When MARPE treatment reaches a plateau where the device cannot be turned further without causing discomfort or tipping teeth, cortical puncturing may be necessary to weaken the dense palatal bone and allow continued expansion progress.

MARPE at 39 Update Day 19: Hitting a Wall at 14 Turns (No Split Yet)
Added:All right. So, hopefully you can hear me. Okay. Um, it's been about uh a little over a it's been about a week and a half since I gave my last update on the BP journey. Um, there hasn't really been that much to talk about, which is why I haven't recorded a video, but what I will say is uh I have not gotten a split uh yet. So, I got a couple of comments about um am I getting better breathing? Do I have braces yet? better breathing marginal because I don't yet have a split. So, when I turn the activation screw, I do get a sense of I'm having like better oxygen, but it's very limited because I just I'm not really expanding that much yet. Uh, so I'm probably about 14 turns in at this point. And that's actually where I've been since about Thursday of last week. Today is Tuesday.
I hit a wall. I hit an absolute wall at 14 turns and I just cannot turn any more than that without I feel like I feel uh it's tipping my teeth. So, I haven't turned beyond that. My orthodontist is aware. Uh we we're trying a couple uh troubleshooting steps which has been like I turn the screw back in the morning and then in the evening I turn the screw forward again.
But so far that hasn't produced any additional results. When I do turn the screw I do feel like I feel pulling in my front teeth. So I'm like that seems like a good sign. But all the same if I get up to 14 turns it just completely stops.
So, limited progress so far. I won't get braces until I have a split and until I uh I have a split for about a month is the current plan. So, as of right now, no split. I'm actually on my way to the orthodontist right now and we are going to be talking about what we can do if this strategy we're doing right now doesn't produce results. I suspect next on the menu will be cortico punctuing, which to my understanding is just an insertion of essentially a tad, the anchoring device into the uh pallet of my mouth in the areas of most resistance. So that would just weaken the bone enough hopefully where it will no longer be fighting me as much as it is. I suspect that will be the next move. Um, we talked about potentially a Paso cut as well.
I would like to avoid that for a number of reasons.
I wouldn't say anything's off the table as of yet. I just want to have the minimal amount of inter intervention that I can get away with.
So, yeah, just a quick update. I'll let you all know what's happening after when this wraps up.
Okay. So, uh I just got out of the appointment with my orthodontist and um as expected, we are going to schedule the cortical puncturing. I think I called it puncturing in the last video, which is fine. But the cortical puncturing is scheduled for Tuesday next week, which is exactly one week out from today. Um between now and then I am going to be doing a protocol of uh if we can we'll go back four turn or go back three turns and then go forward four turns uh every day. So I've been doing two and two. We were kind of just seeing how I responded to that and that was not going to really cut it. So, now we're going to go back three, go forward four, or at least as close to that as I can.
So, we'll we'll see. Good news, though.
Uh my ortho felt like we were able to cut off the um the teeth uh anchors today. So, I'm no longer anchored on my teeth. I just have the eight tabs now.
So, that is huge. That feels huge. And hopefully that leads to some more comfort for me as this whole process continues.
So yeah, the the puncturing is scheduled for Tuesday. Hopefully I don't need it.
We shall see.
So just a quick followup from the video I recorded in the car.
Uh, this is actually sometime in the future. I've been a little bit behind on updating you all with the videos. There's still been a lot going on. But, uh, long story short, I was not able to follow the protocol that my orthodontist had recommended for me.
I turned back two turns and I was at about about 14 maybe 15 turns. So let's say I was at 12 or 13.
And that night I was going to try to go forward three turns roughly based on the protocol that the orthodontist had prescribed me.
Unfortunately, the moment I started turning forward started activating again, I started getting a very sharp pain in the posterior of my pallet uh right at the screw sides, which I have two screws that are really tightly uh right into my uh pal Taurus. And maybe I'll throw a picture on the screen so you can see what I'm talking about. Uh things have actually developed since then. I can't really show you what my mouth looks like right now because it's in a different state than it was in this video at the time of this video, but I did neglect to film something showing the expressing that I wasn't able to do those turns. So, I just figured I would do something now and that will give some continuity uh to the future videos that I've recorded and I'll be releasing those shortly.
So essentially at this point in my treatment, I was just stuck waiting for the cortical puncturing and I had ceased turning entirely up until I I did end up receiving the cortical puncturing and I again I will share those videos shortly.
So yes, um thanks for tagging along so far. I hope these videos are helpful for people.
That is ultimately the goal is to help people who maybe don't feel like they have a good resource. For somebody in their late 30s like me who is male, traditionally not so much the target audience for this kind of treatment, but with the advances that we're seeing in this space, I think orthodontists are more and more willing to take on harder cases like mine. So, thank you for watching. I hope it's been helpful, and there will be more to come.
