When you stop CPR?
Added:Right now I am sitting in the emergency of the hospital and many times patients come to us with CPR in progress and when they come here we continue continuous CPR and check their pulse and if there is no pulse then we activate cold blue and as per SCLS protocol we start our resuscitation. But when should this resuscitation be stopped? Sometimes families become so emotional that they do not allow doctors to stop resuscitation. But at a certain point in time we have to stop resuscitation.
So after how many cycles can we generally stop resuscitation? We usually do three cycles. When the patient comes to us, we continue CPR and after 1 minute of CPR, we check whether there is electrical activity on the ECG monitor or not, whether there is a pulse or not. If both are negative, we give an adrenaline shot and continue CPR as per ACLS protocol.
This is a complete cycle. We give three such cycles and if the patient does not revive even after three cycles, then we declare him expired because it is very difficult for the patient to survive after the third cycle. Even if the patient survives after the third cycle, there is a risk of hypoxic brain injury. This means that due to prolonged CPR, oxygen may not have reached the brain. Due to this, brain tissues start dying and this is called hypoxic brain injury.
And we also know that this is a very emotional moment for the family. But after three cycles of CPR, the chances of morbidity and mortality of the patient become high.
