Okay.

[00:06:50]Um, what other questions do you have, you know, about the medication?

[00:06:55]>> Um, nothing. Nothing really, you know, just was concerned about the long-term effects, but like you said, I'm willing to try it for at least 3 months and see >> how how how I adjust to it.

[00:07:08]>> Yeah. And it could take a little bit longer than that, but um 3 to 6 months if you're able to tolerate it. If you do continue with the medication, you know, I will reach out to you in about 9 to 12 months to check in, see how you're doing. Um, and that we can do over the phone. You're always welcome to come in though. Um, the other thing is with vaccines, um, we want to avoid live vaccines, uh, since we are suppressing your immune system slightly. Um, most vaccines nowadays are not live vaccines.

[00:07:39]So ones that are, you know, the the nasal um flu vaccine is a live vaccine, so you would want to avoid that. If you want the flu shot, just get the regular like shot. Okay.

[00:07:49]>> Um but everything else um let me pull up your >> So I can still take my vitamin D and all that.

[00:07:55]>> Yeah, you can still take all that and we will review your meds in a in a moment so I can check for interactions. Um >> so it looks like Yeah. So uh you already have your your shingris done. Um next year your tetanus is due. Um but like u numaccoal uh which is for pneumonia flu shot that's those are all inactivated vaccines. You can get those.

[00:08:24]Now when we do suppress the immune system um our response to vaccines is not as robust. So, they're not quite as effective as, you know, if we're not suppressing the immune system, but it's still >> okay to get them um since they are inactivated. Just avoid that nasal flu.

[00:08:45]And again, most most places don't even offer it um from that standpoint.

[00:08:51]>> Now, I'm willing to try this for 30 days. Okay, I'll give it a shot. If I experience any side effects, even one, because there are quite a few of them, I'm immediately going to stop taking it.

[00:09:05]Plus, over the past six years, I haven't encountered any success stories about anyone growing their hair back from the medication until like maybe two months ago. There's even a doctor here on YouTube. He's a He's a hair loss specialist and he has alopecia and he has not been able to grow his own hair back, but he specializes in hair loss. So really, after I've been watching his videos for so many years and he has had zero success growing his hair back, I have made no effort at all to uh grow my own hair back. The fact of the matter is is that I lost my hair. I accepted my fate and I've adjusted quite well. Plus, I make wigs that emulate my own real hair.

[00:09:50]So much so that it makes me feel comfortable about wearing wigs. I don't feel like I look wiggy. That's important to me. I want them to look realistic.

[00:09:59]For those reasons, I've never sought treatment. The reason why I'm seeking treatment now is because microlade or nanoblade my eyebrows, and I am scared to death that someone's going to mess up my face. do not mess up my eyebrows. You know, because I've seen videos of creators that had alipcia that eyebrows microbladed and then when they started treatment, the micro blade was too far above where their natural eyebrows grew in. So, what I wanted to do before I got my eyebrows microladed, I wanted to take the medication. I wanted my eyebrows to grow in and then microlade over my natural eyebrow where my eyebrows naturally grow in. I do not want my eyebrows to be out of place when I get them microbladed. So, and by the way, out of nowhere, there's no explanation for this. In 2023, my hair started growing back. Don't know why. either again is still a mystery to me. My eyebrows grew back in fully. My eyelashes and my hair grew an inch. So, I will uh post a video about that. I've already recorded a video. I didn't turn the mic on. So, but I will ex tell you more about it.
