Y90 radioembolization (rad seg method) achieves complete pathonecrosis (CPN) in hepatocellular carcinoma by implanting radioactive seeds that remain in place, eliminating the drug metabolism phenomenon that causes liver decompensation in traditional TACE treatments, allowing safe delivery of high-dose radiation (400-600 gray) without added toxicity.

Added:So, whether this is bridging or curative, I'm trying to achieve CPN now.
We have great techniques now. Again, the rad seg method, other ablative methods, etc. Where we can get CPN. [music] And And my my job is to maximize that because I don't know the patient could have a problem. They could have developed comorbidities that that that prevent transplantation, etc. And I want to make sure that I've done the best job possible up front. And again, because we're able to give very effective therapies >> [music] >> without added toxicity has made my my job much easier. It's one thing if Oh man, I want to give my really high dose, but that means I'm going to have a lot of toxics. That that's not the case, right? Let me look at Look how how nice that that technique is is that Well, I can give you know, 400 gray, 500 gray, 600 gray and I'm not adding the toxicity. And so, I can bring that patient back to neil. I'm not going to say, "Oh gee, I'm going to He may decompensate based on what I'm doing."
That's a really nice place to be. Let's go back 20 years, you know, the taste era, you know, the other embolizations.
I mean, you know, there is a perception that if you do segmental taste, that patients do not decompensate. That's actually not true because you're giving them drug that has to be metabolized.
And so, there is a drug metabolism phenomenon that still happens with taste that does not happen with Y90. You're just implanting something that sits there. And so, that's been a huge improvement over 25 years.
