Live Brugada VT mapping

[00:00:00]there's a little bit of a drop-down, but there's 3 mm ST elevation, which fulfills the criteria for a type 1 Brugada there and some terminal T wave inversion. So, we feel good about that.

[00:00:10]We might want to think about giving procainamide, which we've prepared for, which can exaggerate the abnormalities and give you a real, you know, J wave, very coved type, cuz this isn't maximally coved. One thing that we're looking at here on ice, um and why don't we go full ice, Chris?

[00:00:29]This is an image of the RV, and it's quite enlarged.

[00:00:34]It's globally a little bit hypokinetic, but as we get into the RVOT region here, you can see that the anterior portion has a little bit of aneurysmal dilatation. It could be my imagination, but there's a little bit of an outpouching there on that anterior RVOT.

[00:00:55]And when I was a fellow, people talked about overlap between ARVC and Brugada.

[00:01:01]There's been biopsies of these regions that show that there could be fibrosis or fibrofatty, but then you can get a Brugada manifestation. Could this be an overlap of this?

[00:01:09]But ARVC does not typically cause polymorphic VT and VF at night. So, I think the phenotype is still more consistent, but it's interesting to think about the structural abnormality that we're associated here.
