When you don't drink enough water, your cells lose water through osmosis because the extracellular fluid becomes saltier than the intracellular fluid, causing water to move out of cells through aquaporin channels; this leads to cell shrinkage, protein crowding, reduced ATP production, brain fog, and headaches, with thirst being a late warning sign that cells have already started losing water.
