Pelvic organ prolapse, affecting up to 50% of women who have given birth, occurs when pelvic floor muscles weaken or become injured, causing the bladder, uterus, or rectum to shift out of position and bulge into the vagina; this condition can result from muscle tension, laxity, or damage to supporting tendons and ligaments, and while intense activities like running or triathlon training may contribute if the pelvic floor hasn't been properly trained, it does not require surgery or cessation of exercise, as there are many treatment options available.

What That Dropping Feeling Can Mean
Added:I want to address this comment that I received. It's something that I hear very, very often and upwards of 50% of women who have given birth have some degree of what's called pelvic organ prolapse. That's where the bladder, the uterus, or the rectum can shift out of their proper anatomical position and bulge into or descend into the vagina.
Now, I can't say for sure that that's what this woman was experiencing, but that is a very typical sensation that people have. They feel this sudden drop or like they have something inside the vagina that wasn't there before. So, when the pelvic floor muscles lose their ability to support the organs for whatever reason, sometimes it's tension in the muscles, sometimes it's laxity in the muscles, sometimes it's injury to the support structures, the tendons, and the ligaments, there are many contributing factors to the development of pelvic organ prolapse. It doesn't mean that running or triathlon training is bad for the pelvic floor.
It just means that if people haven't been really made aware of their pelvic floor and they are doing intense activity, when the pelvic floor hasn't been trained to manage that activity, we could potentially experience something like pelvic organ prolapse.

It doesn't mean that you need surgery. It doesn't mean you need to stop running or jumping or lifting heavy weights. What it means is it's your pelvic floor asking for some help.
